Attachment 2 – Statement of Work

SPECIFICATIONS FOR JANITORIAL SERVICES

Humboldt-Toiyabe National Forest Carson Ranger District Office

I. GENERAL REQUIREMENTS

A. SCOPE OF CONTRACT

It is the purpose of this contract to secure janitorial services at the Forest Service (FS) office located in Carson City, Nevada. The Contractor shall furnish all labor, equipment, supervision, transportation, supplies (except those designated as Government-furnished), and incidentals to perform all work necessary as specified herein. Contractor shall be able to climb stairs and ladders to perform certain services. Services shall not be subcontracted unless approved in writing by the Contracting Officer (CO) or Contracting Officer’s Representative (COR).

B. LOCATION AND DESCRIPTION

The contractor shall perform all contract services at the Humboldt-Toiyabe National Forest Administrative Office located at 1536 South Carson Street, Carson City, NV 89701. The Carson Ranger District Office is two buildings consisting of one level, totaling approximately 8100 square feet and one smaller office totaling approximately 2400 square feet in area.

C. CONTRACT TIME

The contract period is from April 1, 2024, to March 31, 2028. The Government, at its option, may renew this contract up to 3 years not to exceed one additional 6-month period at the same terms and conditions. The Government shall notify the Contractor of intent to renew not less than 30 calendar days prior to the expiration of the current contract period.

D. SAFEGUARDING PREMISES

All reasonable precautions shall be taken to prevent loss or damage to Government buildings or property. The Contractor is to ensure that the offices are inspected at the end of each cleaning period for fire hazards, turn off unnecessary lights, and ensure that all means of egress are securely locked.

E. SAFETY

The Contractor shall be responsible for instructing his/her employees in appropriate safety measures. Employees must not place mops, brooms, machines, and other cleaning equipment at locations in such a manner as to create safety hazards. Cautionary signs shall be displayed in areas where cleaning operations are in progress which necessitate the use of any equipment or supplies such as buckets, wires, ladders, water, wax, wet floors etc., which could cause a personal hazard. Contractors shall comply with all Federal, State, and Local regulations, laws, codes, and requirements relating to safety.

Fire prevention shall be exercised at all times by the Contractor’s employees. Janitor’s closet, etc. shall be kept clean and free from odors and fumes. All combustible material shall be used and stored in a safe manner. The Contractor shall report to the COTR any fire hazards, and if possible, correct them.

The Contractor shall be responsible for maintenance and upkeep of the MSDS forms and chemical list for all chemicals used and stored on site.

F. GOVERNMENT-FURNISHED PROPERTY

The Government shall have available certain furnishings for use to the Contractor (hereinafter referred to as "Government-furnished property"). The Contractor shall be liable for all loss or damage of such delivered Government-furnished property until completion and final acceptance of work required under this contract. The Government-furnished property includes the following:

• All necessary electricity and water needed for the Contractor's performance on the job.

• One building security code.

• Storage space for any materials kept on site (this shall be kept in a clean and orderly fashion as described in section II).

G. CONTRACTOR-FURNISHED PROPERTY

The Contractor shall furnish all janitorial supplies such as cleaning equipment, soap, wax (shall be non-skid), mops, brooms, and all equipment such as floor machines. The Contractor shall supply these items at a minimum and may require other incidentals necessary to perform work and maintain a sanitary, clean work area. Additional items the Contractor shall furnish include:

• Vacuum cleaner equipped with a high efficiency particulate air filter (heap filter) and attachments necessary for cleaning: under furniture, tops of cabinets and shelves, windowsills and trim, screens, corners of rooms, and miscellaneous equipment.

• Rags, sponges, cloths, or any other materials needed for cleaning shelves, countertops, windows (inside and outside), or any appliances.

• Carpet cleaning equipment and supplies.

• Any cleaning materials necessary for the completion of listed services in section II.

• Toilet seat covers, all indoor trash bags, toilet paper (at least 2-ply), liquid hand soap in the break room, and the 4 bathrooms, and C-fold towels for the San Jamar ultra fold fusion folded towel dispensers.

• SDS Book on site near chemicals and kept up to date according to OSHA standards.

The Contractor shall furnish all sanitary container liners and urinal cakes. Additional supplies shall be of suitable type and grade for purpose and are subject to approval by the COR.

The Contractor shall be responsible for new dispensers if they choose to refill with a product that doesn’t conform to the existing dispensers.

In compliance of the United States Department of Agriculture’s (USDA) Green Purchasing Affirmative Procurement Program (GPAPP), products supplied under this contract shall meet or exceed the following requirements:

Sanitary Tissue Products:

(Note: The content levels should be read as X% recovered fiber, including Y% postconsumer fiber and NOT as X% recovered fiber plus Y% post consumer fiber.)

The Contractor must comply with Section 9002 of the Farm Security and Rural Investment Act of 2002 (FSRIA), Executive Order (EO) 13423, “Strengthening Federal Environmental, Energy, and Transportation Management,” and the Federal Acquisition Regulation to provide biobased products.

