Walker Culvert Replacement/Small Dam Removal Project

SECTION C – DESCRIPTION/SPECIFICATIONS

C.1 Project Scope

a. This project is comprised of all required labor, equipment and materials necessary to perform the following work:

b. Work includes but is not limited to dewatering, removal and installation of culverts, excavation, placement of embankment, removal of metal and concrete structures and associated impoundment embankment, placement of embankment, placement of waste, conservation of topsoil, reconditioning and decommissioning of roads, erosion control, seeding and mulching.

c. Timing restrictions: Construction to take place during the low water periods of August through October 2010 by approval of CO, with the exception of Amik Lake Impoundment site, construction shall take place between August and October of 2011 by approval of CO.

d. Permits Required: The Forest Service shall be responsible for obtaining all necessary licenses and permits for working within the waterway. The Contractor shall be responsible for obtaining all other licenses and permits.

d. Estimated Price Range: Between $100,000 and $250,000

RD 2137 North Objibway Impoundment, MP 01.30

Remove and dispose of existing CMP and water impoundment control structure. Furnish and install a 96 inch diameter stepped CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 2107C Moccasin Lake, MP 0.20

Remove and dispose of existing CMP. Furnish and install a 72 inch diameter CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 2133 Cuba Hill, MP 4.8

Remove and dispose CMP. Furnish and install a 48 inch diameter CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 2671 Little Smith Lake, MP 0.45

Remove and dispose of existing CMP. Furnish and install a 72 inch CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 2136 Highland Impoundment, MP 0.37

Remove and dispose of existing CMP and water impoundment control structure. Furnish and install a 60 inch CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 2135 Sucker Creek Tributary, MP 1.90

Remove and dispose of existing CMP. Furnish and install a 72 inch CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 2130 Six Mile Tributary, MP 3.30

Remove and dispose of existing CMP. Furnish and install a 60 inch CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 3917 Twin Lakes Camp, MP 0.35

Remove and dispose of existing CMP. Furnish and install a 60 inch CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

RD 3917 Lake 13 Tributary, MP 2.15

Remove and dispose of existing CMP. Furnish and install a 48 inch CMP, reconstruct fill slopes up and downstream. Construct and reshape horizontal and vertical road alignments as shown on drawings.

Six Mile Impoundment Decommissioning, RD 2266, MP 0.45

Removal of existing sheet pile bulkhead, removal of a wooden walkway that accesses impoundment structure, removal of two concrete spillways between the sheet pile bulkheads, and disposal of the steel, wood and concrete off Forest Service Property.

Sucker Bay Decommissioning, RD 2950, MP 0.75

Remove existing fill material from the impoundment from Station 0+00 to 9+30. Material shall be shaped or moved so that the impoundment is graded down to the surrounding native preconstruction elevation. Following removal of fill, all graded areas should be no steeper than

3H:1V. Haul and place suitable material in 6 inch lifts from the junction of 2950A and the intersection with 2135 (approximately 0.42 miles). Compact and grade surface to drain.

Unsuitable material will be placed in a single lift not to exceed 18 inches in depth on FR 2950 between the impoundment and the junction of 2950A.

Remove water impoundment control structure and dispose of off Forest Service Property.

Conserved topsoil shall be placed on the upland portion of the impound site.

Obliterate Road 2950 between the impoundment and the junction of Road 2950A, by first ripping road surface twice to a depth of 2 feet below existing surface and complete obliteration by restoring the natural drainage. Clearing within the clearing limits will be set aside until drainage is restored then scattered on roadway surface to close it to motorized traffic. If brush from clearing limits is not adequate to effectively close the road to motorized traffic, as determined by the GR, the Contractor will generate additional woody slash material from forest land adjacent to the project area and scatter the material on the roadway surface.

Straw bales for erosion control will be required at the base of the constructed slopes located at the ends of the removed impoundment and at the edges of the constructed channel as directed by the Engineer.

Seed and mulch the disturbed upland area of the impoundment, disposal site, and obliterated road

2950.

Amik Bay Decommissioning, RD 3802A, MP 0.30

Removal of an existing sheet pile bulkheads, removal of a wooden walkway that accesses dam, removal of one concrete spillway between the sheet pile bulkheads from Forest Service Property.

Removal of a CMP water control structure from Forest Service Property. Remove existing fill material from the impoundment. Material shall be shaped or moved so that the impoundment is graded down to the surrounding native preconstruction elevation. Following removal of fill, all graded areas should be no steeper than 3H:1V. Haul and place suitable material in one 6 inch lift on Forest system roads accessing the site. Location of fill placement shall be approved by the

CO prior to placement. Compact and grade surface to drain. Unsuitable material shall be disposed at a site designated by the CO within 1 mile of the impoundment site.

