1. BACKGROUND:

1.1. Main Telecom line: Existing telephone lines are copper analog cable. This runs from the provider box to the existing building of the Herrmann House (admin offices) and the Robeson House (maintenance). There is currently no analog, copper lines or fiber running to or from the Visitor Center. Buildings are connected through a Point-to- Point System for data using a Wi-Fi System from local provider.

1.2. Current infrastructure includes copper cable line running from telecom phone termination boxes on highway to the administration and maintenance buildings. This is only analog copper lines. No copper or fiber is running between the buildings with good connections. All connections at the site are reliant on Point-to-Point Antenna system for data. The analog lines are used primarily for phone service and alarm system as there is no VoIP system at the park. This leaves the Visitor Center with no wired connections and creates safety and communication concerns for visitors and employee. The Point-to- Point system is unstable due to weather and equipment failures with connections failing often requiring reboots and equipment resets.

2. OBJECTIVES:

This contract is installing fiber between the existing buildings from the Herrmann House (administration) to Visitor Center and from Herrmann House (administration) to the

Robeson House (maintenance). The product will provide better telecommunication, computer systems and safety advantages for all the buildings located at Minidoka National Historic Site.

3. PLACE OF PERFORMANCE:

4. PERIOD OF PERFORMANCE:

All tasks and deliverables for this contract must be completed within 90 days after Notice to proceed. Days listed herein refer to calendar days. The project cannot be performed from July 1st to the 15th due to park ceremony.

5. SCOPE:

The project consists of running 24 strand Single Mode Fiber Optic Cable through conduit from the Herrmann House (Administration) to the Visitor Center terminating into the wire room. Another 24 strand Single Mode Fiber Optic Cable through conduit from the Herrmann House (administration) to the Robeson House (maintenance) terminating into the building (approximately 4000ft). The new 24 Fiber Optic Cable will be run in one UD(1X2-1) conduits (approximately 3200ft).

6. TASKS:

This section includes all labor, materials, tools, and other equipment, services and supervision required to complete the fiber optic cable installation work as indicated. The contractors work sequence shall include, but is not limited to:

• Responsible compliance and right of way access outside the park boundaries will be done before work will be started. All work should be inside the boundaries.

• Conduit/Fiber will be laid from Herrmann House (administration) to the Visitor Center, along existing park trail. Minimal disturbance of ground and responsible for maintaining the trail integrity. This includes any gravel, dirt or moving of existing trail contents.

• Conduit/Fiber will be laid from Herrmann House (administration) to the Robeson House (maintenance). Conduit/Fiber will run along existing roadway. Will meet the Robeson House driveway and move along roadway to terminate into the building. This will need several pull boxes installed with the distance. Minimal disturbance of ground and responsible for maintaining the roadway integrity. This includes any gravel, dirt or moving of existing contents

• Conduit will be buried maximum of 18” deep. Excess dirt will be worked back down into the trench.

• Fiber will be terminated on both ends of the install. Fiber termination boxes will be required and installed in the buildings.

• Contractor will need to work closely with Archeologist/park staff while laying conduit/fiber to maintain less amount of digging and be aware of archeology findings.

• During all phases of construction involving roadways, contractor will be required to ensure the public safety by implementing and utilizing proper traffic controls.

7. EXECUTION:

CONTRACTOR TASKS

A. Prior to beginning work, coordinate work schedule with the Contracting Officers

Representative (COR) and no physical work shall commence at the site until a notice to proceed is issued by the Contracting Officer.

B. Locate, identify, and prevent damage to known utilities. If damage occurs, contractor shall repair utility at no additional expense to the government. Contractor is to be liable for damage to any other infrastructure within the work area while work is being performed on the project.

8. ACCIDENT PREVENTION:

i. Accident Prevention Plan (APP): Prior to beginning work contractor shall submit for approval a site-specific APP. The Contracting Officer will review the proposed program for compliance with OSHA and project requirements. If the program requires any revisions or corrections, the Contractor shall resubmit the program within 10 days.

The Contracting Officer will not issue the Notice to Proceed until the APP is approved.