The Contractor shall utilize products and material made from biobased materials to the maximum extent possible without jeopardizing the intended end use or detracting from the overall quality delivered to the end user. All supplies and materials shall be of a type and quality that conform to applicable Federal specifications and standards. All supplies and materials to be used in the performance of work described herein are subject to the approval of the COR.

The following is an example list of products that may be used in this contract for custodial services for which biobased products are available. The list is not all inclusive. It is desirable that contractors be able to supply the greatest number of biobased products listed meeting the health and environmental specifications.

The Contractor shall comply with the provision at FAR 52.223-1, Biobased Product Certification. The Contractor shall comply with the clause at FAR 52.223-2, Affirmative Procurement of Biobased Products under Service and Construction Contracts.

H. DELIVERIES OR PERFORMANCE

The base contract period of performance is from April 1, 2024, through March 20, 2028.

The contract period shall be for a 12-month period beginning April 1, 2024; however, at the option of the Government, the contract may be renewed for three additional one-year periods, at the quoted terms and same conditions. The Government will notify the Contractor of its intent to renew not less than 60 calendar days prior to the expiration of the current contract period.

I. SUBMITTALS

Seven days prior to an employee starting work, the Contractor shall submit for each employee working on government premises for this contract:

• I-9, Employee Eligibility Verification Form, with a copy of the documents that establish employment eligibility and identity. This information may be used for an initial background check.

• Full Name of Employee with a valid email address and telephone number. This information will be submitted to the Department of Agriculture’s Human Capital Management (HCM) department who may then contact the employee and require the employee to provide their full name, date of birth, place of birth, social security number, working contact phone number, and working email address. The employee may then be required to complete a background investigation package as requested by Department of Agriculture’s Human Capital Management (HCM).

The Government reserves the right to remove any Contractor employee that is, in the Government’s estimation, found to be unsuitable or deemed a security risk for the Government.

If such a finding occurs, either by the Government or the Contractor, the employee shall be immediately dismissed from duty and shall not be permitted to enter the buildings. The Contractor shall immediately inform the COR of any such issues.

The Contractor shall, within 10 days from start of contract, submit for the CO’s approval the following:

• Complete list of all personnel employed under this contract with a designated-on site working supervisor. This list shall include individual tours of duty (such as 6:30pm – 12:00 midnight) and scheduled work assignments.

• Schedule for contracted services to be performed bi-monthly, quarterly, and semi-annually.

• Quality Control Plan ensuring contract compliance.

• Submittals as detailed under G. Contractor-Furnished Property, biobased products.

• Material Safety Data Sheets (MSDS) and chemical lists in a notebook for chemicals stored in Government-Furnished storage spaces. MSDS notebooks shall be kept up-to-date and stored in storage spaces with chemicals.

II. TECHNICAL SPECIFICATIONS

Quality cleaning will be required, including full performance of all specified daily services on the first official working day of the contract. A fully qualified workforce shall be maintained throughout the contract period.

The Contractor shall prohibit employees from disturbing papers, computers, and terminals on the desks, opening drawers or cabinets, or using the telephone, except for emergency calls to police or fire departments. The Contractor or employees shall not bring children to the premises while the work is being performed.

The Contractor shall keep the Government provided storage space for bulk supplies and equipment in a clean and orderly condition. Failure to keep any of the storage areas in a clean condition may result in the withdrawal of the privilege of using them, or the CO may have them cleaned and deduct the cost from Contractor's payment. All cardboard boxes need to be knocked down and placed in the appropriate designated container.

The Government will not be responsible for damage to the Contractor's stored supplies, materials, equipment, or the Contractor's employees' personal belongings brought into the buildings.

The Contractor shall require employees to comply with instructions pertaining to conduct and building regulations issued by duly appointed officials. The Contractor shall have a working supervisor available to ensure contract compliance.

The Contractor shall supply all restrooms to maximum capacity during the last contract day.

Dispenser stock of paper supplies and hand soap remaining at end of contract period shall not be removed.

A. SCHEDULE OF SERVICES

To facilitate contract administration and inspection by designated Representatives:

The contractor shall clean the Carson Ranger District Office on scheduled days of Monday through Friday. (Contractor option on Friday, Saturday, Sunday) All services shall be performed before 7:00 a.m. or after 6:00 p.m. Services performed outside the building may be accomplished anytime during the week. Cleaning is not required on Federal holiday’s list to be enclosed. Any office that is locked with no key provided is exempt from cleaning listed below:

1. The following services shall be performed daily:

a. Floor/Carpet Maintenance

All carpeted areas (including carpeted stairways) are to be vacuumed to remove surface dirt, dust, and litter. (Surface dirt referred to is any visible foreign matter found on the floor, rug, or carpet surface.)

Soiled areas shall be spot cleaned.

Sweep or vacuum and mop all non-carpeted floors with water and floor cleaner (all bathrooms, and mail room). Mop water shall be changed when it becomes dirty. No standing water shall be left on floors after mopping. Special attention shall be paid to the floors when black scuff marks appear (from work boots) as well as in the coffee area due to high use. Black scuff marks shall be lifted from non-carpeted floors with each cleaning.

