About this file
This document is a set of three required affidavits for bidders, offerors, and contractors participating in Kentucky state government contracts, issued by the Commonwealth of Kentucky. The affidavits include: 1) a general campaign finance compliance attestation, 2) a resident bidder status declaration, and 3) a qualified bidder status declaration. Each affidavit requires the bidding entity to swear under penalty of perjury that they meet specific legal requirements, such as being authorized to transact business in Kentucky, having filed state corporate taxes, made unemployment insurance payments, maintained workers' compensation policies, and potentially qualifying for special bidder classifications.
The affidavits are standardized forms that must be completed, signed, and notarized for contract consideration, with the Commonwealth reserving the right to request supporting documentation and potentially disqualify bidders who cannot substantiate their claims. The forms are dated September 16, 2022, and are valid for one year from execution, covering various legal compliance aspects including campaign finance laws, business registration, tax payments, and special status for nonprofit organizations or agencies serving individuals with severe disabilities. Each affidavit provides specific checkboxes and sections for bidders to declare their particular status and compliance with Kentucky Revised Statutes (KRS).
