In accordance with Washington Administrative Code (WAC) 296-62-07721, employers shall communicate the asbestos hazard to:
1) Employees
a. Include asbestos in the hazard communication program established to comply with the Hazard Communication Standard (HCS), WAC 296-901-140.
b. Employers shall ensure that each employee has access to labels on containers of asbestos and to safety data sheets, and is trained in accordance with the requirements of HCS and WAC 296-62-07722.
c. Housekeeping personnel must be informed of the presence and location of asbestos containing materials that may be disturbed during housekeeping and maintenance activities.
2) Contractors
a. Provide a copy of this asbestos report to any contractor conducting renovation or demolition activities within the facility.
3) Subsequent Owners
a. Provide a copy of this asbestos report to any future owner of the building.
All asbestos containing materials must be labeled in accordance with the requirements WAC 296-62-07721 (5). WAC 296-62-07721 (6) requires that the employer annually provide, at no cost to employees who perform housekeeping operations in a facility which contains ACM or Presumed ACM (PACM), an asbestos awareness training course to all employees who are or will work in areas where ACM and/or PACM is present who work in buildings containing asbestos containing materials.
The ACM identified in this survey was in generally good condition at the time of the survey.
The ACM should be monitored semi-annually for signs of deterioration or damage. Any ACM that are identified in poor condition (or that will be impacted by renovation or demolition activities) must be abated by a Washington licensed asbestos abatement contractor. An AHERA accredited Project Designer should design the asbestos abatement activities and the abatement should be monitored and cleared by an AHERA accredited Contractor/Supervisor and NIOSH 582 analyst.
The survey was limited to the client directed areas and/or building materials within the interior and exterior of the building. At the request of the Client, the roof was not included in the scope of the inspection. Destructive sampling procedures were not used to access the interior of wall cavities, fire doors or above hard ceilings, interior of boilers, electrical panels or electrical wiring. If any materials are encountered during renovation or demolition activities that were not sampled in this report, the materials should not be disturbed until they have been sampled and identified as non-asbestos containing materials through laboratory analysis.

1 INTRODUCTION
AirQuest Environmental, Inc. (“AirQuest”) performed an asbestos survey within Building 2162 at the Joint Base Lewis-McChord in Washington (“the site”). The survey was performed on October 19, 2015, by AirQuest Senior Project Manager Paul LeBlanc and Nicholas Nalepa, both trained Asbestos Hazard Emergency Response Act (AHERA) certified asbestos inspectors (see Appendix I).
The purpose of this survey was to identify asbestos-containing materials (ACM) within the interior and exterior building. At the request of the Client, the roof was not included in the scope of the inspection.
The scope of services for this project consisted of the following five steps:
Building Walk-Through and Observations, Bulk Sampling of Suspect Asbestos-Containing Building Materials (ACBM), Polarized Light Microscopy (PLM) Analysis of Bulk Samples, Hazard Assessment and Evaluation, and Final Report Preparation.
2 FACILITY DESCRIPTION
Building 2162 was located within the Joint Base Lewis-McChord (“JBLM”) in Washington. The 2,300 square foot structure functioned as a boiler plant with two (2) storage rooms at the north end of the building.
The building was constructed of concrete and brick on a concrete foundation. The interior walls were constructed of unfinished concrete, wood and brick. The ceilings of the plant were constructed of wood while the storage room ceilings were constructed of concrete. The floors within the plant and storage rooms consisted of unfinished concrete. The plant was furnished with a steel catwalk system to access upper portions of the boilers.
3 SURVEY METHODS AND LABORATORY ANALYSIS
The sampling protocol used in this asbestos survey are those detailed in Title 40, Code of Federal Regulations (CFR), Part 763.86 for the sampling of friable surfacing materials, thermal systems insulation and miscellaneous materials (i.e. ceiling tiles). The Environmental Protection Agency (EPA) regulations require that sample locations be randomly selected. Suspect ACBM were identified and samples of each different type of material were obtained.
The bulk sampling procedure utilized for the collection of samples suspected of being ACBM required the establishment of homogeneous sampling areas. A homogeneous sampling area is defined as an area of friable or non-friable material of similar type that appeared to be applied or constructed during the same general period of time. This is the most acceptable and cost-effective method for the sampling of suspect ACBM.
Samples of suspect materials were collected from predetermined homogeneous sampling areas and the location of each sample was recorded. Samples were labeled and transported to the laboratory for analysis, using polarized light microscopy (PLM) coupled with dispersion staining. The samples were analyzed in accordance with Title 61 Code of Federal Regulation.
Polarized light microscopy is a method of analysis that utilizes the unique optical crystallographic properties of the various crystalline forms in the samples. These properties, such as refractive indices, birefringence, sign of elongation and extinction angle, are unique to crystalline asbestos forms and therefore can be used to identify the type of asbestos mineral as chrysotile, amosite, crocidolite, anthophyllite, tremolite, or actinolite. Each type of asbestos displays unique characteristics when subjected to these tests. Percentages of the identified types of asbestos are determined by visual estimation. Any material containing over one percent (1%) by weight is considered by the U.S. Environmental Protection Agency (EPA) to be an asbestos containing material and if disturbed must be handled according to specific local, state and federal regulations.
4 SUSPECT ASBESTOS-CONTAINING MATERIALS
Thirteen (13) homogeneous areas (HA) of suspect material were identified and a total of twenty-nine (29) samples were collected, including two (2) quality assurance/ quality control (QAQC) samples that were sent to a separate laboratory for analysis. Sample descriptions and laboratory results can be found in Table 1 - "Laboratory Analysis and Summary". A copy of the laboratory reports and chain of custody records are included in Appendix II. A diagram denoting sample locations is included in Appendix III. Photographs denoting each HA are included in Appendix
IV.

EMSL maintains liability limited to cost of analysis. This report relates only to the samples reported and may not be reproduced, except in full, without written approval by EMSL. EMSL bears no responsibility for sample collection activities or analytical method limitations. Interpretation and use of test results are the responsibility of the client. This report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST or any agency of the federal government. Non-friable organically bound materials present a problem matrix and therefore EMSL recommends gravimetric reduction prior to analysis. Samples received in good condition unless otherwise noted. Estimated accuracy, precision and uncertainty data available upon request. Unless requested by the client, building materials manufactured with multiple layers (i.e. linoleum, wallboard, etc.) are reported as a single sample.
