QUALITY ASSURANCE PLAN

1.0 Objective

Provide a brief description of the Quality Assurance Plan’s intent.

The QAP provides a systematic method to evaluate performance and monitoring for the Dale Bumpers White River NWR Janitorial Contract. The Contractor is responsible for management and quality control actions to meet the terms of the contract.

2.0 Performance Indicators and Acceptable Quality Levels

Provide a description of the performance indicators to be measured and their acceptable quality levels.

The Contractor shall not miss more than three standards (below) per check.

Center for Education/Exhibits:

Vacuum carpet: After weekly vacuum, the carpeted floors shall be free of all visible litter and soil. Spot clean carpets as needed. Report any stains unable to remove or carpet damage to COTR.

Floors: All tiled floors in Center for Education (foyer and restrooms) shall be swept and wet mopped with pre-approved green seal cleaner daily. Prior to being wet mopped, all areas to be mopped shall be swept to remove litter, dust and foreign debris from the floor surface. After being mopped, the floor shall have a uniform appearance with no streaks, swirl marks, detergent residue or evidence of soil stains, film, debris or standing water. The Contractor shall spot clean floors when there are noticeable spots.

Countertops and office furniture: All surfaces shall be cleaned of fingerprints and dust daily with a soft scratch free cloth and pre-approved green seal cleaner.

Doors, windows between book sales area and exhibit hall, and inside windows in birders room: Windows should be dust and fingerprint free. Clean with pre-approved seal cleaner.

Toilets, urinals and lavatories: Both the interior and exterior of all sinks, toilets (to include seats), urinals, walls, baby changing stations, door and partition surfaces and lavatory surfaces shall be cleaned daily with pre-approved green seal cleaner.

Trash removal: Empty all waste baskets and put all trash in trash disposal weekly.

Refill towel, toilet paper and soap dispensers: All towel and tissue dispensers shall be refilled as needed.

Interior spaces, area around porch and entry doors should be kept clean of spider webs as needed.

HVAC vents and ceiling fans: Keep clean of dust.

Drinking fountain: Keep clean using pre-approved green seal cleaner.

Empty and clean all cigarette butt receptacles.

Administration office: Cleaned every week

Rest Rooms:

Sweep and mop floors; spot clean as needed.

Clean all fixture surfaces: (inside and out) toilets, urinals, washbasins, mirrors, dispensers, baby changing stations and waste receptacles. Clean with pre-approved green seal cleaner.

Empty waste receptacles, service paper towel, soap and toilet paper dispensers, and wipe railings.

Clean all wall, door, and partition surfaces.

Clean/remove spider webs on interior walls and exterior entrance doors.

Reception Area, Office areas, Staff lounge, and Hallways:

Vacuum carpeting: After vacuum, the carpeted floors shall be free of all visible litter and soil. Spot clean carpets as needed. Report any stains unable to remove or carpet damage to COTR.

Uncarpeted areas: Broom sweep and damp mop all uncarpeted areas.

Wall, door, desk tops, office furniture, telephones, and partition surfaces: Keep clean with pre-approved green seal cleaner.

Waste receptacles: Keep emptied and clean.

Ceiling fans and HVAC vents: Keep clean of dust.

Clean/remove spider webs on interior walls and exterior entrance doors.

3.0 Surveillance Methods

Provide a description of the evaluations that will be performed based on the indicators in section 2.0.

The COR will conduct random inspections. The Government reserves the right to inspect the Contractor’s performance using 100% inspection and validated customer complaints. When notified of a deficiency, the Contactor shall correct items designated as unsatisfactory or below standard at no additional cost to the Government. All corrective action must be completed within 24 hours of notification of the deficiency; if not, a negative performance rating will be given. Failure of the Government to notify the Contractor of a deficiency does not relieve the Contractor of the obligation to perform services nor does it relieve the Contractor of the responsibility to ensure that quality controls measures are followed to ensure contract compliance.

4.0 Incentives

Provide a description of any incentives/penalties.

When janitorial assignments fail to meet the Acceptable Quality Level (missing three or more standards per check), a negative performance rating will be given.
