STATEMENT OF WORK

FOR BARRIER FREE PRIVACY CURTAIN SYSTEM /

TEXTURED CURTAINS

FOR THE

VA LOMA LINDA HEALTHCARE SYSTEM OUTPATIENT WOMENS CLINIC

1. Introduction

The purpose of this statement of work (SOW) is to outline the tasks and responsibilities of the successful offeror in relation to providing one new barrier free track and privacy curtain track system and textured curtain- Lorton Group Patient Lift Privacy Systems (Non-Ceiling Mounted Privacy System) oval track or Equal to, compatible Easy-On Off textured cubical curtains or Equal to, to be used at our new VA Loma Linda Healthcare Systems Outpatient Women’s Clinic.

2. Background The VA Loma Linda Healthcare System currently is utilizing the Lorton Group Patient Lift Privacy Systems (Non-Ceiling Mounted Privacy System) oval track and Easy-On Off textured cubical curtains in exam rooms, which have been proven to be effective for barrier free around patient lift assist, resulting in overall better patient care for our veterans. The VA Loma Linda Healthcare System is activating a new Women’s clinic and it now needs another system to treat patients there.

3. Scope This procurement is to provide our Loma Linda Outpatient Women’s Clinic with (26) configurations of Lorton Group Patient Lift Privacy Systems oval track with Easy-On Off textured cubical curtains in exam and procedure rooms. Field verification and installation are required on-site.

4. General Requirements Vendor shall provide to VA Loma Linda Hospital, (26) configurations of Lorton Group Patient Lift Privacy Systems track with two sets (52) total of Easy-On Off textured cubical curtains at the new Loma Linda Women’s clinic.

5. Mandatory Tasks

5.1. The vendor to coordinate with the VA point of contact (POC) for the delivery of items.

Notification of expected shipment date shall be provided no later than (14) business days prior to shipment.

5.2. The vendor shall verify quantity, configuration.

5.3. Vendor to make a site visit to field verify upon award prior to product installation.

5.4. The Vendor shall ship Lorton Group Patient Lift Privacy Systems oval track, Easy-On

Off textured cubical curtains to the 10391 Corporate Dr., Redlands, CA 92374

5.5. The vendor shall also install to Loma Linda Women’s clinic 10391 Corporate Dr., Redlands, CA 92374

5.6. Once product is received at the site, the vendor is to be responsible for any replacement punch items to be delivered within (10) business days to ensure product to be installed complete.

5.7. Vendor shall provide all necessary materials, equipment, labor, supervision, and management to provide, deliver, and install items.

5.8. The vendor’s installers shall deliver the Lorton Group Patient Lift Privacy Systems oval track with Easy-On Off textured cubical curtains, assemble and install onsite.

5.9. The vendor shall contact the VA POC to coordinate the punch walk for final inspection of product installation.

6. Period of Performance/Acceptance

6.1. The Period of Performance (PoP) is 60 After Receipt of Order (ARO) this time frame includes delivering, assembling and installation of the equipment.

7. Delivery Schedule

7.1. Coordination with the VA POC is required for the delivery of items. Notification of expected shipment date shall be provided no later than (14) business days prior to shipment.

7.2. The contractor shall provide a delivery schedule based on the procurement requirement

(14) days after award of the contract.

8. Schedule and Order Management

8.1. The above delivery schedule is based upon the current construction schedule for this building where Lorton Group Patient Lift Privacy Systems oval tracks with Easy-On Off textured cubical curtains will be installed.

8.2. The vendor shall not increase prices for a change in delivery date for up to 180 days due to construction delays.

8.3. The awardee actual delivery date will be confirmed by VA POC upon award

8.4. Post- award the vendor will coordinate delivery prior to beginning any work. Once the dates have been confirmed, modifications to the schedule are subject to written approval by the VA POC and the vendor. Schedule changes that are beyond the dates specified in the contract will require approval by the Contracting Officer and a contract modification. Vendor shall follow FAR 52.212-4 (f) for Time Extension Requests.

9. Delivery and Receiving

9.1. Delivery and receipt of the proposed items is anticipated to be directed to the location identified below. Confirmation of delivery location will be provided post-award. To coordinate delivery, vendor shall contact the VA POC.

9.2. The delivery of items identified in this document shall take place during normal business hours which are defined as: 0700 to 1530 (i.e.: 7:00am to 3:30pm Pacific Standard Time), Monday through Friday, and excluding Federal Holidays.

9.3. There is no loading dock, however, there will have a designated alley area large enough for designated trucks to offload product. Vendor shall coordinate with the VA POC details of the designated area for the delivery / deliveries.

9.4. Trucks shall be unloaded, moved from the alley, and then brought back to the alley if required to haul out any waste, tools, or excess materials.

