PART I – PROJECT SPECIFIC GUIDANCE

1.3. LIMITATIONS: The contractor shall be escorted into any areas they do not have the appropriate clearance to be in as determined by the government.

2. DESIGN REQUIREMENTS: TABLE OF DELIVERABLES
a. Demo slab & excavate for 3 new Serco dock levelers at 3 loadout door locations.
b. Pour back slab with rebar reinforcement epoxied into existing slab.
c. Install 3 new Serco dock levelers at 3 loadout door locations.
d. Work to be conducted during normal business hours.
e. Electrical tie in if hydraulic system is selected.
f. Work to be completed during normal business hours and on consecutive days
g. Contractor will patch/repair and paint areas that are impacted (Footprint) from the removal and installation of new equipment to match existing.
3. CONSTRUCTION REQUIREMENTS
a. In accordance with (IAW) United Facilities Guide Specifications (UFGS) 01 33 00
Submittal Procedures shall govern construction submittals.
b. Demolish existing floors to prepare for new install.
c. Remove exterior equipment around outer dock.
d. Patch holes and paint interior and exterior walls to match existing.
e. Submittals: Provide color sample for rooms and hallway for approval.
f. Submittal's: Provide submittals for materials being used for approval
g. Warranty work for 1 year after final BOD.
4. PERMITS – The Contractor is responsible for all applicable permits, licenses, and certificates required to perform and execute this contract.
5. CONTRACT ADMINISTRATION REQUIREMENTS
a. PROGRESS SCHEDULE, AF FORM 3064 – The Contractor shall prepare and submit, for approval, by the Contracting Officer (CO), a work breakdown structure. The work breakdown structure shall be used to report the cost and schedule status for this project. All tasks required under this type of contract shall be included in the work breakdown structure.
b. NOTIFICATION REQUIREMENT – The Contractor is required to notify the
Contracting Officer and the Project Manager of critical issues that may affect life, health, safety, contract performance, or the environment. Verbal notification should be made immediately followed by written notification as soon as practical.
6. SECURITY:
a. SECURITY INSTRUCTIONS: All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the
Installation shall abide by all security instructions and directives of JBER.
b. At the close for each work period, government facilities, equipment and materials shall be secured, lights, heat and water turned off and all doors and window secured.
c. The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general wellbeing or operational mission of the Installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified during the Criminal
Background Check (CBI).
c. Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Employees and subcontractors later found to be undocumented workers will be remanded to the proper authorities.
d. All contractors and subcontractors when working in a Controlled, Restricted or other sensitive area must be escorted at all times while. The military agency or unit responsible for the project is responsible for providing the escorts. The Contractor shall follow existing procedures and instructions for obtaining entrance to restricted or controlled areas.
7. CONTRACTOR BADGE POLICIES:
a. All requests for contractor badges will be submitted through the 673th Contracting
Squadron (673 CONS) and/or the requesting agencies base point of contact. 673
CONS or base point of contact will then complete an Operational Risk Management
Assessment Form (ORM) for the requesting agency and submit it to the 673 SFS Pass and Registrations section for approval in accordance with Operation Plan (OPlan) 31-
101. As a minimum the ORM will be submitted 10-days prior to the requested date of employment. At no time will a contractor contact 673 Security Forces Squadron (673
SFS) directly regarding badge denial.
b. All contactor employees requesting unescorted access to the Installation are required to consent to a Criminal Background Investigation (CBI) prior to being granted entry to the Installation. Continued employment is contingent upon successful completion and favorable reporting of the CBI.
c. Joint Base Elmendorf-Richardson (JBER) Contractor badges will be issued for a maximum period of one year. Prior to reissuing new badges, all old badges must be returned to 673 SFS, Pass and Registration for destruction. Prior to reissuing a badge an ORM must be resubmitted to the 673 SFS Pass and Registration section for approval. In the event a badge is lost or stolen, immediately contact the 673 SFS, Pass and Registration office. The badge holder must accomplish the Loss/Theft of
Identification Worksheet and provide it to his/her supervisor. The supervisor of the contractor will investigate the loss and report in writing the circumstances in which the badge was lost to the 673 CONS and 673 SFS, Pass and Registration office. A new request for a badge must be completed prior to a new badge being reissued.
d. The 673 CONS or base POC will immediately notify 673 SFS, Pass and Registration when a contractor’s employment has been terminated. The Site Supervisors are responsible for notifying and returning the contractor’s badge to 673 SFS Pass and
Registration when this occurs.
8. INSTALLATION ACCESS:
a. The contractor shall obtain personal contractor identification badge for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto JBER from 673 SFS, Pass and Registration, for the duration of the contract.
b. Employees are only permitted to enter the Installation during the date and time periods indicted on their contractor badge. Vehicle registration, proof of insurance and a valid driver’s license must be presented for all vehicles while operating on the Installation.
All vehicles entering JBER are subject to search. Any refusal or non-consent by an employee will result in termination of their base access and immediate confiscation of this access badge.
c. During Force Protection Condition (FPCON) Normal and Alpha, personnel without base issued badges must be sponsored onto the installation.
e. During FPCON Bravo, Charlie and Delta, personnel without base issued identification shall be physically escorted onto the installation.
f. During Higher FPCONs (Charlie and Delta) the base will normally curtail non-essential operations/functions; access by non-essential Contract operations will be suspended as the direction of the Installation Commander.
g. Department of Defense (DoD) contractors or subcontractors without a Common
Access Card (CAC) or who do not have access to a government Non-classified
Internet Protocol Router Network (NIPRNET) computer performing construction on the installation should consider completing Anti-Terrorism (AT) Awareness Level I training IAW AFI 10-245, Antiterrorism, Standard 25, by completing the― Force
Protection computer based training (CBT) course through the Joint Knowledge Online
(JKO) Learning Management System (LMS) at http://jko.jten.mil/courses/atl1/launch.html IMPORTANT: The standalone course
MUST be completed on the computer in which it is started. Course progress is not transferable between computers. The standalone course does provide a completion certificate but will not be tracked on JKO and no record will be maintained. AT
Awareness Level I Training should be provided by the requiring unit after contract award or the sponsoring organization.
9. FLIGHTLINE DRIVING: Not applicable for this project.