The plan shall include:

1. Name of responsible supervisor to carry out the program.

2. Weekly and monthly safety meetings.

3. First aid procedures.

4. Description of each phase of the work, the hazards associated with each major phase, and the methods proposed to ensure property protection and safety of the public, National Park Service personnel, and Contractor's employees.

5. Training, both initial and continuing.

6. Planning for possible emergency situations, such as earthquakes, floods, windstorms, fires, cave-ins, slides, explosions, and power outages. Such planning shall take into consideration the nature of construction, site conditions, and degree of exposure of persons and property.

7. Waste control and potential hazardous materials precautions.

8. Personal protective equipment.

9. Hauling routes – coordinate with Contracting Officer.

ii. Ensure first aid is available and training adequate for the number of employees and the type of construction at the site.

iii. Report all accidents to the COR immediately. A reportable accident is defined as death, occupational disease, traumatic injury to an employee or the public, property damage more than $100, and fires. Form DI-134 must be completed and submitted to the COR within three days of an accident. Forms are available from the COR.

iv. Provide personal protective equipment that meets the requirements of OSHA, NIOSH and MSHA, Material Safety Data information where applicable, as well as ANSI.

v. Inspect personal protective equipment daily and maintain in a serviceable condition.

Clean, sanitize, and repair, as appropriate, personal items before issuing them to another individual. Inspect and maintain other protective equipment and devices before use and on a periodic basis to ensure safe operation.

vi. Provide all necessary traffic barriers, traffic control signs, markers, and flaggers when required during operations. Traffic control for occupied work areas shall be in accordance with National Standards contained in the Manual of Uniform Traffic Control Devices (MUTCD).

vii. Post specific reporting instructions for contacting Emergency Response Personnel in conspicuous locations at the work site.

viii. Identify materials considered to be “Hazardous Materials” by industry standards and provide protection and security for identified materials throughout contract period.

Hazardous materials spills shall be reported immediately and clean up shall be the sole responsibility of contractor.

ix. As a minimum, conduct weekly 15 minute "toolbox" safety meetings. These meetings shall be conducted by a foreman and attended by all personnel at the worksite. Conduct monthly safety meetings for all levels of supervision. Notify the COR so that they may attend. These meetings shall be used to review the effectiveness of the Contractor effort, to resolve current health and safety problems, to provide a forum for planning safe work activities, and for updating the accident prevention program. The COR will enter the results of the meetings into their daily log.

x. The Contractor shall assume full responsibility and liability for compliance with all applicable regulations pertaining to the health and safety of personnel and environmental management during the execution of work and shall hold the Government harmless for any action on his/her part or that of his/her employees or subcontractors, which results in illness, injury or death.

9. QUALITY ASSURANCE REQUIREMENT:

The government will periodically evaluate the contractor’s performance in accordance with the statement of work.

While performing work, the contractor shall comply with the latest applicable versions of the following:

• National Environmental Balancing Bureau (NEBB)

• Associated Air Balance Council (AABC)

• NFPA 101 (National Fire Protection Association's Life Safety Code)

• Local codes only if required by utility companies for service connections.

• National Electrical Code (NEC)

• Occupational Safety and Health Administration (OSHA) requirements

Additional Requirements:

• All affected areas must be restored to their original condition from before the work began. Any damages will be fixed and repaired at no cost to the government.

• Any unused conduit, or other misc. equipment must be removed from the worksite by the

10. SUBMITTALS:

Submittal Documents: The following documents shall be submitted no later than 14 calendar days after contract award and a minimum of seven (7) calendar days prior to the Preconstruction Meeting. If all of these documents have not been received prior to the scheduled Preconstruction Meeting date, the meeting will be cancelled and the Notice to Proceed (NTP) will not be issued. Work shall not commence until written Notice to Proceed (NTP) has been issued.

A. Letter designating Project Superintendent.

B. Accident Prevention Plan (APP) – Submit a “site specific” accident prevention plan with the following information (at a minimum). Refer also to PART 5 Safety Requirements.

a. Name of the supervisor responsible for executing the plan.

b. Emergency contact phone numbers.

c. Map of local area emergency medical facilities relative to the jobsite.

d. First aid and emergency medical procedures / forms.

e. Company safety plan / protocol.

f. Job hazard analysis (JHA) completed for each major work activity.

