PERFORMANCE BASED WORK STATEMENT

FOR

Combat Medicine and Technical Rescue Specialist Training

31 RQS

Kadena Air Base, Japan

1. DESCRIPTION OF SERVICES.

The contractor shall provide all equipment, tools, parts, lodging, personnel, labor, supervision, and management to provide National Registry of Emergency Medical Technicians – Paramedic (NREMT-P) refresher and Technical Rescue Specialist (TRS) skills to Pararescuemen assigned to the 31st Rescue Squadron, Kadena AB, Okinawa Japan.

1.1. BASIC SERVICES. The contractor shall provide one (1) ten (10) day in person course of instruction for a maximum of 16 Pararescue personnel and the period of performance shall begin 03 January and end 14 Jan 2022. The course of instruction shall include the latest proven medical procedures, casualty treatment, and triage techniques currently utilized throughout today’s evolving battlespace. Course of instruction shall combine classroom academics, along with procedure-based training on fresh human tissue (FHT) simulation aids and Live Tissue Training (LTT) simulation aids provided by the contractor. The course of instruction shall include a National Registry of Emergency Medical Technicians-Paramedic (NREMT-P) refresher to recertify Pararescuemen accordingly. The course of instruction shall incorporate the tactical employment of medical treatment rendered by Pararescuemen through full mission profile (FMP) training. The contractor shall provide realistic training scenarios in which personnel will have the ability to utilize all medical skills taught throughout the course. These medical skills should follow the 9th edition Pararescue Medical Operations Handbook (January 2021). The course shall also be utilized to provide tactical instruction on sound employment of Close Quarters Battle (CQB), threat recognition and elimination, precious cargo recognition, recovery, and treatment, and scene security. The course will instruct technical objectives to include rope rescue (high and low angle), confined space, collapsed structure, and extrication scenarios. Participants will exercise their Technical Rescue Specialist (TRS) skills in full-mission-profile events. All FMPs will be generated to meet 5 and 7 level CFETP line items required for upgrade training. The contractor shall provide medical equipment and supplies to include all items listed in Appendix A.

1.2. TRAINING REQUIREMENTS

1.2.1. The objective of the training schedule is to provide realistic CMT and TRS training for up to 16 Pararescuemen (PJs). The training, which incorporates academics and scenarios, shall be applicable to the current areas of operation and injury patterns as well as provide the ability to train to the specific skills of the Pararescueman as technical rescue specialists. The training schedule will also provide a NREMT-P recertification for Pararescueman. There shall be a total of 10 training days per student. Each day shall end with debrief/lessons learned. All training will take place at the contractor’s owned and operated facility.

1.2.2. Instructor to student ratio for this 10-day CMT shall not exceed 1:6.

1.2.3. Patient model to student ratio shall not exceed 1:3 during CMT scenarios.

1.2.4. The contractor must be accredited by United States Special Operations Command (USSOCOM) as an approved Live Tissue Training provider.

1.3 Minimum Contractor Requirements

1.3.1. The CMT/TRS course will take place during a 10-day block. The course of instruction will include (at a minimum) the following topics:

1. Current concepts in Tactical Combat Casualty Care

2. Human anatomy review

3. Patient assessments

4. Mechanism of injury

5. Blast and burn injuries

6. Ocular trauma

7. Neck trauma

8. Chest trauma

9. Abdominal trauma

10. Crush injuries

11. Pediatric emergencies

12. Orthopedic emergencies

13. Hemorrhage control

14. Advanced/difficult airway emergencies

15. Respiratory emergencies

16. Shock

17. Environmental emergency considerations

18. Patient packaging

19. Critical care transportation

20. Intravenous and intraosseous fluid therapy

21. Improvised and field expedient medicine

22. Sick call medicine

23. Patient handoff

1.3.2. The TRS portion of the course will cover (at a minimum) the following topics:

1. Rope Rescue (High and Low Angle)

2. Solid Anchor selection and construction

3. Proper belay/rappel techniques

4. Safe mechanical advantage employment

5. Proper litter rigging (vertical and horizontal)

6. Heavy Lift and Extrication

7. Safe tool selection and employment

8. Confined space entry and operations

9. Threat recognition

10. Personal protective equipment considerations

11. Lightweight & minimalistic gear packing considerations

1.4. Instructor Minimum Qualification Requirements

1.4.1. Instructors must be experienced in medical and trauma skills as well as have extensive knowledge in the application of these skills in operational environments.

