REQUEST FOR QUOTE
483-11MR-6045
SPECIFICATIONS, DESCRIPTION, AND WORK STATEMENT FOR PLANTS
1. SCOPE OF WORK:
The Contractor shall furnish all materials, accessories, services, transportation, and equipment, plus perform all necessary labor required to maintain and guarantee all live plants in indoor public areas and outdoors per attached plant inventory list at the National Mine Health and Safety Academy, Beaver, West Virginia.
The Contractor shall be responsible for maintaining all plants and performing work incidental thereto by replanting, replacing, watering, weeding, cultivating, fertilizing, pruning, controlling insects and diseases, reguying, and performing all operations necessary for the care and promotion of root growth and plant life.
All work shall be performed at the Academy. Working hours will be scheduled around Academy occupancy. If services are performed after normal Academy working hours, the Contractor shall obtain written consent from the Contracting Officer’s Technical Representative COTR prior to performing the service.
LICENSE REQUIREMENTS:
The Contractor must be licensed to apply pesticides and shall provide proof to the COTR of a valid West Virginia Applicator's Permit.
2. CONDITION AND TREATMENT:
The Contractor shall perform the required maintenance as listed:
(a) All plants shall have soil, stems, and foliage treated to remove all soil and foliage insects and diseases. Essential nutritional elements shall be provided to promote growth.
(b) All plants (18 feet in height or greater) shall be rooted and foliage-pruned.
(c) All plants (up to 14 feet in height) shall be shade-grown or acclimatized.
3. GENERAL CARE SCHEDULE:
The Contractor shall:
(a) Water large trees once every two weeks or as needed to maintain a healthy, vibrant condition. Water small trees and plants once a week or as needed to maintain a healthy and vibrant condition.
(b) Mist three times a week or as needed: Monday, Wednesday, and Friday.
(c) Remove all trash and foliage three times a week: Monday, Wednesday, and Friday.
4. INTENSIVE CARE:
The Contractor shall:
(a) Fertilize once every three or four months in winter and every two months in summer or as needed to maintain a healthy, vibrant condition of the plants.
(b) Apply insecticides and pesticides once every two months or as needed to maintain a healthy, vibrant condition of the plants.
(c) Restock and/or realign as required by the Contracting Officer (CO), the Contracting Officer’s Technical Representative (COTR), or the Contractor's supervisor.
(d) Move or turn plants for light response as directed by the CO, the COTR, or the Contractor's supervisor. The COTR will approve the moving of all government owned plants.
(e) Leaf polish and foliage shine as directed by the CO, the COTR, or the Contractor's supervisor to maintain a healthy, vibrant appearance.
5. FACILITIES PROVIDED:
The U.S. Government shall provide access to hot and cold running water in standard closet facilities on each floor. The U.S. Government shall provide ladders and lifts necessary to maintain tall plants and trees. Use is to be coordinated with the COTR. The Contractor shall coordinate with the U.S. Government in advance to assure that elevator service is available for the necessary period of time to move plants in their containers from one floor level to the next if required.
6. GUARANTEE:
The Contractor shall guarantee as part of the bid that all products will carry a guarantee against insects, diseases, and all defective materials for a period of one (1) year from the effective date of contract. The Contractor shall be required to correct any and all defects in materials and/or workmanship that may appear during the guarantee period by repairing and/or replacing with new materials, if necessary, at no cost and to the satisfaction of the U.S. Government. If a new Contractor wins a successor contract, the Contractor shall be responsible for turning over all plants live and in healthy condition to the new Contractor.
7. REMOVAL OF DEAD OR DYING PLANTS FROM THE ACADEMY:
The procedures listed below shall be used whenever the Contractor replaces plants (shrubs, vines, and/or trees) at the Academy:
(a) The Contractor shall show the COTR the dead or dying plant and the replacement plant.
(b) The COTR shall ensure that the replacement plant is equal in value to the plant being replaced.
(c) The COTR shall prepare the Optional Form 7 (Property Pass), describing the plant to be removed and will give the property pass to the Contractor.
(d) When removing the dead or dying plant from the Academy grounds, the Contractor shall stop at the gatehouse and give the security guard the property pass for the plant that is being removed.
(e) After the above procedures have been complied with, the dead or dying plant will be considered to be the property of the Contractor.
(f) All of the plants that are maintained belong to the Academy. As property of the Federal Government, they cannot be given away. Before any plants are replaced, the Contractor must first notify the COTR of the Contractor's intent and receive the COTR's approval.
8. CONTRACTOR PERSONNEL:
The Contractor shall furnish personnel who are medically fit, trustworthy, competent, and well qualified for their work. Prior to employment, the Contractor shall submit a resume (with a list of three character references) for each proposed employee to the COTR. All employees must have the approval of the COTR before they begin work.
The Contractor employees shall wear uniforms and identification of suitable type and design as approved by the COTR. The Contractor shall furnish such apparel, and it is to be clean and sanitary when worn.
The Contractor shall assure that all Contractor employees comply with Academy regulations concerning safety, health, and the maintenance of discipline. Prior to the commencement of work under this contract, the Academy's safety office shall conduct an orientation session with the "key" Contractor personnel, advising them of safety and security regulations and policies which are to be observed at all times.
All employees of the Contractor shall be restricted only to areas involved with the performance of their assigned duties.
The Contractor shall provide his employees with identification cards that clearly designate each individual as an employee of the Contractor. The Contractor shall supply the COTR with a list of all employees and update the list, as personnel turnover requires
9. LOCATIONS OF PLANTS:
Attached is a plant inventory that lists the maximum number of plants the Contractor will be required to maintain and guarantee at the National Mine Health and Safety Academy. On or before the first day of contract performance, the Contractor and the COTR will jointly review the inventory for accurate number and placement of plants. If extra maintenance is required to restore the plants to normal conditions or to replace them, an equitable adjustment will be made to this contract. If no special care and/or replacements are required, the Contractor shall maintain the plants for the price that is included in the contract. The Contractor shall not begin any restorative work or make any replacements until an agreement is reached between the Government and the Contractor and the CO authorizes the restorative work.
The locations of the plants that the Contractor shall maintain and guarantee at the National Mine Health and Safety Academy, Beckley, West Virginia, are listed on the attached Plant Inventory.