9.5. Vendor shall coordinate with the VA POC details of the designated area for the delivery/deliveries.

9.6. Labeling of delivered items shall include the VA facilities contract number and VA purchase order and contract number for identification and reference upon receipt of product.

12. SALIENT CHARACTERISTICS

12.1. Product Features for Lorton Group Patient Lift Privacy System Oval Track or Equal to, Easy On- Easy Off Cubical Curtains or Equal to

13. Vendor Responsibilities

13.1. The vendor shall be responsible for delivery of all the products listed in Section 11.

13.2. The vendor shall be responsible for configuring and assembling (26) custom configurations of Lorton Group Patient Lift Privacy Systems oval track.

13.3. The vendors shall provide installation for (26) custom configurations of Lorton Group Patient Lift Privacy Systems oval track, two sets of Easy-On Off textured cubical curtains (52 total).

13.4. The vendor shall provide design services- verification of quantity, custom configurations, and field verification.

13.5. The vendor shall not provide refurbished equipment.

13.6. After award, the vendor shall provide the VA POC shop drawings must be provided as PDF and/or AutoCAD.

13.7. After award, the vendor shall provide the VA POC the warranty literature information must be provided.

13.7.1. Minimum (1) One-Year Warranty.

14. Job Site Cleanliness

14.1. There are no dumpsters available for vendor use. All vendor trash is to be handled by the vendor and removed from site daily.

15. Assembly, Installation and Final Acceptance

15.1. The vendor is required to manage and coordinate delivery and acceptance at the

Loma Linda Women’s Clinic with the VA POC.

15.2. The vendor is required to define the need for a staging area to accommodate item delivery, if needed. Post-award, this information will be confirmed with the vendor.

15.3. The vendor is required to provide tools, labor and materials to complete assembly and installation of the items detailed in this document.

15.4. The vendor shall protect all finished spaces and surfaces as required from delivery - damage. The vendor shall use covering and protection to the extent necessary to prevent damage to finished spaces. Any damage occurred during delivery is the responsibility of the vendor. The vendor will be responsible for paying and repairing any damage or noted deficiencies to finished spaces and surfaces that occur as a result of the vendor’s (or associated sub-contractors) installation.

15.5. During the entire duration of assembly and installation, the vendor will have a representative reachable via phone as the vendor’s contact, and to serve as the interface between the vendor and VA POC. All instructions provided from the VA POC to the representative will be binding as if given to the vendor’s main contact. The VA POC may provide specific instructions, however, only the Contracting Officer may change the terms or conditions of the contract.

16. Design Services

16.1. The contractor will be required to provide basic design services and layout for curtain tracks being requested to included quantity per each specific item and room placement as part of the design submittal described in this contract.

16.2. The VA POC will provide AutoCAD drawings for the floor plans to the contractor after award. Contractor shall be responsible for all field measurements. Contractor shall not rely on AutoCAD drawings for final measurements. See floor plans (KM CBOC FL01 and KM CBOC FL02) to use as reference of room locations and space layouts.

16.3. The vendor shall provide a designer with a 2 year minimum experience with the specified product under this requirement.

16.4. Meeting Requirements for the vendor:

16.4.1. Vendor shall coordinate conference calls with the VA POC as required to review curtain track layouts, to share progress and to make necessary revisions. Meetings shall be documented by vendor, with meeting minutes provided to VA designated POC for review and concurrence.

16.4.2. After award, a site visit to the VA Loma Linda Women’s Clinic for field measurements is to be coordinated with the VA POC.

16.4.3. All calls, meetings, and site visits will be during normal business hours which are defined as: 0700 to 1530 (i.e.: 7:00am to 3:30pm Pacific Standard Time). Monday through Friday, and excluding Federal Holidays.

16.4.4. During first meeting the vendor shall provide the VA POC with the PDF/CAD drawings showing layouts of awarded product.

16.4.5. The vendor shall attend a post-installation meeting to assess, address, and document any punch-list items and shall submit to VA POC a remediation plan within 5 business days.

16.5. Design Deliverable requirements:

16.5.1. The vendor shall allow for three (3) revisions per line item included in design services including updating AutoCad Drawings and PDFs as request by the VA

POC.

16.6. The vendor shall be responsible for providing all sample materials for awarded product (2-Day Delivery) as requested by the VA POC.

16.7. The product must not be placed into production without clearance from the VA

POC.

16.8. The vendor shall provide final drawings of product layout for sign-off prior to scheduling manufacturing.

16.9. The vendor shall track manufacturing schedule and notify VA POC with updates of estimated completion date by email.

17. Product Inspection and Warranty

17.1. The vendor shall verify and inspect all items. Any and all deficiencies

(damage/overage/shortage) shall be brought to the attention of the assigned VA POC in writing, preferably via email, immediately upon identification.

17.2. The Vendor shall store damaged items in the location designated by the VA POC. The Vendor shall maintain a complete file of all documents relating to each discrepancy and copies of all Discrepancy Reports shall be forwarded to the VA POC daily.

