ADDENDUM TO 52.212-1 INSTRUCTIONS TO OFFERORS - OFFER SUBMISSION REQUIREMENTS
Offers shall be submitted electronically to Richard.leistiko@va.gov. Offerors are cautioned that there are email size limitations. Consequently, offerors should email each volume separately. Also, if a volume is large in size, offerors should submit multiple emails for that volume. Offerors whose emails are rejected due to size limitations are not excused for late submissions.
Note late offers will not be accepted. The designated Government point of contact must receive the proposal by the date/time specified. For example, if the proposal is due at 2:00 PM PST, and the Government POC receives it at 2:01 PM PST, the proposal will be deemed late and will not be opened or considered for award unless an exception under FAR 15.208 applies (which is very rare). Offerors are advised that large attachments to emails may delay transmission and should plan accordingly. Offerors may want to request acknowledgement from the Government POC of its offer.
Email subject lines should be: Proposal - Company Name - 36C26118R0080 Volume X.
Offers shall consist of Volume 1: Business Proposal, Volume 2: Technical Factor, Volume 3: State Licensing, Volume 4: Past Performance, Volume 5: SDVSOB/VOSB Credit. If all five (5) proposals are not received, the offer will be considered non-responsive, and will not be considered for award. A proposal submission checklist is included below. Offerors are highly encouraged to use it to avoid being non-responsive.
All offers will be evaluated on a comparative analysis basis by the station SME.
Responsiveness

Cover letter which affirmatively states the period for acceptance of its offer is at least 120 calendar days.
Cover letter which provides the name, telephone, and email address of the person authorized to engage in discussions should the Government elect to negotiate.
Have an active account with System for Award Management (SAM) at the time the proposal is submitted and complete the representations and certifications as an entity eligible for contract awards. Ensure the NAICS identified by the SF 1449 is listed in SAM.
Cover letter which validates that the SAM information has been updated and is valid at the time of proposal.
Fully complete the contract line items (CLINS) in the price schedule.
Fully complete Attachment C.

If the offeror intends to sub-contract any of the temporary staffing, provide the following information:
1. Letter of Commitment or Subcontracting Agreement signed by authorized agent for subcontractor;
2. Acknowledgement from the offeror and proposed subcontractor that as a small business the offeror will perform at least 50% of the work with its own personnel, unless the subcontractor is a similarly situated concern. Offeror should identify what percentage of performance will be attributed to each entity.
Evaluation Criteria Volume 1: Technical Factor
Provide a performance plan that identifies the following information:
1. Whether or not sub-contractors will be used during the life of the contract
2. Plan of Action for if/when a delivery is going to be late, or a substitute driver will be used
Volume 2: State Licensing
Provide proof of licensing for the State of California to distribute or administer contracts covering the delivery and inspection of gases and supplies – will accept subcontractor licensing if California state law permits.
Volume 3: Past Experience
Provide up to five (5) recent and relevant past experience references. Recent is defined as within three (3) years of the proposal due date. Contracts that have been performed less than 6 months from proposal due date will not be considered. Relevant is defined as providing medical gases and supplies.
1. Title and description of the service
2. Location
3. Contract value
4. Period of performance
5. Contact information for the reference to include: Name, Company Name, Phone, Email
Volume 4: SDVOSB/VOSB Credit
Please read “VAAR 852.215-70 Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors (DEC 2009)” very carefully and proceed with your answer. Failure to respond to this Factor will be evaluated as ‘no credit’.
1. For SDVOSBs/VOSBs: In order to receive credit under this Factor, an Offeror shall submit a statement of compliance that it qualifies as a SDVOSB or VOSB in accordance with VAAR 852.215-70 (Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors). Offerors are cautioned that they must be registered and verified in Vendor Information Pages (VIP) database (http://www.VetBiz.gov) by no later than the solicitation response due date. Also include a print-out of your current verified VIP record.
2. For Non SDVOSB/VOSB Offerors Proposing to Subcontract to SDVOSBs/VOSBs: To receive some consideration under this Factor, an Offeror must state in its proposal the names of SDVOSB(s) and/or VOSB(s) with whom it intends to subcontract, and provide a brief description and the approximate dollar values of the proposed subcontracts. Additionally, proposed SDVOSB/VOSB subcontractors must be registered and verified in Vendor Information Pages (VIP) database (http://www.VetBiz.gov) by no later than the solicitation response due date in order to receive some consideration under the Veterans Involvement Factor. Also include a print-out of the current verified VIP record of the SDVOSB or VOSB with whom your firm intends to subcontract.
1. QUESTIONS
Offerors must submit questions in writing to Richard.leistiko@va.gov. The cut-off for questions is on or about December 27, 2021. Telephone calls will not be considered.
2. EVALUATION OF OPTIONS
For purposes of award of this Contract, the Government intends to evaluate the option to extend services under FAR 52.217-8 as follows: The evaluation will consider the possibility that the option can be exercised at any time and can be exercised in increments of one to six months, but not for more than a total of six months during the life of the contract. The evaluation will assume that the prices for any option exercised under FAR 52.217-8 will be those rates in effect under the contract each time an option is exercised under this clause. The evaluation will therefore assume that the addition of the price or prices of any possible extension or extensions under FAR 52.217-8 to the total price for the basic requirement and the total price for the priced options has the same effect on the total price of all proposals relative to each other, and will not affect the ranking of proposals based on price, unless, after reviewing the proposals, the Government determines that there is a basis for finding otherwise. This evaluation will not obligate the Government to exercise any option under FAR 52.217-8.
