Attachment B - Specifications Referral Lab Testing Eastern Louisiana Mental Health System (ELMHS) RFx #3000024861

LABORATORY TESTING SERVICES, AS NEEDED FOR EASTERN LOUISIANA MENTAL HEALTH SYSTEM (ELMHS) TO INCLUDE:

SPECIMEN TRANSPORT:

Specimens shall not be processed in any manner of transport other than that necessary to preserve specimens.

EQUIPMENT & SUPPLIES:

The Vendor shall provide all equipment, software, supplies, forms, etc., necessary for the collection, labeling, and transporting of lab specimens for the period of the contract.

The Vendor is to provide an adequate supply of urine cups with sealing lids that allow for identification labels. Urine tubes and a suction needle are also to be supplied for the transfer of urine to the tubes.

The Vendor will supply ELMHS with a computer capable of receiving test results through a secure, direct connection to the Vendor’s system. THIS SECURE CONNECTION SHALL BE INDEPENDENT OF ELMHS’ COMPUTER NETWORK. While online access to lab services is expected, the Vendor-provided computer must also be capable of serving as an alternative backup service onsite with daily backups, onsite data storage, and the ability to order and receive lab results.

The Agency Requires the Following Capabilities:

· Have the ability to access ELMHS Lab results and summaries from the Vendor’s server (database)

· Ability to access results by trends or other methods used by the Vendor to obtain a summary of reports (such as a range of cholesterol levels, all abnormal Hep C results, Rapid Plasma Reagin, Quantiferon, Glycated Hemoglobin, etc.)

· Ability to access labs that are reported to the State of Louisiana Office of Public Health or any other entity that would be notified

· Ability to research labs on a client at ELMHS that were performed out of the facility prior to admission

· Ability to access Technical Support All replacement supplies requested by ELMHS are to be delivered within 48 hours of request.

PHLEBOTOMIST SERVICES:

The Vendor shall be able to provide phlebotomy service, as requested, if the need arises. All phlebotomists to provide service to ELMHS shall have a Louisiana phlebotomy certification and shall be able to provide a copy of their certification upon request.

PICK-UP REQUIREMENTS:

Monday through Friday, the Vendor shall provide Morning pick up of lab specimens from 9:00 A.M. to 10:00 A.M. (CT) and the afternoon pick up of lab specimens from 1:30 P.M. to 2:30 P.M. (CT), at the two ELMHS locations listed below:

New Orleans, LA 70119 The Vendor shall call the ELMHS Laboratory for any delays in pick-up.

The Vendor shall make arrangements to pick up and process Short Turnaround Time (STAT) requests.

REPORTING OF TEST RESULTS:

The Vendor must contact ELMHS by telephone upon completion of short turnaround time (STAT) requests, within 4 hours of receipt of the test.

The Vendor must contact ELMHS by telephone within 4 hours with all critical lab values as determined by ELMHS.

All other reports are to be furnished to ELMHS in a timely manner, not to exceed 24 hours.

Turnaround time (TAT) for all line items should be 24 hours for preliminary results.

Routine tests are typically reported within 24 hours of receipt of the specimen by the testing facility. Significant anomalies may require more time.

Esoteric tests that cannot be processed at a regional laboratory or a regional laboratory affiliated with the Vendor shall be performed by a State Laboratory or by the Centers for Disease Control and Prevention (CDC) in accordance with Federal law. The Vendor shall bill ELMHS for all esoteric laboratory services performed.

Lab results shall be made available to the ELMHS computer furnished by the Vendor with secure online viewing within 24 hours, and the Vendor shall fax when necessary or requested.

CONTRACTOR LICENSING REQUIREMENTS:

The Laboratory shall be licensed to perform medical laboratory analysis under the provisions of the Clinical Laboratories Improvement Act (CLIA) of 1967 (42 U.S.C. 263A). Laboratories for the Vendor must be currently licensed, and maintain license throughout the contract period.

The Laboratory must have an accreditation certificate from the College of American Pathologists (CAP). The Laboratory must have been accredited for at least the previous 10 years and must maintain accreditation throughout the contract period.

The Vendor shall also be licensed with a Board Certified (Anatomic and Clinical) Pathologist to review all results and be available for consultation on laboratory results or clinical problems.

The Office of State Procurement must be notified of any changes in licenses and/or accreditation.

Pricing - The Vendor’s prices indicated on pricing sheets are all inclusive, including, but not limited to, the following:

1. All necessary equipment, supplies, forms, etc., to perform tests.

2. Collection of specimens.

3. Transmission of lab results.

4. Transportation of all collected specimens to the Vendor’s laboratory for analysis.

Vendor Responsibilities:

1. Provide all necessary equipment.

2. Provide necessary lab slips.

3. Lab results transmitted to each ELMHS Facility within 24 hours or sooner in the event of a finding of Critical Results and Values. Critical Results and Values shall be laboratory results that indicate the presence of a life-threatening condition that may be corrected by appropriate and timely intervention. At ELMHS, Critical Results and Values shall be the Panic values determined by the Vendor as agreed upon by ELMHS and the Vendor.

4. Maintain all required certifications/accreditations listed within and meet all Federal and State licensing laws.

5. Process each client according to the referring physician’s orders.

6. The Vendor will be responsible for billing Medicare patients and legacy Medicaid patients that are not covered by Magellan or one of the Managed Care Organizations (MCOs).

7. The Vendor will be responsible for billing the client’s insurance.

8. Keep copies of all orders issued against the contract and provide usage reports as requested.

9. Provide test results including the following information:

Patient Name Agency Identification Number Patient Date of Birth and Age Sex of Patient Name of the Referring Physician Time and Date of Specimen Collection Date the Results are reported Test Results Reference (Normal) Range

10. The Vendor is to submit an itemized invoice to the clinic on a monthly basis for all services.

11. Pricing must be firm throughout the contract period.

12. The Vendor must be able to perform other miscellaneous tests not listed. The same terms and conditions agreed to will apply to any and all tests offered by the Vendor throughout the term of the contract.

13. At the option of the State of Louisiana, line items may be deleted or added, if deemed to be in the best interest of the State.

14. The Vendor is to provide proper disposal of all bio-hazardous products generated in the performance of contracted services

OUTSIDE TESTING:

In the event that the tests are not performed in the Vendor’s laboratory and are sent to an outside lab(s), there will be no additional referral charge. The certification and licensing requirements for the outside lab(s) are the same as those specified for the contract laboratory.

BILLING:

The Vendor shall submit an itemized invoice to ELMHS on a monthly basis for all contracted services, providing client name and individual laboratory procedure with the cost of each test performed.

The Vendor shall be responsible for billing ELMHS, the patient, or a third party as appropriate. All billing information, including payable diagnosis codes, will be provided to the contracted Vendor for third-party billing.

The Vendor shall bill ELMHS for any laboratory service performed for ELMHS facilities at locations other than the Vendor’s laboratory.

The Agency shall be responsible for verifying that invoices are accurate.

LABORATORY SERVICES PERFORMED FOR TESTS NOT LISTED SHALL NOT BE BILLED HIGHER THAN THE CURRENT MEDICAID RATE.

TRAINING:

The Vendor will provide periodic in-service training sessions with qualified personnel to update ELMHS FACILITIES personnel on test ordering procedures, specimen collection, etc., associated with the contract.
