1.0. DESCRIPTION OF SERVICES: This is a non-personnel services contract to provide aerobic instruction. The Contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to provide aerobic instruction services. The Contractor shall provide classes at the Domenici Fitness & Sports Center (DFSC) and Outdoor Sports Complex (OSC) located on Holloman Air Force Base (HAFB), NM.

1.1. Scope of Work: The Contractor shall provide up to and on average 25 aerobic classes per week of varying intensities. Instruction shall include, but is not limited to, Zumba, Spin, Yoga, Body Pump, Power Yoga, and Cardio Strength. Classes range from one to over 45 participants and shall be conducted indoors and outdoors.

1.2. General Requirements:

1.2.1. Hours of Operation: The Contractor shall perform services required Monday through Sunday during DFSC operating hours stated in paragraph 4.3. Services are not required on Federal Holidays.

1.2.2. Contractor shall comply with COVID-19 DoD, HAFB, and DFSC directives. Contractor will follow instructions post COVID-19 “Re-Opening” Guidelines for Group Exercise Classes: Section Capacity, Upon Arrival to Session, Wellness Screen Physical Distancing and Sanitation. Contractor will adhere to COVID-19 “re-opening” of CrossFit Holloman. Guidelines will be provided by the COR.

1.2.3. Contractor shall present a neat and professional appearance IAW Wing Fitness Center Dress Code.

1.3. Minimum Qualifications:

1.3.1. Contractor personnel shall possess a current, valid, and maintain a primary instructor certification by a DoD recognized organization IAW AFI 34-266, Fitness, Sports, and World Class Athlete Program, paragraph 3.2.7 (Appendix C). Instructors must obtain the additional necessary specialized training and certifications for specialized classes.

1.3.2. Contractor personnel shall maintain first aid certification and either the Basic Lifesaving automatic external defibrillator or the cardiopulmonary resuscitation certification through American Heart Association or American Red Cross.

1.4. Class Schedule: The Contractor shall develop and implement monthly schedules which shall be submitted to the COR no later than the fifth working day of the previous month. The COR will inform the contractor if less than 25 classes per week are required for the upcoming schedule. COR will review class participation quarterly and may request Contractor to replace a class with low participation.

1.5. Specific Tasks:

1.5.1. The Contractor shall sign in the Group Exercise Instructors logbook at the front counter of the DFSC before each class. The Contractor will receive a patron “Sign-in Sheet” for each class. The Contractor shall ensure patrons complete the information on the sheet. At the end of each class the sign-in sheet must be placed in the “Contractor Folder” in the Operations Office.

1.5.2. The Contractor shall hold classes indoors or outdoors. Outdoor areas for aerobics classes are the outdoor basketball court, PT Pad (Alpha Warrior Rig) and pavilion by the outdoor basketball court

(Appendix D). Contractor will be under no obligation to instruct aerobic classes held outside if the Contractor determines "unsafe” or “unhealthy” aerobic environment based off of the Heat Index and Wet Bulb Globe Temperature.

1.5.3. The Contractor shall notify the COR of any disputes or problems with participants in the aerobics instruction classes.

1.5.4. The Contractor shall notify the COR immediately of any injury which occurs and assist in completing AF Form 978.

1.5.5. The Contractor shall contact the COR at least 24-hours in advance of any Contractor initiated cancellations. Contractor cancellations shall not exceed two per month.

1.5.6. The Contractor shall be required to give attendees a grace period up to 10 minutes past class start time before cancelling due to no participants.

1.6. Deliverables:

1.6.1. Certifications: Contractor shall provide the COR copies of required certifications for Contractor personnel listed in paragraph 1.3 before performance.

1.6.2 Schedule: The Contractor shall submit the monthly schedule to the COR no later than the fifth working day of the previous month.

1.6.3. Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office IAW Federal Acquisition Regulation Subpart 42.5. The CO, COR, and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the CO will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.

1.6.4. Contractor Level I Antiterrorism Awareness Training:

Each contract employee must accomplish Antiterrorism Awareness (AT) Training as stated in DFARS clause 252.204-7004, Level 1 Antiterrorism Awareness Training for Contractors. This training provides contractor employees with the requisite knowledge necessary to remain vigilant for possible terrorist threats. Coordination for training should be done by the requiring Antiterrorism Representative (ATR) after contract award. Tracking for each contractor or subcontractor employee is the responsibility of the COR or unit ATR. Contractor employees are required to complete Level I antiterrorism awareness training within 30 days of requiring access and annually thereafter. AT certificates for each contract employee must be submitted to the COR or unit ATR. Level I AT awareness training can be taken at JKO https://jkodirect.jten.mil/Atlas2/faces/page/login/Login.seam for non-common access card holders.

3.0. GOVERNMENT AND CONTRACTOR FURNISHED SERVICES:

3.1. Government Furnished Items and Services:

3.1.1. The Government will provide electricity, water and sewer service.

3.1.2. The Government shall provide use of the following equipment: Exercise and strength training equipment available at the DFSC to perform aerobic exercise instruction. If additional items are needed the Government will provide temporary storage to the Contractor for items necessary to perform aerobics instruction classes.