Conserved topsoil shall be placed on the area of the impoundment site.

Seed and mulch the upland disturbed area of the impoundment, and the disposal site.

NOTE: Segments of the road accessing this site, FR3802A, are steep and narrow. It is advised that the contractor visit this site to verify accessibility. The road will be brushed, bladed, and resurfaced by the Forest Service during the spring and summer of 2010. However, additional minor road improvements (if required) may be performed by the contractor to improve access.

These improvements will be limited to minor shaping and filling of the road bed and brush removal not to exceed 18 feet in width and 15 feet in height. No trees over 4 inches diameter at breast height shall be removed or damaged by equipment.

Straw bales for erosion control will be required at the base of the constructed slopes located at the ends of the removed impoundment and at the edges of the constructed channel as directed by the Engineer.

C.4 Safety and Public Notification

Warning signs will be placed at the proper locations to warn public of road work. All signs shall meet the minimum requirements as established by the Manual of Uniform Traffic Control devices (MUTCD), 2003 edition. A notice of where and when the public might expect delays due to project work will be posted on access roads to project.

C.5 Damage to Structures

The Contractor shall be held liable for any damage caused by his/her personnel to any existing bridges, ditches, culverts, or signs. Repair of structures damaged by the Contractor’s operations, shall be their responsibility at no cost to the Government.

Schedule of Items--A schedule in the contract that contains a listing and description of construction items, quantities, units of measure, unit price, and amount.

Whether amount spilled is EPA or state reportable, and if so whether it was reported, and to whom.

Exact time and location of spill including a description of the area involved.

Summary of any communications the Contractor had with news media, Federal, state and local regulatory agencies and officials, or Forest Service officials.

Description of clean-up procedures employed or to be employed at the site including final disposition and disposal location of spill residue.

When available provide copies of all spill related clean up and closure documentation and correspondence from regulatory agencies.

The Contractor is solely responsible for all spills or leaks that occur during the performance of this contract. Clean up spills or leaks to the satisfaction of the CO and in a manner that complies with Federal, state, and local laws and regulations.

152.01 General.

Upon request, before the start of the work, the Government will provide temporary staking indicating the required end positions of the each culvert. This temporary staking may be a single stake at the end location or a set of ties which can be used to establish this position. A bench mark for each culvert location has been established and is shown on the drawings. Protect and maintain benchmarks as required.

152.02 Construction Staking

Before starting work, the Contractor will review the temporary staking, clarify any uncertainties, and accept the staking in writing. After acceptance of the temporary staking, any and all of the required construction staking is the responsibility of the Contractor.

Establish and maintain survey ties to each end of each culvert until final acceptance of the work.

Maintain survey notes on site for review by the GR at all times. Use moisture-resistant paper for survey notes. Keep notes in books with covers that will protect the contents and retain the pages in numerical sequence.

157 - Soil Erosion Control

Provide bales, wattles, logs, rolls, and erosion control mats from a certified noxious weed free source.

157.03 General

Delete the second paragraph and add the following:

"This work includes submitting an Erosion Control Plan detailing permanent and temporary control measures. Submit the erosion control plan proposal at least 7 days before operations begin to the Contracting Officer for approval.

The erosion control plan reflects special concerns and measures necessary to protect resources and government improvements. The Erosion Control Plan shall, at a minimum:

1. Describe the construction activities and sequence of implementation relating to specific erosion control measures.

2. Describe the location and type of permanent controls to be implemented during construction.

3. Describe the location and type of temporary controls to be implemented during construction.

4. For work in stream channels include a detailed dewatering plan. The dewatering plan shall include at a minimum: method(s) of preventing water from entering the excavation from upstream (i.e. cofferdam); method(s) of routing, collecting, and removing water which may enter the excavation; method(s) of preventing water from entering the excavation from downstream (i.e. cofferdam); method(s) of removing sediment from water before returning it to a stream channel; and a plan of action in case of a large storm event and/or failure of a method in use.

5. Include procedures and temporary control measures to be implemented to minimize sediment delivery during re-watering of any new culvert with streambed simulation material.

6. For work in stream channels without flowing water describe level of ground and vegetative disturbance and measures to reduce potential sediment delivery.

