Moody Air Force Base Fire & Emergency Services
150lb Halon Extinguisher
Inspection, Maintenance and Testing
Performance Work Statement
1. SCOPE OF WORK: The contractor shall provide 23 CES/CEF all labor, tools, equipment, test equipment, personal protective clothing and equipment, materials, transportation and any other incidentals necessary to travel to Moody AFB, GA to perform the on-site testing of Wheeled 150lb Halon Extinguishers. The Contractor must perform all inspection, testing and maintenance IAW T.O. 13F4-4-121 and TCTO 13F4-
4-501.
a. Perform 6-year maintenance and inspection of a quantity of 99, 150lb Halon
Extinguishers.
b. Make any needed repairs of 150lb Halon Extinguishers to ensure compliance of T.O. 13F4-4-121 and TCTO 13F4-4-501.
i. Contractor must first provide 23d Contracting Squadron (Contracting
Officer) a quote that includes the cost of but not limited to parts, repairs, tools, etc., needed to bring to compliance outside items a.-c. above prior to completing the work.
Note: Item i. above is needed in the event more funds are necessary to complete required work to maintain compliance.
c. Provide proof of inspection, testing and maintenance as outlined in T.O. 13F4-4-
121 and TCTO 13F4-4-501 for all extinguishers.
2. STANDARDS AND CODES: The contractor shall comply with all Federal, State and local regulations pertaining to the required work including, but not limited to:
a. National Fire Protection Association (NFPA)
b. Occupational Safety and Health Administration (OSHA)
c. Air Force Instructions and Technical Orders
3. DELIVERY SCHEDULE: All work is to be completed no later than 3 September
2021.
4. SECURITY REQUIREMENTS: The Contractor is responsible for gaining access to
Moody Air Force Base through the contract. The Contractor shall coordinate with the
Contracting Officer and Fire Department to gain access to Moody AFB.
a. While on Moody AFB, the Contractor shall comply with Moody AFB security measures. These regulations are available for review in the contracting office.
b. Security and base access requirements are contained in clause 5352.242‐9000
“Contractor Access to Air Force Installations.” Contract employees will be required to obtain and display identification badges.
c. Contractor employees shall provide information to assist in the completion of background checks for employees requiring access to Moody AFB. This information shall consist of, but is not limited to: full name (first, middle, last), social security number, date of birth, address, driver’s license number and state of issue. This information will be protected under the procedures listed in the
Privacy Act of 1974. A background check must be completed prior to unescorted access while on Moody AFB. If the background check reveals a
Contractor employee or prospective employee with an adverse criminal history or as a potential threat to the health, safety, operational mission of the installation, then the Government reserves the right to restrict or deny access to
Moody AFB for that individual.
d. Contractor Badges. The Contractor is responsible for tracking and retrieving badges from employees upon their separation, termination and/or completion of business on Moody AFB. Badges from employees who are no longer employed on the contract must be retrieved and turned in to the Contract Office
Representative within 30 days of the individual’s separation or termination.
Employees who abuse their badge privileges by attempting base access beyond the dates, times, locations or specified work and other limitations may be subject to revocation of base access. Businesses may lose base pass privileges if they fail to control use of the badges or do not return them upon completion of use.
Failure to control or turn in badges may result in contract termination or a decision not to exercise the option periods of this contract.
5. ANTITERRORISM: Moody Air Force Base has an Antiterrorism program that is responsible for securing and protecting the base and its personnel from terrorist attacks.
All contractor personnel and sub‐contractors are required to obey all orders from Security
Forces and adhere to all security measures implemented during Force Protection
Condition (FPCON) changes and Random Antiterrorism Measures (RAM’s). Security
Forces members may conduct inspections of your company as well as your sub‐ contractors while operating on the installation. Moody Air Force Base is currently in
FPCON Bravo, meaning an increased and more predictable threat of terrorist activity exists. All contractor personnel and sub‐contractors are expected to report any suspicious persons, activities, packages, or items you see while conducting work on Moody Air
Force Base to base Security Forces. Suspicious activities could include personnel conducting surveillance, unauthorized personnel requesting access, or information probing pertaining to security operations or assets on the installation. In the event of an emergency, there are multiple mass warning notification systems on Moody Air Force
Base (AtHoc, weather radios, telephone notification, and giant voice system). Any contractor personnel or sub‐ contractor that obtains a Common Access Card (CAC)
MUST complete the Antiterrorism Level I training within 30 days of receiving your
CAC (see paragraph 4.8.1.).
6. GENERAL PROVISIONS:
a. Notification: The contractor shall coordinate all work with the end‐user POC. The contactor shall provide notice of five days prior to work starting to allow end-user time to prepare equipment getting tested.
b. Safety: The contractor shall maintain safe and healthy working conditions for all contractor personnel and must ensure conformance with standards developed under United States Public Law 91‐596, OSHA standards, American National
Standards Institute and NFPA.
c. Mishap Reporting: Report all mishaps involving any person sustaining injury or illness from contractor operations and all mishaps involving Government property or equipment damaged by the contractor to the end‐user.
d. Environmental: Contractor is required to comply with all environmental standards as required by Moody AFB 23 CES/CEIE. They can be contacted at (229) 257‐3849.
