1=Requested data/information not supplied

2=Requested data/information supplied complete but does not adequately meet RFQ requirements

3=Requested data/information supplied incomplete, difficult to determine if vendor meets RFQ requirements

4=Requested data/information supplied complete, appears to meet RFQ requirements, little risk to government

Instructions:

Each technical evaluation team member will rate vendor supplied quotes independently and rate each evaluation criteria using the scale listed above. Once each evaluation criteria is rated the scores for each criteria will be added together to determine the rating score per vendor. Each evaluation team member will document or rationalize the score for each evaluation criteria.

Once all the evaluation team members have reviewed and rated each vendor, the total rating score will be determined by combining the rating score identified by each evaluation team member for each individual vendor.

When added together past performance and technical is equal to the importance of price.

1. Three examples of successful past performance of projects of a similar magnitude and nature.

2. Example of the type of equipment to be used for grubbing of mesquite (description of equipment make, model, and capabilities).
