STATEMENT OF WORK
Patrick Space Force Base CATM (Gun Range) Exhaust Duct Replacement

Abstract
Required Statement of Work (SOW) PSFB CATM Exhaust Duct Replacement
Statement of Work
1.0 DESCRIPTION OF SERVICES.
The required services will take place on Patrick Space Force Base (PSFB) B9693 CATM (Gun Range). Scope of work requested is the removal and installation of ductwork attached to exhaust fans 1 & 2. Current ductwork is deteriorated to due wore/ tear from vibration of the unit and the corrosive marine environment in which B9693 is located (Refer to pics section 3). The scope of this project is to remove the existing ductwork of both exhaust fans, install new duct work to include vibration eliminators and weather proof insulation.
2.0 SERVICES SUMMARY.
2.1 Utilizing industry means and methods, licensed contractor provides all lift equipment, tools, trained labor and material to perform services that is being requested. The scope of work should be accomplished within 30 days by the contractor. Due to mission requirements this project requires at least exhaust fan operational during the renovation.
2.2 Site Set up. Place dumpster, necessary equipment, barrier on site.
2.3 Lock out/Tag out Exhaust Fan 1.
2.4 Remove/ Demo all duct work (estimated 60’, combine total of 120’ for both units), stands, access doors and louvers/vents that is connected from exhaust fan 1 (refer to fig. 1) to the facility (refer to fig. 2)

2.5 Inspect entry connections to both exhaust fans opening and openings to facility.
2.5.1 If necessary make any repairs/adjustments of opening to allow for proper fit of new duct work connections.
2.6 Inspect concreate slab
2.6.1 If necessary make any repairs/adjustments to properly secure duct work.
2.7 Utilizing trade specific materials (metal, seals, etc.), install new duct work to include:
2.7.1 Connect duct work from exhaust fan to facility.
2.7.1.1 Electrical ground unit to earth.
2.7.2 Secure ductwork with appropriate supports (i.e. Hurricane straps, and stand support)
2.7.3 Install accessible insulated door access
2.7.4 Install vibration eliminators throughout ductwork
2.7.5 Install if applicable louvers/vents.
2.7.6 Insulate the exterior duct work with weather proof material.
2.9 Verify operation
2.10 Complete CFM report of exhaust fan demonstrating proper CFM ouput.
2.11 Clean up site
2.12 Repeat Steps 2.2 - 2.12 for exhaust fan 2.
3.0 STATE OF CURENT EXHAUST DUCTWORK

4.0 GOVERNMENT FURNISHED PROPERTY (GFP) AND SERVICES.
The Government will provide power source, water, and access to facilities.
5.0 GENERAL INFORMATION:
5.1 LOCATION AND HOURS OF WORK.
Services contained in this SOW requires work at the following locations:
All work will be performed on hours; 0700 hrs to 1700 hr, Monday through Friday.
Period of performance for this project is 30 days. Reporting to and from work should include the necessary time of scheduling of vehicle inspections at both installations.
5.2 BADGING AND IDENTIFICATION.
All contractors to include their direct employees, subcontractors and subcontractor employees entering both Patrick Space Force Base and Cape Canaveral Space Force Station are required to have an individual badge prior to beginning work at the facilities.
Therefore it is essential the contractor completes 45th SFS Credential request form SFS V10 (Subject to Privacy Act of 1974) visitor’s information section.
All employees to include sub-contractors employees who will be working on PSFB and CCSFS must provide the completed forms to the Contracting Officer Representative (COR) within 15 days of being awarded the contract.
Once the COR receives the 45 SFS credential request form it will take approximately 3 Federal working days before a short term entry badge can be prepared. The contractor will be notified by the COR when each employee is to report to facility 577 badge and identification office for photographs and issuance of a badge.
All employees must bring TWO (2) forms of identification and all vehicles cleared with updated Florida registration and motor vehicle insurance.
5.3 Safety and Personal Protective Equipment (PPE).
Contractor shall comply with all applicable Federal, State and local legal requirements regarding workers health and safety. The requirements include but are not limited to those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR) Parts 1910 and1960. Contractor is solely responsible for determining the legal requirements that apply to activities, and shall ensure safe and healthful working conditions for its employees. The Act’s General Duty Clause, Section 5(a) (1), requires employers to provide their employees with a workplace free from recognized hazards likely to cause death or serious physical harm. The Occupational Safety and Health Act requires employers to comply with safety procedures providing fall protection--personal protective equipment not limited to gloves, hard hat, and safety toe shoes.
