PERFORMANCE WORK STATEMENT

for

Side Room Floor Replacement

Hangar 5

Joint Base Elmendorf-Richardson (JBER), Alaska

1. DESCRIPTION OF SERVICES. To replace existing carpet and add fitness impact flooring to concrete to the side rooms of Hangar 5. The fitness areas shall be at least 15mm think multi-use sport flooring designed to take weight drops, heavy static equipment seen in a weight room setting and the admin area shall have at least 8 mm sport flooring. The flooring system should be designed for shock absorption and energy return to ensure a high performance sports flooring. All flooring shall be adhered to the foundation using the flooring manufacturers recommended procedures and adhesives. All work performed by the contractor shall be performed in accordance with (IAW) all applicable laws, regulations, Air Force standards, instructions, and commercial practices.

a. The Contractor shall supply all labor, materials, transportation, equipment, and supervision to provide a complete, finished, functional product meeting 673 FSS requirements.

b. Include in the proposal a timeline for delivery/installation including on-site construction time.

c. Site Survey: Prior to the start of any on-site installation activities, the Contractor shall conduct a condition survey with the 673 FSS, after which the Contractor shall note the condition of the concrete and any structural damage that may exist immediately within or adjacent to work site.

d. The sport flooring installer shall be responsible for all work required to prep the concrete floor and remove the carpet from the rooms. Contractor shall level out any gaps in the concrete, remove existing cove base and transitions. The installer shall adhere the sports floor to concrete. Contractor shall install new cove base and provide transitions between different floor thicknesses.

e. Area is known to have lead based paint based on testing samples (attached).

Removal of lead based paint is required for floor leveling. Contractor shall confirm lead based paint requirements prior to disturbing sub-floor. Contractor shall remove lead based paint IAW Federal regulations.

2. GENERAL CHARACTERISTICS: The contractor shall be responsible for the following.

a. Shall remove and dispose of existing flooring.

b. Shall remove lead based paint IAW Federal regulations.

c. Shall prep the concrete base flooring for installation.

d. Shall be adhered to the concrete floor with manufacturer recommended adhesive or similar product.

e. Shall install transitions between existing fitness center floor and new sport flooring.

f. Shall ensure all seams between new flooring rolls and tiles are flush and smooth.

g. Shall install a roll product flooring to cover a roughly 4100 sq-ft side rooms and may not be a poured or tile floor. The roll product flooring shall be a lighter grey speckled color to be approved by the 673 FSS.

h. Shall install 6-flush in-laid platforms (6'x8') made of the same material with a different color scheme to clearly identify the lifting platform. Platforms shall be flush mounted.

The impact zone (bumper drop zone) and the lifting platform shall be contrasting colors.

Lifting platform shall be royal blue in color and drop zones shall be dark grey or black in color, final color to be approved by the 673 FSS.

i. Transitions must be installed to existing flooring and between flooring of different thicknesses.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES

a. Government Furnished Facilities. The government will provide storage space.

b. Government Furnished Supplies and Equipment. There is no Government furnished property.

c. Government Furnished Utilities. Government will furnish electricity, water and sewage service as necessary for accomplishment of work IAW this contract.

d. Utility Conservation. Contractor shall adhere to all base level utility conservation practices or requirements. Contractor shall be responsible for operating under conditions that prevent waste of utilities.

e. Security, Fire and Medical Services. Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis.

f. Refuse Collection and Disposal. Contractor shall use existing bulk containers to dispose of trash or refuse generated from accomplishment of services detailed in this PWS. The trash shall be deposited in the nearest outside trash collection container and contractor shall ensure that no spills of fluids or trash remain behind.

4. GENERAL

a. Hours of Operation. The facility will be closed down for the duration of the installation. The contractor shall be able to utilize all hours of the day, seven days a week to complete this contract in the most expedient manner.

b. Base Closures. Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.

c. Security Requirements. The Contractor and all employees will comply with all JBER security requirements imposed by the Installation Commander at all times while on the premises. Contractor will obtain all necessary passes, decals, badges or other items required for access to perform custodial services at JBER.

d. Hazardous Material Identification. Safety Data Sheet (SDS) are required as specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract) for any other material designated by a government technical representative as potentially hazardous and requiring safety controls. SDS must be submitted by the Contractor upon contract award and updated as inventory is added or deleted. Failure to provide SDS or certificate when requested could result in the Contractor being considered non-responsive and may result in termination of the contract. Contractor shall provide an inventory of materials being brought onto JBER.

If applicable, SDSs will be readily available to personnel working with hazardous materials.

e. HAZMART. Contractors conducting operations on JBER are required to comply with the Federal Emergency Planning and Community Right-to-Know Act (EPCRA). To meet compliance, the Contractor must supply the HAZMART, the government hazardous materials distribution unit (HAZMART personnel) and shall supply

HAZMART with a Chemical Authorization List (CAL) of all chemicals proposed to be used on base 30 days prior to conducting any work on base. All hazardous materials shall be approved of by HAZMART prior to being transferred to, or used on, the base.