3.1.3. Law Enforcement and Fire Protection services are provided to the Contractor while performing service on HAFB. If required the Contractor shall call the following numbers: Security Forces (575) 572-

7171 and Fire Department, 911. The Contractor shall inform the CO if one or both of these agencies are contacted.

3.1.4. The Government shall provide a portion of a bulletin board for marketing Contractor’s information and publicize the service with the appropriate marketing techniques.

3.2. Contractor Furnished Items and Services:

3.2.1. The Contractor shall furnish everything required to perform the services required except for the items specifically stated in Section 3.0.

3.2.2. All equipment, materials, and services supplied by the Contractor shall conform to applicable safety and health standards as established in Title 29 of the Code of Federal Regulations Part 1910, Occupational Safety and Health Standards.

4.0 GENERAL INFORMATION:

4.1. Contractor Quality Control (QC) Plan. The Contractor is responsible for Quality Control (QC).

The Contractor shall develop QC procedures via a Quality Control Plan (QCP). At a minimum, the plan shall include the following:

4.1.1. Document should be on contractor letterhead or include Contractor Name, Address, Phone Number & Email.

4.1.2. The Contract Number (located in Block 2 of the SF1449) shall be included.

4.1.3. The plan shall define the role of management.

4.1.4. The plan shall provide an organizational flow chart (can be very simplistic) and identify the individual(s) who will be performing quality control checks.

4.1.5. The plan shall address contractor's inspection system that will be used to cover the services stated in the Service Summary (Section 2.0) of PWS.

4.1.6. The plan shall identify contractor's method(s) for:

identifying defects in the quality of how services are performed:

o identifying how to prevent defects in the quality of service;

o state how founded defect(s) are recorded;

o include preventative measures to preclude recurrence;

o Describe the method for identifying and recording corrective actions taken

4.1.7. The plan shall address specific tasks of the contract requirement, not just items listed under Section

2.0. If using random and/or periodic performance checks, please outline. Include any written work instructions, procedures, processes or checklists used to meet the contract terms.

4.1.8. The plan shall address how company recruits and maintains employees to ensure no lapse of service.

4.1.2. Government Review and Acceptance. The initial QC Plan shall be submitted to the CO for

Attachment 1 – PWS Page 7 of 15 acceptance no later than ten (10) days after award. The Government will review the Contractor’s proposed QC Plan, and if found to be acceptable, will provide written approval to the Contractor. If the plan is unacceptable, the unacceptable areas will be identified to the Contractor in writing. The Contractor shall correct the unacceptable areas and submit a revised plan within ten (10) calendar days.

The Contractor shall perform required services in accordance with the Government accepted plan to facilitate Government quality assurance processes.

4.1.3. Revision. If the Contractor desires to revise the plan at any time during the life of this contract, the proposed revisions shall be submitted to the Contracting Officer for review and acceptance. The Contractor shall not implement any revised plan until the Government has approved the revised plan in writing.

4.1.4. The COR will periodically assess the contractor's performance through intermittent on-site inspections to ensure services are received not only in accordance with (IAW) the contract/PWS, but also IAW the Contractor's QCP and from any receipt of complaints from base personnel. Government inspection rates and/or methods may vary throughout the life of the contract to ensure standards are maintained. The Contractor shall be responsible for providing its own QC inspection system, and shall not rely upon the Government's QA program to identify performance defects.

4.2. Security Requirements: Project is unclassified – access to classified information not required.

4.2.1. Illegal Aliens. The contractor shall not hire an illegal alien to work or perform any service on HAFB. Employees who are not U.S. citizens can only be persons possessing a current resident alien immigrant status (Green Card) or if a non-resident immigrant must possess a current Employment Authorization Document. While on HAFB, non-U.S. citizen employees must be able to produce for inspection the appropriate credential: Green Card or Employment Authorization Document. Any illegal alien found on the installation shall be immediately detained by the base security forces and barred from the installation.

4.2.2. Listing of Employees. The contractor shall maintain a current listing of employees that require access to HAFB. The list shall include the employee's name, social security number and type of investigation if contract work involves unescorted entry to Air Force (AF) restricted areas or other sensitive areas designated by the installation commander. The list shall be provided to the Program Manager and sponsoring agency security assistant. An updated listing shall be provided when an employee's status or information changes (i.e. terminated for cause, reassignment, retirement, etc.) or when a new employee is added.

4.2.3. Security Training. The contractor ensures all employees receive initial and recurring (annual) security training from the sponsoring agency security assistant. Training must be conducted IAW DODM

5200.01 DODM 16-1404 Vol 1, 2 &3, DoD Information Security Program, and Air Force Information Security Program. Contractor personnel who work in AF controlled/restricted areas must be trained IAW AFI 31- 101, Integrated Defense. The sponsoring agency controlled area monitor will provide this training when required.