1.4.2. Licensed and practicing MD's and PhD’s must have oversight of instructors.

1.4.3. Instructors must have intimate knowledge of Pararescue and Combat Controller operations and skill sets.

1.4.4. Instructors must have prior operational experience as a combat medic, PJ or 18D.

1.4.5. All medical instructors must hold and maintain at least NREMT-P certification or higher.

1.5 Specific Contractor Requirements for CMT

1.5.1. Contractor shall provide all medical supplies required for training as well as the shipment of any unused medical equipment to the 31 RQS.

1.5.2. CMT simulation aids must be properly and legally procured.

1.5.3. CMT shall adhere to applicable testing for Hepatitis B, Hepatitis C and HIV and any other communicable diseases as required by state law and institutional policy, and the contractor shall provide relevant information about the results of testing for communicable diseases to the 31st RQS point of contact. All personnel who may come in contact shall be made aware of the positive test result and maintain necessary precautions to prevent disease transmission.

1.5.4. CMT simulation aids disposal must be in a manner consistent with donor intent and legal requirements.

1.5.5. Problems related to the conduct involving CMT use of human simulation aids must be promptly reported to the 31 RQS point of contact and the Contracting Officer and in accordance with Section 1.5.3 of this PWS. Examples of problems include, but are not limited to loss of confidentiality, breach of security, significant deviation from the approved protocol, failure to comply with state laws and/or institutional policies and public relations issues.

1.5.6. Contractor will provide confined space facilities to thoroughly exercise technical rescue skills to include at least 100 continuous feet of vertical confined space for advanced scenario training.

1.6. Deliverables

1.6.1. On the final day of instruction, a certificate of completion for each student passing the course shall be provided, along with documentation outlining the number of continuing medical education (CME) hours earned throughout the course. Upon completion Pararescueman will have been recertified as NREMT-Paramedics.

1.6.2. A comprehensive after-action report (AAR) of attending personnel’s performance and a summary report of all activities/training shall be submitted to the 31 RQS point of contact in electronic format no later than 30 days after the final training day. A brief description of the course activities and a brief description of the contractor’s involvement shall be included.

1.7. Applicable Documents and Reference. The contractor shall comply with the following:

1.7.1. Revised Uniform Anatomical Gift Act (UAGA), drafted by the National Conference of Commissioners of Uniform State Laws, 2006.

1.7.2. Laws or regulations applicable to the states and U.S. territories that govern the use of subject CMT activity and curriculum simulation aids.

1.7.3. Pre-Hospital Trauma Life Support/Tactical Combat Casualty Care (PHTLS/TCCC) protocols as established by the American College of Surgeons Committee on Trauma, the National Association of Emergency Medical Technicians.

1.8 Licenses and Permits. The contractor shall, without additional expense to the Government, obtain any necessary licenses and permits, and comply with any Federal, State, and local laws, codes and regulations applicable to this PWS. The contractor shall provide proof of such applicable licenses and permits upon submittal of their proposal and at any time on request by the Government. The contractor shall submit any applicable certifications.

1.9. Place of Performance. Contractor will provide all training facilities. The lodging must be in the vicinity of the training area and would not be additional charges to the government. The training facility shall possess restrooms and adequate classrooms on site. The facilities shall have a consolidated area for pre mission gear preparation, gear storage, and mission planning. The Government will provide transportation for all attendees.

