Attachment 001 Statement of Work (SOW)

Project: Device Evaluation and Service Plan

GENERAL INFORMATION

1.0 Scope of Work: Initial health check and options for repair and service plan for a Sony

Cell Sorter SH800 Serial Number 0309013.

2.0 Background: Plum Island has onsite a Sony Cell Sorter SH800 that requires full functionality for ongoing research. Plum Island is a USDA Biosafety Level 3 (BSL-3) facility located on island only accessible via government ferry.

CONTRACTOR REQUIREMENTS

3.0 Technical Requirements/Tasks:

3.1 CLIN 0001- To provide a one visit health check inspection and service report for a Sony Cell Sorter SH800 Serial Number 0309013. One visit inspection service covers all labor and travel expenses. Service report to be delivered to the USDA technical point of contact within five business days of inspection visit. Note section 4.0 below. Period of performance: 30 days after receipt of order (ARO.)

3.2 CLIN 0002 (Option Line) Upon USDA review and approval of health check service report defined in CLIN 0001, option line item may be exercised to action a one visit service repairs for any functionality concerns identified in the health check. Service repair visit covers all labor and travel expenses. Note section 4.0 below. Level-of-effort is defined by fixed hourly rate, total effort not to exceed 10 hours. Period of Performance: 45 days after option exercise.

3.3 CLIN 0003 (Option Line) Upon completion of repairs noted in CLIN 0002 or if no repairs are needed, implementation of a service plan for the subject device. The service plan covers all labor, parts, and travel expenses. The contractor will schedule repairs when notified the SH800 is not working properly. Level-of-effort is defined by fixed monthly rate, total effort not to exceed 11 months. Period of Performance: Upon option exercise date through 01/31/2025.

FAR 52.217-9 clause included.

4.0 Government Furnished: The government will furnish tools inside Plum Island BSL-3 for repairing the cell sorter. Any tools brought into the facility will not be returned to the service provider.

5.0 Travel: The contractor will be required to travel to Plum Island on a government furnished ferry departing at set times from either Old Saybook CT or Orient NY. Any visit will be a full day commitment as required government transportation in and out of the facility is limited to one workday. Plum Island Biosafety Level 3 protocols will apply to all contractor personnel while on site.

6.0 Contractor’s Key Personnel: The contractor shall supply a repair service technician that is a US citizen, meeting the 7.0 security requirements. See training requirements section 8.0.

7.0 Security Requirements: The service technician shall undergo a background security check to be completed at least 7 days prior to arriving on Plum Island. Plum Island will fund and conduct the background check. PII required of the technician for the background check: SSN, credit history, birthday, age, home address, proof of USA citizenship.

8.0 Training Requirements:

8.1 On site Biosafety Level 3 training will be required and Plum Island Biosafety Level 3 protocols will apply to all contractor personnel upon arrival on site.

8.2 The contractor must ensure their employees (including subcontractor personnel) complete the training course(s) listed below in the frequency identified. The contractor must maintain records of course completion and provide them to the Contracting Officer or

Contracting Officer Representative upon request.
