Statement of Work (SOW) for the purchase of and Installation of

FiberCab Laser System Machine FC 44

70Z03822QK0000006

1.0 General

The United States Coast Guard (USCG), Aviation Logistics Center (ALC), Industrial Operations

Division (IOD) has a requirement for a FiberCab Laser System Machine FC 44.

2.0 Requirements

The Contractor shall provide all the necessary personnel to perform removal and movement of the existing water jet machine and off-loading and movement of the identified equipment into the customer’s desired location inside of building 97, Machine Shop. The Rigging Company will be responsible for the coordination of delivery and set-up of new machine and removal of the existing water jet machine. To schedule, the Rigging Company shall contact the Contractor’s technical point of contract (POC) listed below. The FiberCab Laser System machine shall be installed at;

IOD Shop 133

Building 97

1664 Weeksville Road

Elizabeth City, NC 27909

The Rigging Company shall remove the existing water jet machine and move it to the on-site disposal location. The distance is approximately four hundred (400) yards from building 97. The Rigging

Company shall off-load the FiberCab Laser Machine FC 44, and moved it into its final location, where set-up and rough level can take place. The Rigging Company will provide all Equipment, Tooling, and

Labor to complete this task within this SOW.

The Contractor shall provide only Certified Technicians for on-site installation. The Technicians will install, test, and provide initial training of machine commands and controls.

The Contractor’s representative shall travel to ALC to perform services based on a mutually agreed schedule. The Contractor’s technical POC for this effort is Mr. Darnell Chamblee. He can be reached by phone at (252) 335-6414 or by email at Aaron.D.Chamblee@uscg.mil.

2.1 Additional Requirements

The following are considered the minimum key requirements when determining the appropriate Laser

Cutting System.

Machine must fit into existing foot print size – ninety (90) inch long by ninety (90) inch wide.

Machine cutting minimum capabilities - .5 inch for aluminum (Non-Ferrous) and .5 inch for steel

(Ferrous) metal.

Machine must have the capability to be air and / or water cooled.

Software must be installed as part of the procurement package.

Machine must be able to operate with either oxygen or nitrogen.

Vendor shall provide the best warranty available.

Vendor must offer training for up to ten (10) personal and support for the next twelve (12) mailto:Aaron.D.Chamblee@uscg.mil months as initial provision package. Training will be completed within thirty (30) calendar days after installation.

Vendor shall complete the on-site installation within thirty (30) calendar days after receipt of material.

3.0 Contractor Personnel

3.1 Qualified Personnel

The Contractor shall provide personnel qualified to perform all requirements specified in this SOW.

Qualified personnel shall have been trained and authorized by the OEM to perform these services.

3.2 Identification of Contractor Employees

The Contractor shall ensure all employees present a professional appearance appropriate for the position held. All Contractor personnel shall be identified by a distinctive nameplate, emblem, or patch attached in a prominent place on an outer garment so as not to present a Foreign Object Debris (FOD) hazard and clearly identify the employee as a Contractor representative.

4.0 Policies and Regulations

4.1 Access to USCG Base Elizabeth City

ALC is a tenant command of Base Elizabeth City. The Contractor shall be met at the gate by a

Government representative and shall be continually escorted by the representative for the duration of the visit. Base access requires 100% hands on ID checks. Drivers of vehicles must show a state driver license, valid vehicle registration, and current proof of insurance. All vehicles are subject to random search under Title 18 U.S.C 1382.

4.2 Compliance with Local Regulations, Instructions, Directives

Contractor personnel shall comply with all local base regulations, instructions, directives, orders, and special procedures as promulgated by the Commanding Officer, USCG Base Elizabeth City (and all tenant commands), Elizabeth City, NC. All visitors to ALC shall review the ALC Visitor/Contractor

Safety & Environmental Health Brochure information which is included after page 3 of this SOW.

4.3 Removing Employees for Misconduct or Security Reasons

The Contracting Officer may direct the Contractor to remove any Contractor employee from USCG facilities for misconduct or security reasons. Removal does not relieve the Contractor of the responsibility to continue providing the services required under the order. The Contracting Officer will provide the Contractor with a written explanation to support any request to remove an employee.

4.4 Tobacco Policy

The USCG prohibits the use of smoking tobacco and smokeless tobacco in the workplace to protect the health of all its workers. The workplace includes any area inside or outside of a building or facility under the custody and control of the USCG where work is performed by active duty personnel, civilian employees, or personnel under contract to the USCG. The use of tobacco is prohibited in all USCG

Government vehicles (including Cushman type), in all USCG aircraft, floating unit, or any other aircraft contracted for use in the USCG. The use of tobacco products (smoking and smokeless) is permitted only in designated areas as defined in USCG ALC Instruction 6280.1 (series). Contractors may only use tobacco products in designated smoking areas and are accountable for appropriately discarding their smoking material and/or spit tobacco. Tobacco spit and residue shall be held in containers with sealing lids to prevent odor and accidental spills and disposed of in a sanitary manner that prevents public exposure. Employees caught violating the tobacco policy may be subject to immediate removal.

4.5 Environmental, Safety, And Occupational Health (ESOH)

The Contractor is solely responsible for compliance with all federal, state and local laws, including but not limited to the Occupational, Safety and Health Act (OSHA) (Public Law 91-596) and the resulting standards, OSHA Standard 29 CFR 1910 and any ALC Instructions as applicable, and the protection of their employees. Note: ALC Instructions are annotated because many of the USCG/ALC standards exceed the OSHA standard criteria. If a conflict is noted, the most stringent requirement takes precedence. The USCG shall assume no liability or responsibility for the Contractor's compliance or

Non-compliance with such requirements. In addition, the Contractor is required to flow down the safety requirements/specifications to all subcontracts/subcontractors. Safety requirements listed herein that do not concern Contractor’s operations or services shall be considered self-deleting as mutually agreed by the Contracting Officer and the Contractor.

4.5.1 The Contractor shall be subject to safety inspections of its work sites by the USCG. Contractor safety records shall be available upon request by the Contracting Officer. The Contractor shall be liable for, and shall indemnify and hold harmless the USCG against, all actions or claims pertaining to the

“personal” safety requirements of employees. The Contractor shall provide all personal protective equipment (respiratory protection, hearing protection, gloves, steel-toed shoes, safety glasses, protective clothing, etc.). In addition, the USCG is not responsible for medical examinations, hearing testing, substance abuse testing, or any other type of testing for Contractor employees.

5.0 Purchase Order Information

5.1 F.O.B. Point

The F.O.B. point for this order is F.O.B. Destination. Prices in the schedule include all charges for:

travel time; on-site training hours; airfare; lodging; local transportation; and any other travel costs as well as any applicable customs, duties, and taxes. No additional charges will be allowed unless indicated by a bilateral modification to the order.

5.2 Inspection and Acceptance Criteria

Inspection and acceptance of the training shall be performed at destination by the technical POC identified on this order. Performance under this order will not be accepted by the USCG until the technical POC considers all training to be delivered in accordance with this SOW.
