STATEMENT OF WORK

Carpet Replacement Building 1 Third Floor Hallway Forest Products Laboratory One Gifford Pinchot Drive Madison Wisconsin, 53726 February 2024

1.0 Introduction:

A. The USDA, Forest Service, Forest Products Laboratory requires services of a Contractor to perform work of removal and installation of carpeting add uniform thresholds for inside of Building 1 in the third floor Hallway.

B. Estimated total quantity of the requirement is 2,515 sq. ft.

C. Uniform low profile 39 Threshold Transitions.

2.0 GENERAL REQUIREMANTS:

a. The contractor shall provide all personnel, equipment, tools, supplies, materials, supervision, and transportation necessary to:

b. Removal of the existing flooring as required.

c .Relocation of furniture and return to exact location.

d..Disposal of all indicated existing carpet.

e. Remove and preserve all existing wood base from wall and columns and stairwell banister. If carpeting can be tucked under the existing base this is acceptable. The contractor shall take care not to scuff or markup existing base during installation of carpet.

f. Furnish and install new carpet, all doorway transitions.

g. Contractor shall verify all dimensions during site visit.

h. Contractor shall submit physical samples of carpet and transitions and to customer priorto installation.

i. Forest Products Laboratory requires commercial grade modular carpet tiles.

j. Use of environmentally safe nontoxic adhesive.

k. Installation will be managed by local installers.

l. Measurements: The contractor shall be responsible for measuring and verifying the area and perimeter of all spaces receiving carpet, and door transitions.

Product Data:

1.Contractor shall submit a copy of manufacturer's full technical data and installation instructions for each type of carpet and accessory.

2.Contractor shall submit physical samples to customer prior to installation.

3.0 WORK REQUIREMENTS:

1. The contractor shall conduct the work safely and minimize the impact on Forest Products Laboratory staff.

2.The contractor shall move furniture as necessary to remove and install carpet. Furniture shall be returned to the exact location.

3. The contractor shall submit physical samples to customer prior to installation the samples shall meet the Bio Preferred criteria

4. The contractor shall work using the existing base boards in the hallway.

5. The contractor shall remove and dispose of the existing carpet disturbing the flooring underneath.

6. Perform level floor repairs where needed with quick dry leveling materials.

7. Transitions - The contractor shall install transitions in areas where there is a change in floor type or carpet height. Transitions shall match or compliment cove base material.

8. Removal and Disposal - In accordance with EPA guidelines to reduce land filling, provide for carpet and carpet installation waste handling in the project’s overall waste management plan. When feasible, all carpet and carpet installation waste disposal should require a certification that the products were not deposited in a landfill. There are carpet reclamation centers in most major cities. Other sources for carpet reclamation can be found by contacting the carpet manufacturer/provider for their reclamation/recycling options. 9. Installation - Contractor Qualifications: All work shall be performed by contractors/installers who is Certified Floor covering Installers (International Certified Floor covering Installer Association) or manufacturer’s approved installers. Foremen on the jobsite shall be “Certified C-II Minimum.” For warranties to be valid, all carpet shall be installed in accordance with the installation guidelines set forth by the manufacturer and/or the minimum guidelines set forth in CRI 104-2002, Standard for Installation Specification of Commercial Carpet, and CRI 105-2002, Standard for Installation of Residential Carpet. Carpet shall be installed in full accordance with manufacturer’s recommendations so as to preserve all warranties.

10. Additional material - The contractor shall provide 15% of extra tiles above the amount installed. These will be given to the contracting office.

11. Doors - After carpet installation is complete, the contractor shall ensure that all doors affected by the project can open and close properly.

12. Cleaning - After carpet installation is complete and prior to returning furniture to its original location, the contractor shall remove all debris, scraps, and any other foreign matter. Contractor shall remove any spots or adhesive from the carpet or adjoining surfaces, to include walls, clip any protruding face yarn and vacuum the entire work area.

4.0 PERFORMANCE PERIOD:

The performance period shall not exceed 10 days from date of contract award.

5.0 HOURS OF PERFORMANCE:

To minimize the disruption to the contractor and Forest Products staff work may be scheduled on weekends, or nights outside of the normal working hours of 8:00 A.M. to 4:00 P.M. Monday through Friday excluding Federal Holidays.
