SUBJECT: Request for Current/Past Performance Information
Solicitation #12314422R0011
The United States Department of Agriculture (USDA) Procurement Operations Division (POD) will be evaluating proposals received in response to the subject solicitation for contractor to provide Enterprise Business Solutions. One of the integral considerations in the evaluation is the verification of the quality of the offeror’s past and present performance on contracts that reflect the offeror’s ability to perform on the proposed effort. We depend on information received from agencies/firms such as yours, which have had first-hand experience with the offeror, for the evaluation of their performance.
Our areas of interest in the offeror are summarized in the enclosed questionnaire.
You are urged to submit your comments on other similar contracts your entity has/had with the offeror. Any additional information that you think may benefit the Government in our evaluation of the offeror's proposal is appreciated. Please note that we may contact the offeror on problem areas or concerns identified, to determine whether corrective actions have been taken or are in process. The offeror is required by our solicitation to provide a point of contact for each reference provided to the Government. The name of the individual completing the questionnaire will not be disclosed to the offeror.

Evaluation Ratings Definitions Rating Definition
Exceptional Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being evaluated was accomplished with some minor problems contractor was effective.
Satisfactory Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems contractor appear or were satisfactory.

Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
This Past Performance Questionnaire will be used to assess the QUALITY of a vendor’s performance history. Assessments should reflect only contract performance for which the Contractor is responsible. If an unsatisfactory rating is given, please provide more details in the comments section.
The successful awardee will be responsible for USDA Enterprise Business Solutions. The following questions are specific to this area of interest and are aimed at assessing the quality of performance history with respect to Software Licensing for approximately 123,000 users.
