Attachment 1 - STATEMENT OF WORK

1.0 INTRODUCTION

The U.S. Geological Survey, Eastern Ecological Science Center, requires interior refinishing of the Administration Building to include replacement of existing flooring with a combination of carpet tiles and linoleum planks, a kitchen renovation with some demolition and construction of walls, and painting.

2.0 BACKGROUND

The U.S. Geological Survey’s Eastern Ecological Science Center is located at 11649 Leetown Road, Kearneysville, WV 25430. Research conducted at this facility includes study of infectious and noninfectious diseases of fishes and other aquatic organisms. The facility also conducts research on invasive fish species with the intent of better understanding the adaptations these fish are applying in these new, nonnative habitats and developing ways to stop their spread and potentially eradicate them.

3.0 SCOPE

The scope of work includes:

1. Replace Carpet and mop board.

· Carpet material for the floors is to be brand: Mohawk Rule Breaker (color TBD) 24”X24” tiles and sheet material for steps and landings. Carpet Area estimated at 8,000 sq. ft. (offices and hallways and stairways combined).

· Included in the carpet replacement is the elevator which is 25 sq. ft.

· Both Carpet and vinyl to be installed to manufactured specs

2. Replace vinyl and mop board.

· Kitchen, foyers, and the lobby area are to be vinyl plank. Brand: Karndean, Hartford LLP 112, estimated at 1,700 sq. ft.

3. Paint Walls estimated at 16,500 sq. ft. Areas that have wallpaper will stay as is.

· Paint is to be brand: Sherwin Williams (color TBD)

4. Kitchen remodel to include new cabinets, new lighting, and small demolition of a wall to add space. Paint and install flooring.

· Replace existing kitchen cabinets, sink, faucet, and any necessary plumbing. Include shut offs for maintenance. Sink and faucet style TBD

· Kitchen cabinets to be brand: Shenandoah. style: Bluemont. color: Rye Maple. See attached sketch of kitchen in Figure A.1 and A.2

· Kitchen countertop to be brand: Formica. style: Ouro Romano, # 9277-46

· Two (2) under cabinet microwaves will be installed on the sink wall, requiring two (2) separate 20-amp circuits be pulled from the electric service below. This can be achieved from the kitchen area that is being demolished and expanded. The Facility Manager (FM) will supply microwaves. They will both need a receptacle installed for each 20-amp circuit in the cabinet above.

· The kitchen wall that gets the two (2) pantry style cabinets will have countertop adequately supported in between for a coffee nook.

· Supply 10% stock of each flooring at the end of the contract for future use if needed.

· All measurements to be verified by contractor

4.0 TECHNICAL REQUIREMENTS

Contractor shall obtain all permits required by federal, state and local regulations prior to commencement of work. All required paperwork and calculations required to obtain any permits shall be provided to the and FM and the Contracting Officer’s Representative at time of submission as well as the permits themselves upon receipt of the documents from the permitting agencies.

Contractor shall not proceed with any work until all required submittals are approved by the Facility Manager (FM).

5.0 OTHER REQUIREMENTS

COVID 19 requirements: All contractors are to wear face coverings and self-check each day before entering the building, a sign-in sheet is to be kept and issued to FM each week or upon request.

Pre-Construction Meeting:

The contractor shall schedule and conduct a pre-construction meeting on-site with the FM and COR to review the requirements and conditions of this contract. The contractor shall have the project manager, field superintendent, and all subcontractors on site, or available by conference call, for the pre-construction meeting with Government officials prior to the commencement of work for this project. The contractor must have a superintendent to oversee work on-site, as required at Section I.71.

Outages and Interruptions:

The contractor is advised that the Science Center will continue to perform research functions during the contract. Work shall be conducted as to minimize interruption to service. Contractor shall notify FM at least 3 days prior to any and all outages and interruptions. Contractor shall not proceed with work without approvals from the FM. Changes to the schedule require the submission of a new schedule to the FM for review.

Normal working hours for the construction work at the Science Center shall be 7:30am- 4:00pm, Monday - Friday. The contractor and sub-contractors (if required) shall not work Saturdays, Sundays, legal holidays, or non-normal working hours, unless otherwise authorized by the FM. Request to work other than the normal hours shall be made, and approved, in writing through the FM. Work performed during “other than the normal hours” shall not change the total cost to the government.

Changes/Clarifications:

All work must be completed as shown in the contract documents. If ‘no cost’ change orders are to be made, a written request to the COR must be made detailing why the work cannot be completed as shown, as well as the corrective action suggested by the contractor. For change orders which will alter the price or requirements of the contract, the FM and the COR’s approval must be given in writing before beginning the work described.

Disposal and Transporting of Debris and Material:

All materials and construction debris shall be removed from the work area and placed in an approved dumpster or removed from the site daily by the contractor. All demolished materials shall be disposed of in an environmentally friendly manner, recycling whenever possible

Organization and storage of contractor(s) items at the site:

The FM shall designate areas where the contractor can store or place materials and equipment for short-term storage in preparation for installation. There is limited space on site for the storage of contractor equipment. There is no inside storage space available. Storage and security of materials and equipment as required for the performance of the contract requirements are the responsibility of the contractor. The contractor shall be responsible for making all arrangements for the receipt of materials and supplies at the job site. Government personnel are not permitted to receive or sign for items delivered to the site. Upon completion of the contract, the contractor shall remove all equipment and excess materials, and shall restore the site to its original condition, as approved by the FM, at no additional cost to the government.

Site Cleanliness:

The contractor shall maintain the site free of construction debris outside of approved containers (dumpsters). The contractor shall provide dust protection and shall maintain cleanliness of the work site by promptly removing debris, tools, and excess material.

Contractor Parking, Security, and Smoking Regulations:

The contractor shall maintain a daily log of all contractor and subcontractor employees (date, name, arrival time, departure time) and provide a copy of the log to the FM at least weekly. The FM will designate a parking area (or areas) for the contractor to park vehicles. There shall be no smoking in any buildings at any time. All personnel that smoke will have to do so outside in designated smoking areas.

Environmental Protection:

The contractor and subcontractors shall perform all work and activities in accordance with local, state, and Federal environmental regulations.

Safety:

The contractor and subcontractors shall perform all work and activities in accordance with local, state, and Federal safety regulations, including OSHA, NEC, and USGS policy.

Notice to the Government of Delays:

In the event the contractor encounters difficulty in meeting performance requirements, or when it anticipates difficulty in complying with the contract delivery schedule or date, or whenever the contractor has knowledge that any actual or potential situation is delaying or threatens to delay the timely performance of this contract, the contractor shall immediately notify the COR and the FM in writing, giving pertinent details. This data shall be informational only in character. Notice under this provision shall not be construed as a waiver by the Government of any delivery schedule or date or any rights or remedies provided by law or under this contract.
