PAST PERFORMANCE QUESTIONNAIRE (PPQ)

I. INSTRUCTIONS

The company (i.e., contractor) that sent you this questionnaire intends to submit an offer in response to a U.S.

Coast Guard solicitation and has identified you as a reference to validate their performance. This PPQ must be completed by the person most familiar with the contractors’ performance on a present or previous contract and then submitted directly to the U.S. Coast Guard by the person completing the PPQ. Please DO NOT send the completed PPQ to the contractor being evaluated. In addition to this questionnaire, you may receive a follow-up phone call to confirm or clarify information. We are thanking you in advance for your time, effort, and cooperation in responding to this questionnaire.

Please submit the completed form to the Contract Specialist noted below via one of the following methods:

III. GENERAL GUIDANCE

Please use the following ratings to score the performance elements below (assessments should reflect only contractor-liable performance):

0 = Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance element contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

1 = Marginal Performance does not meet some contractual requirements. The contractual performance element being evaluated reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.

2 = Satisfactory Performance meets contractual requirements. The contractual performance element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

3 = Very Good Performance meets contractual requirements and exceeds some to the

Governments benefit. The contractual performance element being was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

4 = Exceptional Performance meets contractual requirements and exceeds many to the

Governments benefit. The contractual performance of the element being evaluated was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
