INSTRUCTIONS: The offeror’s technical questionnaire is limited to ten (10) pages. For each factor below, in the first table provide a detailed description of specific tasks performed for that factor. Use the reference table to provide up to five (5) references per factor to support the technical experience claimed in the first table.
If your company is not performing ALL the work with in–house personnel, you must explain the contemplated teaming arrangement in Attachment 4, Teaming Arrangement Questionnaire. This includes, but is not limited to, the following teaming relationships: prime/sub-contractor, mentor/protégé, joint venture, etc. Provide the name of each teaming partner, the percentage of the cost of contract performance for personnel (labor) costs for each teaming partner, and a description of the work / effort to be performed by each teaming partner. If a teaming arrangement is not applicable and ALL the work will be performed by your company as the prime contractor, please indicate N/A as your response in the Teaming Arrangements Questionnaire.
Factor 1. Three (3) years of experience providing hot line pressurized insulator washing tasks within the last five (5) years preceding the release of this request for proposal. Provide detailed information in performing each of the specific pressurized washing tasks described in factors 1a through 1c inclusive of RFQ Attachment 1, Performance Work Statement. Provide reference and contact information to support the experience claimed by offeror.
To Be Considered Acceptable: The contractor’s responses evidence that the contractor has three (3) years of experience performing each of the specific maintenance tasks described in sub-factors 1a through 1c within the last five (5) years preceding the release of this request for proposal. References provided must support the experience claimed by offeror.
Sub-Factor 1a: At least three (3) years of experience performing the Hot line pressurized washing of Insulators within the last five (5) years preceding the release of this request for proposal. Use the narrative block in the first table to provide a detailed description tasks performed (see PWS section 1.3 – 1.3.3).
