SCHEDULE OF SERVICES

Contractor personnel will perform all work required in the Statement of Work (SOW) for the amounts agreed to below. Work shall be on an “on-call basis” 24 hours per day, seven (7) days per week, for emergency operation or repair of water and wastewater systems.

When Contractor personnel are on vacation, sick leave, or any other absence, qualified substitute(s) shall be provided by the Contractor in accordance with the SOW.

The Contractor will be paid based upon the actual work performed pursuant to the SOW. A detailed breakdown of the proposed prices may be requested for evaluation purposes.

Water/Waste Water Plant Operator (Substitute) Services Part I: General Information

1.1 Introduction

The USDA Forest Service, Ozark/St. Francis National Forest, Cass Job Corps Center, located at 21424 N. Hwy 23, Ozark, Arkansas is a residential, educational, and vocational training facility for students aged 16 and not yet 25 years of age. The mission of our facility is to provide academic, vocational, and life skills training to our nation’s youth.

1.2 Background

Job Corps is a U.S. Department of Labor, Employment and Training Administration (ETA) program. Job Corps is designed to assist young people who both need and can benefit from the wide range of services provided at a Job Corps Center. Through its centers, Job Corps offers academic, vocational, and life skills training using a unique approach of competency-based, and individualized instruction. Services include: basic education; High School Diploma (HSD) program, General Education Development (GED); vocational skills training; residential living programs; healthcare; and related support services. This comprehensive combination of training and support services is designed to help each student become a responsible and productive citizen.

Approximately 100 percent of the students live full-time at the Cass Job Corps Center. The students attend classes at the center. The Center has classroom facilities, a cafeteria, health services, recreational facilities and dormitories. Students are provided allowances for personal expenditures.

Because Job Corps is a self-paced program, lengths of enrollment vary. Students may remain in the program for up to 2 years, but the average length of enrollment is approximately 8 months. An optional third year is granted for students who qualify for advanced training.

Upon completion of the program, students are better prepared to obtain meaningful employment or further their education through Job Corps sponsored Advanced Career Training (ACT) or college. Career transition services are provided to all students separating from Job Corps.

Student Eligibility. Applicants must meet all of the following criteria to participate in Job Corps:

· Applicants must be at least 16 and not yet 25 years of age

· Each applicant must be a U.S. citizen, a U.S. National, a lawfully admitted permanent alien, refugee, asylee or parolee, or other alien who has been authorized by the U.S. Attorney General to work in the United States.

1.3 Scope of Work

The substitute operator shall provide any and all labor, supervision and licenses necessary to perform routine operation and maintenance of the Water Treatment Plant and the Wastewater Treatment Plant at the Cass Job Corps Center in the absence of the full-time operator. The base period of award is through October 1, 2013 through September 30, 2014.

Applicable Documents

· Water Treatment - The substitute operator must be registered/licensed with/through the State of Arkansas Department of Health and personnel performing the work must be licensed with/through the State of Arkansas Department of Health with a “Grade 2” or higher, Water Treatment Operators License and a “Grade 2” Water Distribution License and a “Class C” plumbing inspectors license. Proof of licensing will be required prior to award of contract and each time contract is renewed.

· Wastewater Treatment – The substitute operator of the wastewater system shall have at a minimum, a “Class I” or higher Wastewater Operator License issued by Arkansas Department of Environmental Quality.

Definitions

· CO - Contracting Officer is a person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings.

· COR - Contracting Officer’s Representative. An individual designated by the Contracting Officer to perform day-to-day administration of the contract within specific authority and limitations as specified in the delegation.

· AQL - Acceptable Quality Level. The allowable leeway or variance from a standard before the government will reject the specific service.

