Please use the following definitions to assess contractor performance.

Exceptional
Performance meets contractual requirements and exceeds many to the Government’s benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Very Good
Performance meets contractual requirements and exceeds some to the Government’s benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

2. What is your assessment of the contractor’s timeliness of performance (reliability, responsiveness to technical direction, adherence to contract schedule, etc)? Did the contractor respond to service calls and supply orders in the timeframe specified in the contract requirements? Is the contractor able to provide support to remote locations? Do technicians possess the experience and expertise necessary to maintain and service the equipment?

3. What is your assessment of the contractor’s business relations (contract management, business-like correspondence, notification of problems and problem resolution, etc.)?

4. What is your assessment of the contractor’s invoicing procedures and policies? Does the contractor submit invoices accurately and timely? Is the billing staff proactive in resolving billing discrepancies?

5. What is your assessment of the contractor’s personnel management? Is the contractor’s work force properly trained and given proper direction? Are required tasks successfully performed?

6. What is your assessment of the contractor’s management of subcontractors? Do the subcontractors still meet the requirements specified in the contract? Does the contractor resolve any issues with the subcontractors in a timely manner?
