About this file
This document is a HIPAA Business Associate Agreement (BAA) between the Regents of the University of Colorado Denver | Anschutz Medical Campus and an unspecified vendor. The agreement is related to a Faculty Data Management Project Phase 1, where the CU School of Medicine seeks to replace its existing annual review platform (PRiSM) and faculty database (FIMS) with a modernized, centralized data management system for faculty lifecycle processes. The project aims to consolidate and update the current systems used for tracking and managing faculty information.
The BAA outlines comprehensive terms for handling Protected Health Information (PHI), including strict guidelines for use, disclosure, safeguarding, and reporting of sensitive data. The agreement mandates specific security protocols, such as encryption, minimum necessary data access, breach notification procedures, and requirements for subcontractors. Key obligations include maintaining confidentiality, preventing unauthorized disclosures, conducting annual independent security assessments, and providing immediate notification of any potential data breaches. The document emphasizes the vendor's responsibility to protect PHI and comply with HIPAA rules, with provisions for potential termination if security standards are not met.
