Addendum to FAR 52.212-1 Instructions to Offerors—Commercial

A. Offerors are required to submit proposals in 3 volumes and shall be in the format provided in this addendum, failure to submit proposals in the required format may render the proposal “non-responsive” and may not be considered for award. Each Volume shall be separated within the proposal package and should not be a continuation of the previous Volume. Proposals can be issued on offeror’s letterhead and must include the solicitation number. Proposals that exceed page restrictions may not be considered for award; a cover page and summary may be included and do not count against page restrictions.

1. Volume I, Technical Approach (25 page maximum)

a. Volume I shall contain 2 sections; Section A – Technical Approach and Section B –

Quality Control

b. Volume I - Section A, Technical Approach, shall contain the following information:

TECHNICAL PROPOSAL

i. Organizational Experience. Experience is the opportunity to learn by doing. The

Government will evaluate each offeror's organizational experience on the basis of its breadth, its depth, and its relevance to the work that will be required under the prospective contract. The Government will not evaluate an offeror's organizational experience on the basis of the personal experience of the offeror's key personnel. However, the Government will consider the extent to which the offeror's key personnel have worked together in the past. The Government will evaluate the organizational experience of the offeror's proposed key subcontractors.

ii. Understanding of the Government's Requirements. The Government will evaluate each offeror's relative understanding of the Government's requirements on the basis of its written submittals addressing the following subfactors:

1. Any information that indicates the extent to which the site for this project was visited.

2. The results concluded from the site visit.

3. The resources (personnel and equipment) which are planned for use on the project, and how realistic that plan is given concurrent obligations.

4. Site-specific safety plan.

5. Plan of work.

6. Where applicable, estimated production rates.

c. Volume I - Section B, Quality Control Plan, shall contain the following information:

Quality Control: Describe your method of self-inspection and means of insuring that all contract specifications are met without repeated rework and without reliance upon Government inspections.

CONTRACTOR QUALITY CONTROL PLAN. In accordance with FAR 52.246-4

Inspection of Services -- Fixed-Price (b), the Contractor shall identify the quality control inspection system it will use to ensure that contract performance specifications will be achieved. This Quality Control Plan shall be used by the contractor to monitor its performance and at no time shall the contractor rely upon Government inspections to provide notification of unsatisfactory performance. The Contracting Officer may observe the Contractor's inspection at any time and shall otherwise have unlimited access to the inspection data.

QUALITY ASSURANCE PLAN/REQUIREMENTS and DISINCENTIVES: The

Contractor shall provide a Quality Control Plan that will ensure that the work is being performed in a manner that will meet the objectives of the contract. The Government will, at its convenience, make periodic inspections of the work to assure itself of contract compliance.

- Contractor Quality Control Inspection. The Contractor shall ensure that performance meets contract specifications in accordance with the Quality Control Plan, prior to requesting the Government to inspect for payment or acceptance.

2. Volume II, Past Performance (15 Page maximum)

Record of Past Performance. Organizational Past Performance. Past performance is a measure of the degree to which an offeror satisfied its customers in the past and complied with Federal, state, and local laws and regulations. The Government will contact some of each offeror's customers to ask whether or not that: (1) that the offeror was capable, efficient, and effective; (2) the offeror's performance conformed to the terms and conditions of its contract (specifications); (3) finished within the contract time; (4) the offeror was reasonable and cooperative during performance; and (5) the offeror was committed to customer satisfaction. In evaluating past performance, the Government will contact some of the references provided by the offeror and other sources of information, including, but not limited to: Federal, state, and local government agencies, better business bureaus, published media, and electronic data bases. The Government may evaluate the organizational past performance of the offeror's proposed key subcontractors.

a. Offerors are encouraged to submit at least 3 records of past performance and shall not submit more than 10. Past performance records must be recent (within the last 5 years) and relevant (similar to the proposed work to be performed).

3. Volume II, Price (3 page maximum)

a. Volume III shall contain your proposed total price. USFS intends to award a single firm-fixed price contract for these services; Offerors shall propose a firm-fixed price to perform all of the services outlined in the Statement of Work (SOW).

b. Price proposals shall be submitted in the following format inclusive of all costs, profit, overhead, etc. associated with the effort:
