The form establishes mandatory health insurance requirements for offerors on state and local contracts solicited and awarded on or after January 1, 2008. Offerors must provide health insurance coverage to employees who work an average of at least 20 hours per week over a six-month period if the contractor grows to six or more employees and the expected annual aggregate contract value with the state exceeds $250,000. Health insurance must be offered no later than July 1, 2010, and offerors must maintain records documenting employee acceptance or declination of coverage, which remain subject to state review and audit.
Offerors are required to advise all employees of publicly financed health care coverage availability by providing information regarding the BeWellNM website (http://www.bewellnm.com). For Indefinite Quantity, Indefinite Delivery price agreements without specific quantity limitations, these requirements apply upon the first day of the second month after the offeror reports combined sales of $250,000, $500,000, or $1,000,000 from the state and applicable local public bodies. The audit services solicitation requires proposals from Independent Public Accountants in good standing with the Office of the State Auditor, with audits to be conducted in accordance with State Auditor Rule 2.2.2 NMAC, generally accepted auditing standards, and Uniform Grant Guidance regulations under OMB's Super Circular.
