RFQ: 36C24820Q1398
(i) In accordance with FAR Part 13, this combined synopsis/solicitation for commercial items is prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Department of Veterans Affairs, North Florida / South Georgia Veterans Health System located at Malcom Randall VA Medical Center in Gainesville, FL has a need to purchase Eight (8) Automated Chest Compression Systems with accessories.
(ii) The combined synopsis solicitation number is 36C24820Q1398. It’s issued as a Request for Quotation (RFQ).
(iii) This combined synopsis solicitation RFQ and the provisions and clauses incorporated are those in effect through Federal Acquisition Circular 2020-08 (08-13-2020).
(iv) The combined synopsis solicitation is 100% Set-Aside for Service-Disabled Veteran Owned Small Businesses (SDVOSB). The NAICS is 339112. Business size standard is 1,000 Employees.
Note, Interested SDVOSB/VOSB offerors must be listed and registered in Vendor Information Portal (Vip.Vetbiz.gov) to be considered for Award.
(v) This combined synopsis solicitation is for the purchase of Eight (8) Automated Chest Compression Systems and accessories. (See Cost and Price Schedule for specif ics). Responses shall be on an all or none bases, no partial submissions will be accepted. The Government reserves the right to make no award at all.
(vi) A description of the items to be acquired, and/or applicable Government need(s) as follows:

Addendum to FAR 52.212-1 Instructions to Offerors – Commercial Items: Offerors are to e-mail complete quotes to William.long6@va.gov no later than 2:00PM EST on August 21st, 2020. Addendum Provision 52.212-2, Evaluation – Commercial Items (OCT 2014), applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price considered. Government will conduct comparative evaluations of all quotes and award to the vendor which represents the best value based on technical, delivery (on or before 01 October 2020), and price. Once the government determines that there is a vendor that is the best suited to meet the requirements of this RFQ (i.e. the apparent successful contractor); the government reserves the right to communicate with only that vendor to address any remaining issues.

Note, unless exempt per Federal Acquisition Regulation [FAR] Part 4.11, all contractors must be registered and have an active registration in the System for Award Management (SAM) to be considered for an award against this solicitation. SAM combines data that was formerly contained in the Central Contractors Registration (CCR) and Online Representation and Certif ications (ORCA) systems. 52.225-18 Place of Manufacture (MAR 2015).
(a) Definitions. As used in this clause— “Manufactured end product” means any end product in product and service codes (PSCs) 1000-
9999, except—
(1) FPSC 5510, Lumber and Related Basic Wood Materials;
(2) Product or Service Group (PSG) 87, Agricultural Supplies;
(3) PSG 88, Live Animals;
(4) PSG 89, Subsistence;
(5) PSC 9410, Crude Grades of Plant Materials;
(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;
(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;
(8) PSC 9610, Ores;
(9) PSC 9620, Minerals, Natural and Synthetic; and
(10) PSC 9630, Additive Metal Materials.
“Place of manufacture” means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the f inished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.

(b) For statistical purposes only, the offeror shall indicate whether the place of manufacture of the end products it expects to provide in response to this solicitation is predominantly—
