About this file

This is a Contractor's Notice of Fees attachment for a state contract (STATE 0000000178SL) that establishes fee structures for payment card services. The document outlines fee requirements for two distinct card types: Department-issued non-reloadable cards and cards converted to personal reloadable cards. The notice requires that contractors clearly distinguish and separately identify all fees charged to each card type in their fee submissions.

The fee structure includes charges for specific transaction types and services. For ATM balance inquiries, non-reloadable cards carry no fee while personal reloadable cards charge $1.25 per inquiry. Inactivity fees are assessed at $0.10 for non-reloadable cards and $1.00 for personal reloadable cards. Replacement card fees are free for non-reloadable cards but cost $10.00 for personal reloadable cards. The sample table provided establishes the baseline format and categories that contractors must use when submitting their fee schedules, ensuring consistency and transparency in fee presentation across all bidders.