Baseboards, walls, furniture, and equipment shall in no way be splashed, disfigured, or damaged during cleaning operations.

b. Bathrooms and Sinks/Drinking Fountains

Damp-wiping: This task consists of using a clean damp cloth or sponge to remove all dirt, spots, streaks, and smudges from walls, glass, and other specified surfaces and then drying to provide a polished, streak-free appearance. The wetting solution shall contain an appropriate cleaning agent. When damp-wiping in restroom areas, a multi-purpose (disinfectant-deodorizing) cleaner shall be used. Damp-wipe walls around sinks and toilets. Damp-wipe and dry dispensers and chromium fixtures.

Clean and sanitize all sinks, restrooms stalls, toilets, urinals, mirrors, etc. Spot clean all hand and foot marks on walls, stalls, and floors in restrooms/showers. Solution to be used for scrubbing and damp mopping in all restrooms shall contain disinfectant as well as cleaner/detergent/water mixtures. All cleaned surfaces shall be wiped dry to prevent spotting, streaking, and smudges.

Toilets-Scrub bowl daily, leave seat in up position. Special attention shall be given to floors around urinals and commodes for disinfecting and elimination of odors and stains and to provide a uniformly clean appearance throughout.

Bright Metal Polishing- Bright metal polishing may be performed by damp-wiping and drying with a suitable cloth if a polished appearance is to be obtained; however, if a polished appearance cannot be produced, the COR shall be contacted for direction as to use of an appropriate metal polish.

Service and fill all touchless towel dispensers, soap and toilet paper dispensers, note water may not be added to soap dispensers to fill to the top only soap can be added to make the dispensers full. Empty and replace any sanitary containers in women’s restroom stalls. One extra toilet paper roll shall be placed on the back of each toilet.

Porcelain Cleaning-Porcelain fixtures (drinking fountains, wash basins, urinals, toilets, etc.) shall be clean and bright. There shall be no dust, spots, stains, rust, green mold, encrustation, or excess moisture.

c. Offices/Reception Area/Break Room/ Coffee Areas

Empty contents of all wastebaskets into large plastic garbage bags and deposit into the appropriate designated dumpster. The Contractor shall conserve plastic wastebasket liners whenever possible.

Damp-wipe all tables in breakroom, and small conference room. Damp-wipe the interior and exterior of microwave, top of refrigerators and cabinets in breakroom. Damp –wipe coffee maker and cabinet area in breakroom. Scrub and disinfect sink.

d. Entrances, Lobbies, and Corridors

Floors shall be cleaned as specified under “Floor/Carpet Maintenance” (see above).

Front entrance/front desk windows and glass doors will be cleaned daily with window cleaner. All surfaces (including pamphlet shelving) within approximately 70 inches from the floor shall be dusted, metal and wood surfaces polished; walls spot cleaned, and glass surfaces cleaned of smudges, fingerprints, and water spots.

Exterior concrete sidewalk/entrance shall be swept free of debris at main entrance door.

Front Counter should be damp wiped with clean water only and polished daily.

Drinking fountain shall be cleaned and disinfected.

2. The following services shall be performed once weekly:

Thorough wet cleaning of all furniture, tables. Table legs, chairs, chair legs, desks, all surfaces, filing cabinets, desk bookshelves, and counters.

Dust credenzas, file cabinets, bookshelves, and computer terminals with dry rag and furniture polish.

3. The following services shall be performed once bi-weekly:

Vacuum floors with attachment along wall trim and under credenzas, tables, and desks.

Door thresholds shall be wiped cleaned of spotting and dirt build-up.

Dust windowsills/trim, light fixtures, and upper and lower vents with dry rag and furniture polish.

Clean ceiling and ceiling corners of cobwebs.

Wash glass in doors in all entryways (inside and outside) with glass cleaner and dry.

Empty contents of cigarette/trash receptacles located at entryways to the building (includes front and back door entrances) into metal garbage can to cool any potential fire danger then deposit into dumpster in the parking lot.

4. The following services shall be performed bi-monthly:

Wash inside windows and remove dust, cobwebs and insects that have accumulated between window and screen.

Wash windowsills and frames.

Shampoo carpet in high traffic areas and main hallways.

5. The following services shall be performed quarterly:

Wax and polish all non-carpeted floors.

6. The following services shall be performed semi-annually:

Wash and dry venetian blinds in window frames.

Wash and dry outside windows with window cleaner. Remove insect nests within three feet of windows.

Shampoo all carpets. The Contractor shall notify the COR one week prior to cleaning carpet so Government employees can remove small items from floors.

Wash walls and doors of the entire office.

B. CLEANING SPECIFICATIONS

General: For all operations where furniture and equipment must be moved, no chairs, waste-paper baskets, or other similar items shall be stacked on desks, tables, or windowsills. Upon completion of work, all furniture and equipment must be returned to its original position.

III. INSPECTION AND ACCEPTANCE

Services performed under this contract shall be subject to random inspections by the COR (see attached inspection form). Work not in compliance with specifications contained herein will be brought to the attention of the Contractor through a Notice of Noncompliance. The Contractor shall correct deficiencies as directed or be subject to termination.
