About this file

This file contains laboratory identification and subcontractor identification forms for a South Carolina Department of Health and Environmental Control (DHEC) UST Small Scope Assessment contract opportunity. The laboratory identification form requires offerors to verify that certified laboratories have the capability to analyze specific constituents across three categories: groundwater, drinking water, and soil. Groundwater analysis constituents include BTEXN with oxygenates and 1,2 DCA, dissolved lead, PAHs, lead, EDB, eight RCRA metals, and TPH. Drinking water constituents include BTEXNM plus 1,2 DCA, oxygenates and ethanol, EDB, and RCRA metals. Soil constituents include BTEXN, PAHs, eight RCRA metals, TPH-DRO, TPH-GRO, grain size/hydrometer analysis, total organic carbon, and miscellaneous soil analysis for lead. If a laboratory lacks capability to test any constituent, the offeror must identify an alternative certified laboratory, specify its location, and provide its certification number to maintain DHEC's chain of custody requirements.

The subcontractor identification form must be completed if the offeror intends to use subcontractors at any tier level for work exceeding 10 percent of total cost, involving government information access, or involving services critical to contract performance. The form specifically requests identification of driller subcontractors, including company name, address, contact person information, telephone number, LLR-issued license number, and a description of services to be provided. Both the laboratory identification and subcontractor identification forms are designated as material requirements and must be submitted with the bid proposal. The awarded contractor is responsible for ensuring all identified subcontractors utilize the qualified laboratories specified in the laboratory identification form.