An inventory of the hazardous materials shall be performed prior to commencement of work. All contractors shall comply with Annex's "E" and “F" of the JBER environmental management document, the "O Plan 19-3 EMP", for the management of hazardous materials, in particular Annex "F" Section 3, Subsections b, c and d. All Contractors conducting operations on JBER are required to comply with the federal regulations regarding the management of hazardous waste (Subpart C of 40 CFR 261) "Criteria for identifying hazardous waste"). Contractor shall comply with the Annex "C" of the JBER environmental management document, the "O Plan 19-3 EMP", for the management of hazardous waste. All Hazardous Waste must be characterized, profiled, and approve all manifests by the JBER Hazardous Waste Center prior to removing them from the base.

SUBJECT: Product Specifications for Weight Room Flooring

1. General requirement: To replace existing carpet and add fitness impact flooring to concrete to the side rooms of Hangar 5. The fitness areas shall be at least 15mm think multi-use sport flooring designed to take weight drops, heavy static equipment seen in a weight room setting and the admin area shall have at least 8 mm sport flooring. The flooring system should be designed for shock absorption and energy return to ensure a high performance sports flooring. All flooring shall be adhered to the foundation using the flooring manufacturers recommended procedures and adhesives.

a. General flooring area shall be a roll product and may not be a poured or tile floor. Product should be manufactured in the USA and utilize at least post-consumer recycled content. Contract will install the new flooring in the side rooms at Hangar 5, building 7309 on Joint Base Elmendorf Richardson Alaska. The area to be covered is approx. 4100 sq ft. Product shall be gray speckled in color and be approved by 673 FSS.

b. 6-flush in-laid platforms (6'x8') made of the same material with a different color scheme to clearly identify the lifting platform. Platforms shall flush mounted and may not be made up of pieces smaller than 2’x2’. The impact zone (bumper drop zone) and the lifting platform shall be contrasting colors. Lifting platform shall be royal blue in color and drop zones shall be dark grey or black in color and be approved by 673 FSS.

c. Transitions must be installed to existing fitness center flooring and new cove base installed on the wall/floor.

2. Fitness Area salient characteristics flooring rolls/tile product must be equal or better to the following:

a. At least 15mm thickness.

b. Tensile strength shall meet or exceed 175lbs/sq inch when tested according to ASTM D412.

c. Flexibility shall meet or exceed ASTM F137 Standard Test Method for Flexibility of Resilient

Flooring Materials with Cylindrical Mandrel Apparatus with average results of Pass 0.25” Mandrel

d. Vertical Deformation shall meet or exceed 0.9mm when tested with STM F2157 Standard Specification for Synthetic Surfaced Running Tracks.

e. Force reduction shall meet or exceed a reduction of 32% when tested under ASTM F2569.

f. Skid resistance shall be equal or better to a 99 dry/99 wet when tested under ASTM E303.

g. Coefficient of friction shall be equal or better to a 1.10 dry/.96 wet for neolite and 1.05dry/.89 wet for leather when tested under ASTM D2047.

3. Admin/FAC Area salient characteristics flooring rolls/tile product must be equal or better to the following:

a. At least 8 mm thickness.

b. Tensile strength shall meet or exceed 125lbs/sq inch when tested according to ASTM D412.

c. Flexibility shall meet or exceed ASTM F137 Standard Test Method for Flexibility of Resilient Flooring Materials with Cylindrical Mandrel Apparatus with average results of Pass 0.25” Mandrel

d. Vertical Deformation shall meet or exceed 2.0 mm when tested with STM F2157 Standard Specification for Synthetic Surfaced Running Tracks.

e. Force reduction shall meet or exceed a reduction of 12% when tested under ASTM F2569.

f. Skid resistance shall be equal or better to a 99 dry/99 wet when tested under ASTM E303.

g. Coefficient of friction shall be equal or better to a 1.10 dry/.96 wet for neolite and 1.05dry/.89 wet for leather when tested under ASTM D2047.

4. Salient Characteristics floor base product must be equal or better to the following:

a. 4” thick rubber floor base, color black. Rubber Cove base or similar.

5. Salient Characteristics Transition must be equal or better to the following:

a. 4” x 18mm black transition.

b. Shall be mounted to transition floor to existing flooring.

c. Shall be color black.

d. Shall provide a transition from 3/8” to 0”.

6. Salient Characteristics adhesive product must be equal or better to the following:

a. Floor roll/tile adhesive shall be PLAE Anchor Adhesive or equal or better.

b. Wall base adhesive shall be an approved adhesive for the manufacture of the floor base product.

Cove Base Tube adhesive or similar.

Dear Mr. Murdock, This letter is in regards to the paint samples dropped off at Satori Group’s office for analysis to determine lead content.

The paint samples were analyzed by Alan Caldwell using a Niton XLp X-Ray Florescence (XRF) analyzer.

Alan Caldwell is an EPA certified Lead Based Paint Risk Assessor in accordance with 40 CFR 745. The XRF analyzer irradiates the paint on a given surface causing the lead in the paint, if present, to emit a characteristic frequency of X-Ray radiation. The instrument identifies and counts these x-rays to determine a lead concentration. The intensity of the radiation is measured by the detector and is related to the amount of lead in the paint. The lead concentration results are report in milligrams per square centimeter (mg/cm2).

Test results for the sample submitted are listed below.

The bulk samples were shipped under Chain of Custody procedures to LA Testing in South Pasadena, California. LA Testing is fully accredited by the National Voluntary Laboratory Accreditation Program (NVLAP) for bulk asbestos fiber analysis.

Review of the enclosed results indicates all samples tested negative for asbestos.
