About this file
This document is a Florida Department of Transportation (FDOT) Program Management Local Agency Program (LAP) Certification of Current Capacity form, which is confidential per Chapter 337.14(1) of Florida Statutes. The two-page form is designed for FDOT pre-qualified contractors to certify their current contract capacity when submitting bids. The form requires contractors to detail their total uncompleted work as of the 15th day of the month preceding or during the letting date, including all contracts and subcontracts awarded up to five days before the bid letting.
The form includes a detailed "Status of Contracts on Hand" table where contractors must comprehensively document all ongoing and awarded projects, listing the owner, location, contract amounts, subcontracted amounts, and remaining contract balances. Contractors must consolidate smaller contracts that individually do not exceed 3% of their total contracts and collectively amount to less than 20% of the total. The document requires the contractor to swear and certify the accuracy of the information, with space provided for the firm's name, signature, date, and the signatory's title, ensuring transparency and accountability in the bidding process.
