Statement of Work Building Demolition/Disposal with Asbestos Containing Materials (ACM) Coconino and Kaibab National Forests
Mogollon Rim and Williams Ranger Districts
Project:
This Statement of Work (SOW) outlines tasks for the demolition and disposal of four (4) buildings, asbestos containing materials (ACM) abatement, and lead based paint (LBP) testing.
1. Blue Ridge Yellow Marlette Trailer- SP#4 (70042) located on the Mogollon Rim Ranger District on the Coconino National Forest
2. Blue Ridge New Moon Trailer- SP#9 (70043) located on the Mogollon Rim Ranger District on the Coconino National Forest
3. Blue Ridge Columbia Trailer (70044) located on the Mogollon Rim Ranger District on the Coconino National Forest
4. Kaibab Supervisor’s Office Shed Building (10002) located on the Williams Ranger District on the Kaibab National Forest.
Option #1 work consist of soil sampling and testing for total lead if they test positive for LBP.
Option #2 work consists of LBP abatement if they test positive for LBP.
Background Information:
1. Blue Ridge Yellow Marlette Trailer was built in 1979 and is 938 square feet.
2. Blue Ridge New Moon Trailer was built in 1977 and is 744 square feet.
3. Blue Ridge Columbia Trailer was built in 1967 and is 630 square feet.
4. Kaibab SO Office Shed building was built in 1995 and is 165 square feet.
Sanitary Facilities:
Sanitary facilities for workers are available. Electricity and water are not available at the sites.
The contractor is solely responsible for securing materials and equipment on the sites.
Location:
1. Blue Ridge Yellow Marlette Trailer Project Location: To reach Mogollon Rim Ranger Station from Flagstaff, proceed south on Coconino County Road 3 for approximately 53 miles to the intersection of State Route 87. Turn left and proceed approximately 8 miles to the Ranger Station, which is on the south side of the road. Driving time from Flagstaff is approximately 1 hour and 30 minutes. Coordinates for the Blue Ridge Yellow Marlette Trailer are 34°36'27.09"N 111°11'21.48"W.
2. Blue Ridge New Moon Trailer Project Location: To reach Mogollon Rim Ranger Station from Flagstaff, proceed south on Coconino County Road 3 for approximately 53 miles to the intersection of State Route 87. Turn left and proceed approximately 8 miles to the Ranger Station, which is on the south side of the road. Driving time from Flagstaff is approximately 1 hour and 30 minutes. Coordinates for the Blue Ridge New Moon Trailer are 34°36'30.90"N 111°11'20.04"W.
3. Blue Ridge Columbia Trailer Project Location: To reach Mogollon Rim Ranger Station from Flagstaff, proceed south on Coconino County Road 3 for approximately 53 miles to the intersection of State Route 87. Turn left and proceed approximately 8 miles to the Ranger Station, which is on the south side of the road. Driving time from Flagstaff is approximately 1 hour and 30 minutes. Coordinates for the Blue Ridge Columbia Trailer are 34°36'31.33"N 111°11'19.13"W.
4. Kaibab SO Shed Building Project Location: To reach Kaibab Supervisor’s office from Flagstaff, proceed east on I-40 24.8 miles. Take exit 165 toward W Historic Rte 66. Turn left onto W Historic Rte 66 and continue for 2.5 miles. Turn left onto N 4th St. and continue for 0.1 miles. Turn right onto W Grant Ave and continue for 0.1 miles. Turn left onto S 6th St and continue for 0.5 miles. Take a slight right and the destination will be on the left. Driving time from Flagstaff is approximately 40 minutes. Coordinates for the Kaibab SO Shed are 35°14'28.3"N 112°11'34.2"W.
Period of Performance:
The demolition and disposal must be completed within 90 days of the issued notice to proceed.
No work can begin until the notice to proceed is issued.
Contract Time:
Weekend work is allowed on this project.
Utilities:
The contractor is responsible for ensuring that utilities including propane, water, and sewer are properly disconnected before demolition and capped at appropriate locations if required. The government will disconnect the electrical, water, sewer and other utilities at the four sites.
Tasks:
The contractor shall show proof of certification in compliance by EPA to engage in lead-based paint and asbestos removal activities as per 40 CFR 745.225, 226 & 227. The contractor shall comply with the Federal Regulatory Agencies: U.S. Environmental Protection Agency (EPA), U.S. Housing and Urban Development Agency (HUD), U.S. Occupational Safety and Health Administration (OSHA), U.S. Department of Transportation (DOT), U.S. Department of Health and Human Services (HHS), U.S. Department of Labor (DOL). They shall also comply with local and State laws, regulation and regulatory agencies; and Arizona Department of Environmental Quality (ADEQ).