4.2.4. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance are obtained for employees. Employees shall only be granted access to the installation to perform functions directly related to their respective contract.

(1) All contractor employees requiring access to HAFB shall be identified on a HAFB visit Access Request (VAR) form. The contractor shall submit a completed VAR through the Contracting Officer to 49 SFS Pass and Registration (49 SFS/S5AV) NLT 7 duty/business days, and for foreign nationals, 14 duty/business days, prior to requiring access to the installation.

4.2.5. Retrieving Identification Media. The contractor shall return all identification media from employees who depart for any reason before the contract expires; i.e., terminated for cause, reassignment, retirement, etc. The contractor will notify the Government within 24 hours when unable to retrieve identification media from contract employees.

4.2.6. Travel Restrictions: Contractors are prohibited in areas of the installation other than their appropriate place of work and the routes to and from installation entry/exit points.

4.2.7. Traffic Laws. The contractor and its employees shall comply with base traffic regulations. Traffic regulations include, but are not limited to:

(1) Employees shall wear a seatbelt when operating or riding in any military, contractor-owned, or privately-owned vehicle while on the installation.

(2) Speed limits on HAFB are 30 MPH unless otherwise posted. Be aware of reduced speed limits and exercise caution when driving in base housing and school zones.

(3) All vehicle operators on the installation shall not use cell phones unless the vehicle is safely parked or unless they are using a hands-free device. The wearing of any other portable headphones, earphones or other listening devices (except for hand-free cellular phones) while operating a motor vehicle is prohibited.

4.2.8. Weapons, Firearms, and Ammunition. Employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within military, contractor or privately-owned vehicles while on HAFB.

4.2.9. For Official Use Only (FOUO). The contractor shall comply with DoDM 5200.01-V4/E3, DoD Information Security Program: Controlled Unclassified Information (CUI), requirements. This manual sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.2.10. Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DOD personnel, contractor personnel, resources, and classified or unclassified defense information.

4.2.11. Physical Security. The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment, and materials shall be secured.

4.3. Hours of Operation:

4.3. The Contractor shall perform services during DFSC hours of operation.

4.3.1. Operating hours at the DFSC are Monday through Friday from 0500 to 2100 and Saturday and Sunday from 0800 to 1400 on, excluding federal holidays, family days, and base closures.

4.3.2. Family Day hours of operations are from 0800 to 1400. Days of observance can vary. A Family Day schedule will be provided by the COR.

4.4. Recognized Holidays: Contractor is not required to perform services on holidays.

*If the holiday falls on a Saturday, it will be observed on the preceding Friday. If the holiday falls on Sunday, it will be observed on the following Monday.

4.5. Use of Government Employees: The contractor is cautioned that off-duty active military personnel hired under this contract may be subject to permanent change of station, change in duty hours, or deployment. Military Reservists and National Guard members may be subject to reorder to active duty.

The abrupt absence of these personnel could adversely affect the contractor's ability to perform. However, their absence at any time shall not constitute an excuse for nonperformance under this contract.

4.6. Records Management, Freedom of Information Act (FOIA) and Privacy Act Requirements (March 2020):

4.6.1. Records Management (RM). IAW AFI 33-322, Records Management and Information Governance Program – Records (section 1.3.6) the Contractor shall provide accountability and ensure all records and documents generated during the period of this contract are made available to the Government at all times. Records relating to work that contractors perform are maintained according to AFI 33-322- Records Management and Information Governance Program, Air Force Records Information Management System (AFRIMS RDS), Federal Records Management (https://www.archives.gov/records-mgmt/policy/records-mgmt-language), the Federal Acquisition Regulation, and the Defense Federal Acquisition Regulation Supplement, as applicable. When contractors create data for the government’s use, the contract must identify the background data that contractors must deliver to the government for possible reuse. Before identifying background data that contractors must deliver to the government, program and contracting officers must coordinate with stakeholders. These include records and information managers, historians and other United States Government agencies as appropriate. This is to ensure that all government needs are met, particularly when the data supports new missions or programs.

When the background data includes records, the contractor must deliver enough technical documentation to allow the Air Force or other agencies to use the data. Manage all data created for government use or legally controlled by the government according to the records management procedures in this instruction.

4.6.2 Freedom of Information Act (FOIA) and Privacy Act (PA). The Contractor shall comply with DoDM 5400.7_R, DoD Freedom of Information Act (FOIA) Program, DoD5400_7-R_AFMAN33-302, Freedom of Information Act Program, and AFI 33-332, Air Force Privacy and Civil Liberties Program.

These regulations set policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding Personally Identifiable Information (PII) material.

4.6.3 Contractor Records. The Contractor shall maintain all government records IAW AFI 33-322, Attachment 1 – PWS Page 10 of 15

Records Management and Information Governance Program – Managing Records (Chapter 3), in order to support the COR who is responsible for all lifecycle record management and all duties assigned to the organization's Record Professional (RP) (Tier-1 (T-1)).