7. Describe what monitoring will take place.

8. Describe the measures that will be taken to minimize erosion and sediment delivery from material stockpiles.

9. Describe methods and measures that will be taken to minimize erosion and sediment delivery to the stream from the proposed borrow source as SHOWN ON THE

PLANS.”

Add to the third paragraph:

“Upon completion of construction at the site, all temporary dewatering materials and equipment are to be removed from Government property."

157.04 Controls and Limitations on Work.

Add the following to the second paragraph:

“Temporary erosion control measures will be in place at the end of each day’s operation if greater than 30 percent chance of rainfall is predicated. Earthwork operations may be temporarily suspended by the C.O. if significant rainfall is forecasted.”

157.08 Water Crossings.

“Any site where there is running water through a channel crossing shall be dewatered by rerouting the flow around the site before and during excavation and embankment operations that has the potential for sediment delivery to the live stream.”

Measurement

157.15 Add the following:

“In addition to any temporary or permanent erosion control measures outlined in the Soil Erosion and Pollution Prevention Plan required under this specification, the Contractor will have available on site at all times 5 straw bales and 30 feet of 12 inch diameter straw wattle for the discretionary use of the C.O. Upon completion of the project, bales and wattle that have been incorporated into the work sites will remain in place or be removed as directed by the C.O.

Unused bales will be uniformly spread as mulch over seeded areas. Unused straw wattle becomes the property of the Government upon completion of the project. ”

Add the following new Section:

201 - Clearing and Grubbing

201.01 Description.

Delete the first sentence and replace with the following:

This work consists of clearing and grubbing within clearing limits and other designated areas.

Clearing limits shall be limited to all construction areas necessary for roadway and stream work as well as areas necessary to process the clearing and grubbing slash. All clearing limits must be clearly delineated and then approved by the CO before any clearing and grubbing can take place.

203 - Removal of Structures and Obstructions

203.01_nat_05_12_2004

203.01 Work.

Delete and replace with the following:

This work consists of disposing of clearing slash, salvaging, removing, and disposing of buildings, fences, structures, pavements, culverts, utilities, curbs, sidewalks, and other obstructions.

203.05 Disposing of Material.

(a) Remove from project Add the following:

All materials designated for removal become the property of the Contractor and are to be disposed of by removing off of Forest Service Lands in accordance with all local, state and federal requirements.

(b) Burn. Delete this subsection

(c) Bury. Delete this subsection

(e) Scattering. Scatter pieces of wood less than 3 inches in diameter and 3 feet in length within the clearing limits or in locations designated on the ground by the C.O. Do not place construction slash in lakes, meadows, streams, or streambeds. Immediately remove construction slash that interferes with drainage structures.

204 - Excavation and Embankment

Description

204.01 Delete the first paragraph and replace it with the following:

This work consists of stream channel excavation, excavating material, and constructing embankments. This includes furnishing, hauling, stockpiling, placing, disposing, sloping, shaping, compacting, and finishing earthen and rocky material.

204.07 Subexcavation.

Add the following subsection:

204.07A Excavation for Stream Modification.

Construct the new stream channel as shown on the plans or as directed by the CO. Do not operate equipment in the live stream unless given approval by the CO. Minimize disruption of the existing landscape and impacts to the stream. Provide adequate pumping equipment, piping or other acceptable measures, as approved by the CO, to dispose of any water encountered during excavation in a manner that will cause minimal sedimentation to the existing stream.

204.11 Compaction.

Delete the first paragraph and replace it with the following:

For compaction method (d) no sieve test is required.

Add the following compaction method:

(d) Roller Compaction Method. Place material by end dumping to the minimum depth needed for operation of spreading equipment. Adjust the moisture content of the material to a level suitable for compaction. Operate compaction equipment over the full width of each layer until visible deformation of the layer ceases. Make at least three complete passes.

204.13 Sloping, Shaping, and Finishing.

Delete section (d) and add the following:

(d) Finishing. Finish the roadbed to be smooth and uniform, and shaped to conform to the typical sections. Remove unsuitable material from the roadbed and replace it with suitable material.

Remove all material larger than 6 inches from the top 6 inches of the roadbed and replace with suitable material.

Ensure that the embankment material is visibly moist during shaping and dressing. Bring low sections, holes, cracks, or depressions to grade with suitable material. Maintain proper ditch drainage.

204.14 Disposal of Unsuitable or Excess Material.

Delete the text of the first paragraph and substitute the following:

“Dispose of unsuitable or excess material at designated sites or as directed by the C.O.”