2. SERVICES SUMMARY.

The Contractor service delivery requirements are summarized into performance objectives that relate directly to standards of performance required to meet mission essential needs. For the Performance Objective to be met, service delivery must be in substantial compliance with applicable performance standards. The Performance Threshold describes the minimum overall levels of service delivery required for acceptable quality control. Failure to meet these Performance Thresholds means that contractor Quality Control is unacceptable. The following is a list of the key performance objectives that will be verified as contractually compliant by government personnel; however, inspection of any contract requirement is authorized.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. Not applicable to this acquisition.

4. GENERAL INFORMATION.

4.1. Privacy and Security.

4.1.1. Reporting Requirements. Contractor personnel shall immediately report to an appropriate Government authority any information or circumstances of which they are aware may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.

4.1.2. Privacy Act. Work on this project requires that personnel have access to Privacy Act information. Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations.

4.1.3. Security. Work on this project requires all personnel having access; knowledge and/or involvement complete Non-Disclosure Agreements (NDAs). Personnel shall adhere to the provisions against sensitive information and/or information that effects national security under 18 U.S.C., Sec 1905 ("Trade Secrets Act") and 5 U.S.C. Secs 552(b)(1) and

(b) (4), which prohibit the release of sensitive information trade and national security information, in addition to the provisions under Executive Orders 13526 and 12356 ("National Security information)

4.1.4. Contract Manager. The contractor shall designate a Point of Contact that's responsible for the performance of the work. The name of this person, and an alternate, who shall act for the contractor when the manager is absent, shall be designated in writing to the contracting officer prior to the contract start date. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract and have contract authority to provide the services and obligate the contractor.

4.1.5. The contractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the Installation and its population.

4.1.6. The contractor shall not employ any person who is an employee of the US Government if employing that person would create a conflict of interest. Additionally, the contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to Department of Defense (DoD) Regulation 5500.7R, Joint Ethics Regulations (JER). The contractor shall not employ any person who is an employee of the Department of the Air Force if such employment would be contrary to the policies in AFI 64-106, Air Force Industrial Labor Relations Activities.

5. QUALITY ASSURANCE.

Not applicable to this acquisition.

6. HOURS OF OPERATION.

6.1. Period of Performance. The following is the projected training course date:

10-day Training Course: 3-14 January 2022 (80 hours)

7. SAFETY REQUIREMENTS.

In performing work under this contract, the contractor shall:

7.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work.

7.2. Provide protection to government property in the form of a lockable storage area that is only accessible by the lead instructor and flight leadership during the course of instruction to prevent damage during the period of time the property is under the control or in possession of the contractor.

7.3. Develop and provide at the start of the orientation period or the start of the first operational performance period (if there is no orientation period) a safety plan for the protection of government facilities and property and to provide a safe work environment for contractor personnel.

7.4. Provide protection to government property to prevent damage during the period of time the property is under the control or in possession of the contractor.

7.5. Include a clause in all subcontracts to require subcontractors to comply with the safety provisions of this contract as applicable.

7.6. Record and report promptly (within one hour) to the Contracting Officer or designated government representative (GR), all available facts relating to each instance of damage to government property or injury to either contractor or government personnel.

7.7. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the contracting officer.

7.8. If the government elects to conduct an investigation of the accident/mishap, the contractor shall cooperate fully and assist government personnel in the conduct of investigation until the investigation is completed.

7.9. Include a clause in each applicable subcontract requiring the subcontractor's cooperation and assistance in accident reporting and investigation.

8. CONTINUATION OF ESSENTIAL DOD CONTRACTOR SERVICES DURING

CRISIS

This service is determined to be non-essential for performance during crisis according to DODI 3020.41, Operational Contract Support, paragraph 2.c. In the event of crisis, the contractor will be notified by the Contracting Officer of the need to discontinue services due to contingency, base closure or other causes. If notification to discontinue services is not received, the contractor will be expected to continue performance in accordance with this PWS.

Medical Supply List: This list of medical supplies is to supply the training events. This training equipment is required due to 31st RQS medical inventory being unable to be restocked by the course dates due to continuing resolution funding timing paired with deployments. Clarification of items on this list can be given by contacting the primary POC at the contact information listed.
