1. SF 1442: 13.a; Solicitation Pages 5/6: "Form/Copies"

0. The SF 1442 states "original and 1 copies". The table on solicitation pages 5 and 6 states "Paper: Original" and "Electronic: 1 CD…". Please clarify if the offeror is to submit (1) original, (1) hard copy, and (1) CD copy each for the Technical and General Offer/Price proposals, OR if the offeror is to submit (1) original and (1) CD copy each for the Technical and General Offer/Price Proposals. Please submit one original and 1 CD for volume I and one original and 1 CD for volume II.

1. Volume I - Technical Proposal Contents: Solicitation Pages 6/7 & 9

1. Item "b" at the bottom of page 6 states "Additionally, Volume I shall contain the following technical evaluation and additional information as follows:". It then goes on to mention, at the top of page 7, items "iii. Past Performance of the Prime Contractor" and "iv. Safety or Environmental Violations and Experience Modification Rate (EMR)". However, there are no "Factors" broken out for either of these two items. The bottom of page 9 states "Note: Prime Contractor Past Performance Ratings and EMR/Safety Ratings will be reviewed and ratings assigned under the Determination of Contractor's Responsibility portion of the award process". Please confirm if Past Performance (PPQs) and EMR/Safety are to be physically included in the Technical Proposal, OR, if the Technical Proposal ONLY CONSISTS of Factor 1 (Specialized Experience and Technical Competence) and Factor 2 (Project Schedule). As stated on page 9 of 53 of the solicitation, past performance and EMR will be reviewed and ratings assigned to determine if a vendor is responsible prior to award. This is not an evaluation factor.

1. Past Performance Questionnaire

2. The PPQ states that the reference contact is to complete and submit the PPQ back to the VA by June 26, 2019. As this date falls well after the proposal due date, is this date in error, or is it accurate? We are accustomed to PPQs being due before the proposal due date. Questionnaires should have stated they are due by 5/31/19 (the same as when the proposal is due). However, amendment A001 was posted that extended the due date to June 11, 2019 at 4pm CT. That is also when the questionnaires will be extended to.

3. The second item "ii.", towards the middle of page 7, states "Representations and Certifications and Self Performed Work Statement". Please clarify what is meant by "Self Performed Work Statement". Is this something that is part of the Reps and Certs, or is it in addition to the Reps and Certs? This is simply some type of statement provided by the vendor stating what percentage of work will be completed by the PRIME and what percentage will be completed by subcontractors. Usually vendors support this with what amounts are projected to be paid to the prime and what percentage is projected to be paid to the subcontractors.

4. The solicitation states a minimum of three projects but does not state a maximum number of projects. Is there a maximum number of projects the offeror can submit?

There is no maximum number of projects that can be submitted. Just ensure that the vendor still meets the other proposal requirements (i.e. maximum number of pages, etc.).

a. The solicitation asks for information on recent and relevant projects but does not specifically state what types of information the offeror should submit (e.g., project title; project location; project owner and POC information; project award, NTP, and completion dates; project award and completion amounts; project description; etc.). Please clarify what project-specific information the offeror should submit in Factor 1. The Government did not specify exactly what vendors must provide for this factor, just that “Offerors must submit work on at least three recent and relevant hospital projects and at least one of which must be valued at $1 million or greater. Recent is defined as work completed within the last 8 years. Relevant is defined as projects similar in nature to the project in this solicitation for scope, magnitude and complexity and have a logical connection with the requirements of this RFP.” Therefore, the vendors have flexibility in what proof they would like to supply. However, the Government needs to be able to verify the project existence so basic project information is recommended (i.e. location, POC, contract amount, project description, etc.).

6. The solicitation states "Offeror will identify all self-performed work elements and identify all major work elements performed by subcontractors, including the name(s) of their proposed subcontractor(s).” As this solicitation is LPTA, and we will not know which subcontractors we are intending to use until bid day, and after a subsequent vetting process, we respectfully ask that this requirement be removed from the solicitation. The Government does require to know in advance who the subcontractors are planned to be. We understand that substitutions must sometimes be made after project award but a list of subcontractors projected to be used needs to be submitted with the proposals.