209 - Structure Excavation and Backfill

209.04 General.

Use suitable material in the construction of the roadway embankment and for structural backfill.

Field drain and dry excessively wet material that is otherwise suitable for embankment before placement. Place and shape unsuitable or excess material in the designated sites or as directed by the CO.

213 - Subgrade Stabilization

Delete entire Section 213 and replace with the following:

213.01 General. It is the Contractor's responsibility to stabilize the subgrade below the pipes as required to achieve an acceptable pipe installation. The means and methods of this stabilization are the Contractor's decision.

The COR shall approve the proposed means and method of stabilization before work in this area is performed.

213.02

Work under this section will not be measured.

Payment

213.03

Work under this section will be considered incidental to other pay items. No separate payment for this work will be made.

301 – Untreated Aggregate Courses

Delete the entire Section and replace with the following:

Description

301.01 This work consist of constructing a surface course of aggregate.

Surface course aggregate grading is designated as shown in Section 703.

Material

301.02 Conform to the following Subsections:

Water 725.01

Construction Requirements

301.03 General. Submit documentation of aggregate gradation to COR for approval before placing aggregate. Place aggregate course on compacted backfill or embankment. The COR shall approve the prepared backfill/embankment before surface aggregate placement starts.

Place and compact surface aggregate in a minimum of 2 lifts.

301.04 Compacting. Use vehicle compaction method. Compaction vehicle must be capable of exerting a ground pressure of at least 80 psi.

Make a minimum of (3) complete passes over the full width of each lift. Additional passes may be required to achieve the required compaction.

301.05 Acceptance. Continue compaction operations until signs of movement cease. The GR must observe and approve compaction.

301.06

Work under this section will not be measured.

301.07

Work under this section will be considered incidental to other pay items. No separate payment for this work will be made.

602 - Culverts and Drains

602.03 General.

Change the following in the second paragraph:

"Determine the final location skew, length, elevations, and grade according to Section 152."

Ensure that the final installed alignment of all pipe allows no reverse grades, and does not permit horizontal and vertical alignments to vary from a straight line drawn from center of inlet to center of outlet by more than 2 percent of pipe center length or 1.0 feet, whichever is less.

Protect pipe from damage due to temporary construction conditions. Provide and maintain manufacturer's suggested cover depths for axle loads used during construction. Final condition is designed for anticipated public vehicle loads, and may not be sufficient for all construction equipment loads.

625 – Turf Establishment

Description

Delete the entire subsection and replace with the following:

625.01 This work consists of soil preparation, seeding and mulching/application of erosion control product.

Material

625.02 General.

Temporary rolled erosion control products 713.17

625.03 General.

All disturbed areas are to be seeded and mulched according to the following schedule:

Seeding: (a) Dry Method

Mulching: (b) Covered with temporary erosion control product or straw mulch. See schedule in Subsection 625.08 Mulching.

625.05 Watering.

Delete the entire subsection and replace with the following:

Moisten areas to be seeded immediately before seeding. No additional watering is required following demobilization from site.

625.06 Fertilizing.

Delete the entire subsection.

625.07 Seed.

Seed will be provided by the Government. Up to (2) different seed mixes may be used. Pick up seed at the USFS Cass Lake Supervisor's Office: 200 Ash Ave NW, Cass Lake, MN.

Coordinate seed mix to be used in each area with the GR.

All seeding is to be done by the Dry Method.

Seeding Application Rate: To be designated by GR.

625.08 Mulching.

( c ) Temporary Erosion Control Product. Apply erosion control blanket per manufacturer's recommendations according to the mulching schedule.

703 - Aggregate

Delete 703.05(b) and replace with the following:

703.05 Subbase, Base, Surface Course, and Screened Aggregate.

(b) Surface course aggregate. Furnish hard, durable particles or fragments of crushed stone, crushed slag, or crushed gravel conforming the following specification:

MnDOT Class 5

Do not use material that breaks up when alternately frozen and thawed or wetted and dried.

Do not furnish material that contains asbestos fibers.

Obtain the aggregate gradation by crushing, screening, and blending processes as necessary.

Fine aggregate, material passing the No. 4 sieve, shall consist of natural or crushed sand and fine mineral particles.

713 - Roadside Improvement Material

Delete the entire Section 713.04 and replace with the following:

713.04 Seed.

Seed mix(es) will be provided by the government.

713.05 Mulch.

Add the following to Subsection 713.05 (a):

Straw mulch shall be certified as Noxious Weed Free by the MN Crop Improvement Association