Hantavirus Pulmonary Syndrome (HPS): Because of the location of these buildings, rodent droppings are present. It shall be the contractor’s responsibility to take precautions to minimize the risk of HPS to workers. There will be no additional compensation for hantavirus precautions.
The utilities shall be verified prior to commencement of the work and precautions taken to protect existing ones to remain in place during and after the work. If damage is incurred the contractor shall bear the cost to repair the utility at no cost to government.
Tasks are Pay Items outlined in the Schedule of Items. Payment will be lump sum for each task number.
Task 1: Mobilization and Site Preparation This work consists of all activities associated with mobilization and demobilization to perform all the tasks associated with the Statement of Work. This will include all labor, materials, tools, equipment, appliances, services, testing, supervision, incidentals and permits needed to accomplish the work in the SOW.
Task 2: Lead Based Paint Testing Task 2 consists of activities associated with LBP testing and analysis on the three trailers. The contractor shall test suspect exterior and interior surfaces on the three trailer buildings including pre demolition soil sampling and testing to determine the presence and amount of LBP to be abated and provide the cost for removal.
Task 3: Asbestos Containing Material Removal and Disposal.
Task 3 consists of activities associated with removal and disposal of asbestos-containing material located in the three trailer buildings. Asbestos removal activity in the three buildings shall be conducted per OSHA and NESHAP standards and regulations. The Contractor shall be licensed by the State of Arizona as an Asbestos Abatement Contractor. Additionally, the contractor shall meet all local, state, and federal regulations for the abatement of asbestos.
All Asbestos Containing Material (ACM) shall be wet and double bagged in minimum 6 mil plastic containers with duct taped seams. Asbestos Containing Material shall be disposed of at an appropriate landfill, with a completed transportation manifest provided to the government upon completion of the project. All associated disposal costs are the responsibility of the contractor.
The contractor is responsible for obtaining an Asbestos NESHAP Notification/Permit Activity Application for Demolition Activities. Permit shall be filed with Arizona Department of Environmental Quality prior to commencement of work.
Task 4: Structure Demolition and Disposal Task 4 consists of activities associated with (1) removal and disposal of existing furniture, appliances, wooden decks/steps, street side front fencing/poles and other items from the three trailer buildings; (2) demolition and removal of three trailer buildings and shed; (3) removal and disposal of concrete pads; (4) removal and disposal of foundations at the three trailer buildings sites, and (5) regrading the sites to match existing grade for drainage.
The Contractor shall remove and dispose of existing furniture and all other items stored in the building as part of Task 4.
Following all abatement efforts, the contractor shall demolish all remaining structures, including foundations to at least 24” below grade. Dispose of all material at a certified local landfill. The contractor shall grade all sites to match existing grade to drain water and leave a natural appearance.
The contractor shall remove concrete pads at entrance and fill any depressions left behind.
Dispose of concrete at a certified landfill.
Task 5- Option 1: Soil Sampling and Testing for Total Lead.
The contractor shall take post work exterior composite soil samples at all sites that are positive for LBP. Soil will be collected from five spots during post work for each trailer. The total is 30 samples and analysis, if positive for LBP. All samples shall be located from two taking post-work soil samples. If the post results are >400 ppm and pre results were <400 ppm the contractor shall be liable for soil contamination reduction to <400 ppm.
Task 6- Option 2: Lead Based Paint Abatement This work consists of Lead-Based paint removal and abatement from the three trailer buildings, if tested positive for LBP, prior to demolition. Task 6 consists of all activities associated with the proper disposal of the lead-based paint at an appropriate landfill. For cost purposes, the Contractor shall assume that all structures to be demolished and disposed are to be disposed at RCRA Subtitle D landfill (solid waste landfill).
Abatement includes, but limited to, all preparation, cleanup, disposal, and post-abatement clearance/confirmation testing, and analysis activities associated with such measures.
The contractor shall investigate and determine the best method available for lead-based paint abatement. The method shall be submitted for approval to the COR. Some approved methods available to the contractor are:
a) Machine Sanding – requires HEPA exhaust control.
b) Chemical Removers/Strippers.
c) Vacuum Blasting, Abrasive Blasting, Plastic Bead Blasting, etc., -requires HEPA exhaust control.
d) Hand Scraping and Hand Sanding – for limited areas only.
e) High Pressure Water Spray – must treat runoff and paint chips as hazardous waste.
Deliverables:
1. A construction report, including testing and analysis documents, and if needed, the abatement of Lead Based paint and Asbestos shall be submitted by the contractor. The report shall contain the quantity of removal, method of removal, including labeled, dated, and location of color photographs of materials before, during, and after abatement.
2. Waste disposal information including facility name and location and Uniform Hazardous Waste Manifest for all hazardous and non hazardous waste, and asbestos.
3. Total Lead soil sample test results 4.Sample results of dust lead levels of pre and post work soil samples if needed.
