Performance Work Statement for Lynx Model No 810LX SN2569 Preventa�ve maintenance

A. Scope of Work

Provide preventa�ve maintenance (PM) for a Lynx 810LXS Washer (S/N-2569) in accordance with manufacturer recommenda�ons for one year, with the op�on years available. The objec�ve of this contract is to support mission cri�cal equipment by maintaining required equipment. To meet this objec�ve close coordina�on is required between the Contractor and the Contrac�ng Officer’s Representa�ve (COR).

B. Background

The NBAF is a high and maximum containment facility that has a strong need to train their Animal Resource Unit (ARU) and Facili�es and Opera�ons Management Unit (FOMU) on repairs that can be performed in-house. The level of containment may restrict visits during certain �mes of the year. Training is included in this contract to allow ARU or FOMU personnel to be able to perform minor repairs in-house with virtual assistance (phone or modem). NBAF has a need for a preventa�ve maintenance (PM) service contract which will be used to thoroughly inspect the Lynx Model Rack Washer and Decontamina�on Chamber. Service requested must include a thorough inspec�on of the equipment based on the list supplied by the manufacturer as well as common areas of failure.

C. Technical Requirements/Tasks

The contractor shall provide all, labor, equipment, materials, transporta�on, and supplies for the preventa�ve maintenance of the Lynx 810LXS washer and decontamina�on chamber.

a. The specific goal of this service contract is to provide 1 year of PM and 4 op�on years for a total of 5 years of PM. Maintenance must be performed no less than semi-annually. The maintenance should include a thorough inspec�on of the equipment including areas of common failure with the equipment, lubrica�on of necessary parts, and calibra�on as needed. Post PM, the unit must be able to properly clean and sani�ze animal caging and equipment and pass temperature tape tests to ensure these processes are adequate.

b. During this contract, unlimited in-house factory technical support via phone or email will be provided at no cost to the customer.

c. Es�mates shall be provided for repairs no more than 1 week a�er a problem arises or a PM service is provided. The contractor shall do their utmost to ensure that if the rack washer needs repair that it will be repaired within 30 days. It is understood that parts and labor for repairs are beyond the scope of this contract and will be billed separately but shall s�ll be supplied by the Contractor.

d. A detailed condi�onal report on the Lynx Rack Washing and Decontamina�on Chamber should be prepared by the contractor and reviewed with the point of contact(s) from ARU and FOMU.

e. Due to inability for outside contractors to gain access to the facility at all �me, training shall be provided when the Contractor is on-site for PM to a minimum of the Animal Resource Unit (ARU) and Facili�es and Opera�on Management Unit (FOMU) personnel allowing limited maintenance to be performed by in-house personnel.

f. PM shall be scheduled in advance each year.

D. On Site Requirements

1. The contractor shall furnish a detailed technical proposal based on manufacturer recommenda�ons as well as a detailed cost proposal, including all prime and sub pricing for this SOW and manufacturer recommended maintenance schedule.

1. The rack washer should be inspected based on manufacturer recommenda�ons (see atachment).

2. The period of performance (POP) shall be 365 calendar days with op�on years.

3. The contractor is responsible for providing services between 0700 and 1630, Monday through Friday except Federal holidays or when the USDA-ARS facility is closed due to unforeseen facility closings. Unforeseen facility closings: meaning when the Government facility is closed due to local or na�onal emergencies, administra�ve closings, or similar Government directed facility closings. Contractor will be no�fied by the COR of the NBAF closure.

4. The contractor shall provide the contrac�ng officer (CO) a list of all employees who will perform the work under this contract. The list shall include name of each employee. The Contractor shall maintain revisions and updates to this list and submit them to the COR on a quarterly basis.

5. Personnel employed by the Contractor or any representa�ve of, or agent of the Contractor, entering this facility shall comply with the facility security regula�ons.

The Contractor shall provide all informa�on required for background checks to meet facility access requirements as performed by the facility security office.

Contractor personnel shall comply with all personal iden�ty verifica�on requirements as directed by USDA-ARS Security officer. Each person accessing the site will be required to provide personal details, (full name, DOB, SSN, DL#) and will undergo a rou�ne background check, such as a Na�onal Agency Check with addi�onal inquiries, which may include a criminal and credit check. Details required for the check should be submited directly to the security officer no later than 10 days prior to the visit. Results of this check may return earlier. However, it should not be planned to have results earlier than 10 days. Contractors need to follow Title 41, Code of Federal Regula�ons, Part 102-74, Subpart C Rules and Regula�ons Governing Conduct of Federal Property.

6. If, while on site the contractor/vendor will have access to sensi�ve informa�on, they must complete a Na�onal Disclosure Agreement, along with the required training. This training will likely be completed by reviewing a series of slides covering the topics of the defini�on of controlled unclassified informa�on (CUI), access, handling, distribu�on, storage and destruc�on of CUI. Each person will acknowledge receiving and understanding the training on the NDA. The process should not exceed two hours. Acknowledgement and comple�on of the NDA will be turned in with the form required for the background check. Results will be returned through the Contrac�ng Officer or Contrac�ng Officer Representa�ve.

7. The Contractor shall protect the work, the site, and all exis�ng property and structures within the limits of the service ac�vi�es or that may be affected thereby un�l acceptance of the work. Any damage to property shall be repaired at the Contractor’s expense, to pre-damaged condi�on to the sa�sfac�on of the COR.

The Contractor shall follow all onsite safety requirements.

8. The Contractor shall always keep work areas clean and free of debris. The Contractor shall work with a government representa�ve to iden�fy means and loca�ons for disposal of old parts, material, and waste.
