Site Inspected: Residence #1004 in Hayfork, Ca.
Date Of Inspection: May 2, 2014
A visual inspection was conducted of the above residence known as #1004 in Hayfork, Ca. on May 2, 2014 by Laurie Warren - Certified Site Surveillance Technician (C.S.S.T. #
12-4934) The inspection of the building was conducted in accordance with EPA AHERA standards.
All suspect material was touched and sampled to determine asbestos content as well as friability.
All homogeneous areas of friable and non-friable ACM were identified. This inspection was conducted in accordance with EPA, CAL/OSHA and local regulatory guidelines. Sample results are attached. Samples were sent to Schneider Laboratories in Richmond, Virginia for analysis.
Asbestos content shall be determined using the method specified in Appendix A, Subpart F, 40
CFR Part 763, Section 1, Polarized Light Microscopy. If the asbestos content is less than 10%, verification shall be made using the point counting method specified in Appendix A, Subpart F, 40
CFR Part 763, Section 1,7,2,4 Polarized Light Microscopy, Quantification of Asbestos Content.
There were two positive samples for asbestos in this residence #1004.
Sample #9 was taken of the bedroom by the bathroom closet flooring. There are two layers of asbestos containing material in this closet area.
Sample #12 was taken of the linoleum flooring in the bathroom. It is unknown which layer of linoleum is asbestos containing and as such, all layers in the bathroom shall be considered asbestos containing.
Exterior surfaces were not sampled as part of this inspection.
Only a Department of Occupational Safety and Health Administration certified asbestos abatement contractor shall be allowed to perform this work. Clearance sampling shall be performed on any modernization project in this unit involving the removal of asbestos.