17.3. Vendor shall coordinate with the VA POC for replacement of damaged, defective, or missing items.

17.4. Vendor shall inspect to ensure that the curtain tracks are free of surface dirt, clean and polished, free of defects, and that the installation is complete and ready for use.

17.5. The warranty of an item is to start on the date of acceptance of the products/services by the VA POC.

17.6. The Vendor agrees to furnish, without cost to the Government, replacement of all parts and material which are found to be defective during the warranty period. The cost of installation and travel of replacement material and parts shall be borne by the Vendor.

18. Receiving Reports

18.1. The VA Loma Linda POC will be responsible to provide all the vendors for the clinic activation with receiving reports.

18.2. Vendor must ensure they follow up with the VA POC if they have any issues with the receiving reports. (Please contact VA Loma Linda Healthcare System’s receiving department at 909-825-7084).

2. Easy On – Easy Off Cubical Curtains:

2.1 The Easy On - Easy Off Cubicle Curtains are open C-type fast install and removal curtain system. Cubicle curtains to be manufactured flat and sized approximately 10% wider than the track length, unless specified otherwise. The curtain height will be determined by the floor-to-ceiling height and curtains should be finished approximately 10”-12” above finished floor, as suggested by Joint Commission's patient privacy standards. This distance will allow for ease in periodic floor cleaning and prevents mop and/or floor cleaner from coming in direct contact with cubicle curtain, soiling the treatment.

2.2 The C-type rings to be inserted and spaced minimum 6” and maximum 8” apart on a minimum 4” and maximum 6” National Fire Protection Association (NFPA) approved

vinyl breathable mesh fabric that is integrated to minimum ½” and maximum 1“ vertical and horizontal hole size opening mesh (minimum 70 percent open) allows for fire sprinkler penetration in the unfortunate case of fire. The mesh will be required to have a minimum 18” drop from sprinkler head to fabric. If the distance of the Flat Track is farther than the 18” drop then mesh may not be necessary to meet patient privacy standards.

2.3 The cubicle curtains must use a commercial grade of molded synthetic polymer C-type rings that allow for Easy On -Easy Off handling of environmental services and nursing staff. The ring system must mount on the approximately between .25”-.5” x 1.5”-2” Flat Track and must glide effortlessly without binding over the life of the curtain.

2.4 The bottom hem of the cubicle curtain is double folded and minimum 2” and maximum 3”. The side hems are double folded and minimum 1” and maximum 2”. All hems and seams are sewn with a double needle lock stitch. All vertical seams are surged and sewn with a double needle lock stitch for maximum durability.

2.5 The cubicle fabric will contain the following characteristics to be approved by the facility:

meet NFPA 701 flame spread requirements for vertical flammability (flame spread certification paperwork must be supplied), fabric must have a antimicrobial properties that have been proven to reduce bacteria on soft surfaces, must be “evidence based” third party tested, produces bacterial reduction within one hour and antimicrobial properties must be intrinsic and irreversibly bound to the polymers of the fabric and are able to withstand a minimum of 200 commercial launderings at a temperature of not more than 160 degrees Fahrenheit (71 degrees Celsius).

NO EXTRA TIME WILL BE GIVEN, Offerors are advised that it is your responsibility to review and monitor the website frequently to ensure you have the most up to date information, including amendments. Offeror shall quote all line items under this requirement and the pricing terms of the quotation shall be valid for at least 90 days unless a longer time is stated in the quote. Files should be sent in PDF format and no larger than 10MB or they will be automatically rejected by the server.

Vendor shall expect a confirmation e-mail indicating offer was successfully received by the Government. The Government is not responsible for rejected emails that exceed the e-mail capacity. Telephone and fax responses shall NOT be accepted. Telephone inquiries will not be honored. Inquiries after the allotted period may not be responded to due to the time constraints of the procurement. E.2.3 SUBMISSION OF QUESTIONS: All questions must be received by 11:00 AM PDT, January 24th, 2022. Questions received after the stated date and time, may be addressed at the discretion of the Contracting Officer.

E.2.4 DISCUSSIONS WITH OFFERORS: The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint.

However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be in the best interest of the Government. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.

E.2.5 SUBMISSION OF REQUIRED DOCUMENTS: Submission of Required Documents listed herein is a condition precedent to award of a contract. In general, LIMIT your response to no more than thirty-five (10) typewritten pages with a 12pt font-size and Times new roman font type, including, at a minimum, the following documentation:

Submissions can be sent in multiple emails to avoid computer system email size limitation, to Local CO name and email address: TBD. Quotes submitted by any other method will not be considered. All proprietary information shall be clearly marked. The use of hyperlinks in quotes is prohibited. Late quotes will not be accepted for evaluation. The contractor is responsible for confirming the Government’s receipt of the contractor’s quote.