· EPA – Environmental Protection Agency

· ADEQ – Arkansas Department of Environmental Quality

· NPDES – National Pollutant Discharge Elimination System

· gpd – Gallons per day

Part II Work Requirements

Technical Requirements The Water/Wastewater Treatment Substitute Operator shall perform the following duties in the absence of the full-time operator:

a. Operate and provide all routine maintenance for Wastewater Treatment Plant, to include:

· 35,000 gpd extended aeration sewage treatment plant

· Measuring weir

· Seven, 5,000 gallon aeration tanks

· 6,200 gallon final settling tank

· 1,700 gallon dosing tank

· Four, 30’ X 40’ Sand filters

· Chlorinator

· 5,000 gallon sludge holding tank

· Emergency settling tank – 24,000 gallon

b. Perform all State required tests to include, but not limited to the following:

· Quarterly test

· Daily: Dissolved Oxygen in the clarifier, chlorine residual in final effluent, sludge solids volume in aeration chamber, settleable solids in clarifier effluent, Ph – final effluent and flow rate.

c. Operate and provide all routine maintenance for the Water Treatment Plant, to include:

· Gravity fed raw water intake screen.

· Wet well with submersible raw water pump.

· Two, Neptune Microfloc, Inc. - Waterboy WB-82 Treatment units rated at 82 gpm.

· 3,000 gallon backwash water tank..

· 7,900 gallon settling tank for backwash water.

· 20,000 gallon, baffled contact chamber.

· 30,000 gallon water storage tank.

· Inspection and disinfecting and flushing of distribution system as needed.

d. Maintain documentation required by State and Federal agencies.

· State’s Discharge Monitoring Form, Forest Service R8-7400-1a, Daily Diary, State and Federal Inspection Reports, Quarterly Environmental Report

e. Complete and submit required forms and records as required by the permits. These forms sent through the COR and the Center Director.

f. Perform sample testing as required by permits and transport samples to the laboratory.

g. On-Call emergency response in the absence of the full-time operator.

· The substitute operator shall be on call 24 hours per day, seven (7) days per week, for emergency operation or repair of water and wastewater systems. The operator shall act in a prudent manner to provide on-site arrival in response to emergency calls in a reasonable time in the event that the full-time operator is unavailable.

Part III Supporting Information

a. Government Furnished Property The government will provide the following item(s) of Government property to the operator for use in the performance of this contract.

A laboratory building with sanitary facilities, electricity and potable water is located near the wastewater treatment plant and will be available to the Operator at no cost. A desk and filing cabinet will be provided for storage of records and forms. Cabinets will be provided for storage of equipment and supplies.

· A block storage building at the water treatment plant is used for storage of water treatment chemicals and supplies.

· A portable storage shed located near the wastewater treatment plant is available for storage of bulky materials and tools.

· Owners Manuals for Water and Wastewater Treatment Plants

· Drawings and schematics of Water and Wastewater Treatment Plants.

· Wastewater Discharge Permit

· Reporting Forms

· Site plan showing approximate locations of distribution lines and valves.

· Site plan showing approximate locations of sewer system.

· Equipment replacement not due to negligence.

· Expendable items – Items which are consumed due to the routine work processes called for in the contract.

· All laboratory equipment needed to perform required on site testing.

· Critical spare parts, such as pumps and electrical components needed to keep the treatment plants operational.

· Government will pay the cost of all laboratory testing.

· Labor and equipment will be provided by the Government for all major repairs. Work will be done under the supervision and at the request of the Operator and COR.

VACATION: 1 week paid vacation after 1 year of service with a contractor or successor; 2 weeks after 2 years; and 3 weeks after 10 years. Length of service includes the whole span of continuous service with the present contractor or successor, wherever employed, and with the predecessor contractors in the performance of similar work at the same Federal facility. HOLIDAYS: A minimum of ten paid holidays per year, New Year's Day, Martin Luther King Jr's Birthday, Washington's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans' Day, Thanksgiving Day, and Christmas Day. (A contractor may substitute for any of the named holidays another day off with pay in accordance with a plan communicated to the employees involved.) (See 29 CFR 4174)
