RESPONSE REQUIRED TO BE CONSIDERED FOR AWARD
SAM (System for Award Management) Entity
SAM replaces and consolidates CCR (Central Contractor Registration), ORCA (Online Representations and Certifications) and other systems. Quoters must have a DUNS Number and an Active SAM Entity to receive award.
1. Quoters with an Active SAM Entity shall provide their DUNS Number.
2. Quoters without an Active SAM Entity shall:
a. Fill out FAR Provision 52.212-3, Offeror Representations and Certifications –
Commercial Items, and return it completed as part of the quote. The Provision has been provided in full-text in the Solicitation Provisions section. (FAR 52.212-3 does not supersede a SAM Entity.)
b. Quoters may submit offers before SAM Entity Registration/Activation is complete, but are advised that Activation takes 1-2 weeks to process after Registering and that award may follow as soon as one day after the Solicitation closes. Quoters without an Active
SAM Entity cannot receive award.

Response to Evaluation Factors (Technical and Price Criteria)
All Quoters must respond to all Evaluation Factors as described in the Evaluation Factors section (see
Solicitation Provisions). Incomplete quotes may not be considered.
Site Visit (Pre-Quote)
No pre-quote site visit is planned, however, Quoters are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. (2) While not required, the contractor may include in its subcontracts for commercial items a minimal number of additional clauses necessary to satisfy its contractual obligations.
FAR 52.236-7 PERMITS AND RESPONSIBILITIES (NOV 1991)
The Contractor shall, without additional expense to the Government, be responsible for obtaining any necessary licenses and permits, and for complying with any Federal, State, and municipal laws, codes, and regulations applicable to the performance of the work. The Contractor shall also be responsible for all damages to persons or property that occur as a result of the Contractor’s fault or negligence. The Contractor shall also be responsible for all materials delivered and work performed until completion and acceptance of the entire work, except for any completed unit of work which may have been accepted under the contract.
FAR 52.236-13 ACCIDENT PREVENTION (NOV 1991)
(a) The Contractor shall provide and maintain work environments and procedures which will—
(1) Safeguard the public and Government personnel, property, materials, supplies, and equipment exposed to Contractor operations and activities;
(2) Avoid interruptions of Government operations and delays in project completion dates; and
(3) Control costs in the performance of this contract.
(b) For these purposes on contracts for construction or dismantling, demolition, or removal of improvements, the Contractor shall—
(1) Provide appropriate safety barricades, signs, and signal lights;
(2) Comply with the standards issued by the Secretary of Labor at 29 CFR Part 1926 and 29 CFR
Part 1910; and
(3) Ensure that any additional measures the Contracting Officer determines to be reasonably necessary for the purposes are taken.
(c) If this contract is for construction or dismantling, demolition or removal of improvements with any
Department of Defense agency or component, the Contractor shall comply with all pertinent provisions of the latest version of U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, in effect on the date of the solicitation.
(d) Whenever the Contracting Officer becomes aware of any noncompliance with these requirements or any condition which poses a serious or imminent danger to the health or safety of the public or Government personnel, the Contracting Officer shall notify the Contractor orally, with written confirmation, and request immediate initiation of corrective action. This notice, when delivered to the Contractor or the Contractor’s representative at the work site, shall be deemed sufficient notice of the noncompliance and that corrective action is required. After receiving the notice, the Contractor shall immediately take corrective action. If the Contractor fails or refuses to promptly take corrective action, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken. The Contractor shall not be entitled to any equitable adjustment of the contract price or extension of the performance schedule on any stop work order issued under this clause.
(e) The Contractor shall insert this clause, including this paragraph (e), with appropriate changes in the designation of the parties, in subcontracts.

FAR 52.246-4 INSPECTION OF SERVICES – FIXED PRICE (AUG 1996)
(a) Definition. “Services,” as used in this clause, includes services performed, workmanship, and material furnished or utilized in the performance of services.
(b) The Contractor shall provide and maintain an inspection system acceptable to the Government covering the services under this contract. Complete records of all inspection work performed by the Contractor shall be maintained and made available to the Government during contract performance and for as long afterwards as the contract requires.
(c) The Government has the right to inspect and test all services called for by the contract, to the extent practicable at all times and places during the term of the contract. The Government shall perform inspections and tests in a manner that will not unduly delay the work.
(d) If the Government performs inspections or tests on the premises of the Contractor or a subcontractor, the
Contractor shall furnish, and shall require subcontractors to furnish, at no increase in contract price, all reasonable facilities and assistance for the safe and convenient performance of these duties.
(e) If any of the services do not conform with contract requirements, the Government may require the
Contractor to perform the services again in conformity with contract requirements, at no increase in contract amount. When the defects in services cannot be corrected by reperformance, the Government may—
(1) Require the Contractor to take necessary action to ensure that future performance conforms to contract requirements; and
(2) Reduce the contract price to reflect the reduced value of the services performed.
(f) If the Contractor fails to promptly perform the services again or to take the necessary action to ensure future performance in conformity with contract requirements, the Government may—
(1) By contract or otherwise, perform the services and charge to the Contractor any cost incurred by the
Government that is directly related to the performance of such service; or
(2) Terminate the contract for default.

CONTRACT DOCUMENTS
DESCRIPTION OF WORK:
The purpose of this task order is to secure stand exam data in the Snow Mountain Project area. The
Contractor shall locate and collect data for 700 stand exam inventory plots. Work under this task order includes:
a. Establishing plots within each unit.
b. Collecting stand exam and surface fuel data within each plot.
c. Providing inventory data in electronic file format for Field Sampled Vegetation (FSVeg) upload.
The sample design is focused on gathering tree, vegetation, and surface fuel data. The data collection procedures shall follow the protocol outlined in the Common Stand Exam (CSE) Users Guide, February
2014 (or current version), the CSE Region 5 User Guide Appendix, the CSE Region 5 Common Stand
Exam Field Guide, and the General Technical Report (GTR) INT-16, Handbook for Inventorying
Downed Woody Material by James K. Brown (see attachment) unless specifically modified in the contents of this Contract. Data collection protocol consists of:
Tree data collection will be at the “extensive” level.
The vegetation composition collection will be “quick” for shrub species and presence of grasses and forbs.
The surface fuels data will be collected using a 28 foot planar intersect technique.
The Contractor shall collect data necessary to complete the following forms: Setting Data Form, Plot
Data Form, Sample Design Form, Tree Data Form, Vegetation Composition Form, and Down Woody
Material Form. The Contractor shall furnish all labor, equipment, supervision, transportation, supplies, materials, services and any incidentals necessary to perform the work in accordance with the attached specifications. Completion of Item 1 is required by March 31, 2016.
PERIOD OF PERFORMANCE
Work is estimated to begin on June 1, 2015 and continue through March 31. 2016.
The Contractor will be required to (a) commence work under this contract within 10 calendar days after the date the Contractor receives the notice to proceed and (b) prosecute the work diligently.
PROJECT LOCATION:
The Snow Mountain Project area is located in Butte and Plumas Counties, California, on the Almanor
Ranger District of the Lassen National Forest, approximately 20 air-miles south-southwest of Chester, CA. The legal description is T. 25 N., R. 4 E., Sections 1, 2, 11, and 12; T. 25 N., R. 5 E. Sections 1–11, and 16; and T. 26 N., R. 5 E., Sections 20–22 and 26-35, Mount Diablo Meridian. See also the vicinity and project area maps (attachments).
From Chester California:
Travel 2.2 miles west on State Highway 36 to State Highway 89.
Turn south on CA 89 and travel 4.3 miles to Plumas County road 308.
Turn south (right) onto PL 308 and travel 0.6 miles to Plumas County road 309.
Continue straight on PL 309 for 1.1 miles to Plumas County road 307.
Turn west (right) onto PL 307 and travel 16.6 miles to county boundary at which point the road becomes Butte County road 91513.
Continue straight on BU 91513 and travel 3.1 miles, crossing Butte Creek, to Forest road 26N75 at the northern boundary of the project area.
Additionally, continue on BU 91513 for 6.2 miles to Butte County road 92523 to access the southern boundary of the project area.
From Chico California:
Travel 28 miles east on State Highway 32 to Butte County road 91422.
Turn east (right) onto BU 91422 and travel 5.3 miles to Butte County road 51262 at Butte
Meadows.
Turn south (right) onto BU 51262 and travel 6.1 miles to Butte County road 91513.
Turn east (left) onto BU 91513 and travel 1.9 miles to Butte County road 92523 (road to
Philbrook Lake.
Turn south (right) onto BU 92523 and travel 0.5 miles to the western boundary of the project area at the West Branch of the Feather River. At this point BU 92523 is part of the project area southern boundary.
Continue on BU 92523 for 2.6 miles to Forest road 25N05. Road 25N05 is the rest of the project area southern boundary.
WORK METHODS AND STANDARDS
1. SPECIFIC WORK REQUIREMENTS:
The Contractor shall locate and collect data for stand exam inventory plots. Stand exam plot installation and data collection shall follow Common Stand Exam protocol and GTR INT-16. The Common Stand
Users Guide and region specific field guides may be found at http://www.fs.fed.us/nrm/fsveg/. GTR
INT-16 is provided as an attachment. Data shall be entered into Exams PC program version v11.02.2014, or current version, using the Government provided stand exam template. The common stand exam software, Exams PC/Exams PDR, is available from the above website. The Contractor’s electronic data will run through the edit program by the Contractor without fatal errors and load successfully into
FSVEG by the government or it will be returned for rework.
2. ACCESS:
Access to plot locations may be determined from topographic or aerial photo maps. Maps of stand and plot locations set against topographic map and aerial photo backgrounds will be furnished by the Forest