C. A list of Subcontractors for this project.

D. Written statements from subcontractors certifying compliance with applicable labor standard clauses.

E. Satisfactory evidence of liability insurance coverage and workman’s compensation for the Contactor and all subcontractors.

Submittal Documents: The following documents shall be submitted to the Contracting Officer prior to construction mobilization. If these documents have not been received and approved by the NPS prior to construction start, the construction start date may be delayed.

1. Product Data – Submit product cut sheets / material data on all products to be used for approval. Indicate the size, model, or type of each recommended product.

11. PRECONSTRUCTION AND PROGRESS MEETINGS

A. Preconstruction Meeting: Before the start of construction, the Contracting Officer will arrange for a preconstruction meeting either on site or via teleconference. The meeting agenda will include the following topics:

B. REQUESTS FOR INFORMATION (RFIs)

A. General: On discovery of the need for additional information or interpretation of the Contract Documents, the Contractor shall prepare and submit an RFI utilizing the NPS form.

1. Contracting Officer will only respond to RFIs submitted by the Contractor.

2. Coordinate and submit RFIs in a prompt manner to avoid delays in the work.

B. RFI Form: Include a detailed, legible description of the subject or item needing information or interpretation on the RFI Form with:

1. RFI number, numbered sequentially.

2. Date.

3. RFI subject.

4. Specification Section number and title and related paragraphs, if applicable.

5. Drawing number and detail references, if applicable.

6. Photographs or sketches, if applicable.

7. Contractor's suggested resolution. If the Contractor's suggested resolution impacts the contract time or the contract sum, Contractor shall state the impact in the

RFI.

C. Response to the RFI: The Contracting Officer will review each RFI, determine the action required, and respond accordingly.

12. SUBMITTAL PROCEDURES

A. CM-16 Transmittal Form: All submittals shall be transmitted using NPS CM-16 form sent via email.. No action will be taken on a submittal item unless accompanied by the CM-16 transmittal form. Complete all applicable fields, sign the form, and attach all supporting documentation.

B. General Submittal Procedures:

1. Processing Time: Allow 7 days for NPS review of a submittal. When review is complete, an e-mail notification will be sent to the Contractor. No extension of contract time will be authorized because of failure to transmit submittals in a timely manner.

2. Approved Equals: For each item proposed as an “approved equal” submit supporting data including:

a. Comparison of the salient characteristics of the proposed item with that specified.

b. Name, address, and telephone number of vendor.

c. Manufacturer’s literature.

d. Compliance with the Buy American Act.

3. Electronic Submittals: Transmit all submittals except physical samples in PDF format.

4. Physical samples: Deliver physical samples to COR (or onsite POC).

13. CONTRACT CLOSEOUT PROCEDURES:

• Prior to requesting a final inspection, contractor will provide all information and diagrams of where the 24 Fiber Optic Cable is placed and terminated between the Herrmann House (administration) to the Visitor Center and the Herrmann House (administration) to the Robeson House (maintenance).

FINAL INSPECTION SUBSTANTIAL COMPLETION AND ACCEPTANCE

PROCEDURES

A. Request a final inspection in writing when a project or designated portion of a project is substantially complete. The Contracting Officer will proceed with the inspection within 10 days of receipt of the written request or will advise the Contractor of items that prevent the project from being substantially complete.

B. If the work is determined to be substantially complete, following the final inspection.

Contracting Officer will prepare a Punch List and issue a Letter of Substantial Completion.

C. If the work is not determined to be substantially complete following the final inspection, Contracting Officer will notify Contractor in writing. Contractor shall request a new final inspection after completing the work. Re-inspection costs may be charged against the Contractor in accordance with the Inspection of Construction contract clause.

D. Contractor shall complete the Punch List within 15 calendar days, documented weather permitting prior to requesting final inspection.

E. If Contractor completes all items of work on the Punch List and all contractually required items, Contracting Officer will issue Letter of final acceptance of work.

F. If the Contractor fails to complete the work within the time frame, the Contracting Officer may correct the work with an appropriate reduction in contract price or charge for re-inspection costs in accordance with the Inspection of Construction contract clause.
