INTRODUCTION

Terra Environmental has been contracted by Mr. Jesus Osuna from City of Los Angeles to develop a Procedure 5 work plan to establish a protocol for the excavation, contaminated soil disposal and abatement of underground TSI pipe and Transite pipes, a known asbestos containing material. The proposed land grading and excavation activities required for the VA – Field North of Bldg. 116 Improvement Project necessitates the abatement of the underground TSI pipe. Mr. Ulises Monsalvo, Cal/OSHA

- Certified Site Surveillance Technician- CSST No. 04-3604 and Mr. Israel Monsalvo (CAC #04-3551) of Terra Environmental performed a site inspection and survey on December 14, 2018. Construction or grading activities have been performed to date and the ACM- TSI and Transite pipes had been disturbed due to such activities. Bulk samples of the ACM-TSI pipe were collected, the 12” transite pipe and 4 “ TSI pipe and debris contains asbestos (see attached asbestos report). The site is in the corner of Pershing Ave and Grant Ave, City of Los Angeles inside the Veterans Hospital, on public area., (see attached site map).

SCOPE OF WORK

1. Removal of top soil 70,000 sf - six to twelve inches depth

2. Removal of compacted soil 30,000 sf – six to forty eight inches depth

3. Removal of remaining 12 and 8 inch TSI pipes 260 linear feet each

4. Removal of contaminated stock piled soil (4 piles) 300 cubic yards total

5. Disposal of contaminated pipes 40 cubic yards

6. Removal of compacted soil to 5 feet for plumbing lines

The work of this section shall be performed by a Contractor duly licensed in the State of California in accordance with the provisions of Chapter 9 of Division 3 of the Business and Professions Code, as amended. The Abatement work of this section shall be performed by an entity holding a license with an “C22” Asbestos Certification as issued by the contractors State License Board.

The selected Licensed Asbestos contractor (Unknown) –should perform the following during the ACM-TSI pipe excavation and soil abatement project:

• Prior to start of work, the Contractor, shall isolate the regulated work areas using caution tape and posting warning signs and labels in Spanish and English around the perimeter of the affected area as per 29 CFR 1926.1101 (6) and (7). The barrier tape and signage shall be posted at a distance far enough away from the work area to allow a person to read the sign and take necessary protective measures to avoid exposure.

• A two-stage worker decontamination unit shall be provided at the entrance of the work area.

• Personal worker air monitoring for abatement workers shall be performed by the Contractor.

• Workers must wear disposable coveralls, ½ face air-purifying respirators with P- 100 HEPA filters, safety glasses and any other appropriate safety equipment.

• Documentation shall show that all employees performing the cleanup activities have medical examinations and training in accordance with 29 CFR 1926.1101.

Also, documentation of successful respirator fit testing for all employees within a one-year period of the time of the project.

• Periodic area air monitoring by the Owners Environmental Consultant should be performed as a precautionary measure to evaluate engineering controls during all phases of the abatement project. The samples shall be analyzed by Phase Contrast Microscopy in accordance with NIOSH 7400 method.

The excavation of the ACM pipes, contaminated soil shall be performed as follows:

• Remove contaminated top soil to a 6” – 12” bed, using pressurized cabin excavator operated by AHERA certified worker. See attached site plan.

• Remove contaminated compacted soil, 6” and upon completion of underground survey, remove 4 feet depth for new plumbing and foundation trenches.

• Dispose stockpile soil using pressurized cabin excavator operated by AHERA Certified worker.

• Remove the cross contaminated pipes following safe work methods by AHERA Certified workers.

• The trenches shall be excavated to an approximate depth of 4 feet and a width of 4 feet to access the pipe. Comply with trenching and excavation requirements of 29 CFR 1926.651 and 1926.652 or comparable OSHA- state regulations plan requirements and procedures.

• The pipe will be removed in 4 to 8 feet manageable sections by hand methods.

The pipe will be removed intact, and if cutting or breaking is needed the pipe will be adequately wetted prior to and during the cutting activities.

• A layer of six-mil poly may be spread beneath the break areas extending up the walls of the trench and two feet in either direction of the break to collect the broken pieces of pipe.

• The exposed areas of piping will be sprayed with a lock-down encapsulant prior to transport.

• The excavated sections of piping shall be placed on a drop sheet and then double wrapped in two six-mil transparent, leak tight printed bags, sealed with “goose neck” method. The pipe shall be properly labeled and then transported to a poly lined, labeled and lockable construction dumpster for future transport and disposal as non-hazardous, non-friable asbestos-containing waste.

• The material shall be adequately wetted during the removal and wrapping process to prevent visible dust emissions.

• The poly drop sheets and debris from the trenches shall be double bagged and properly labeled for transport and disposal as hazardous asbestos containing waste.

• Following the removal of the ACM-TSI pipe, each cut zone in the trench shall be inspected for the presence of any remaining ACM-TSI pipe debris. Should any asbestos containing debris be discovered, the materials shall be immediately wetted, manually cleaned by HEPA vacuum methods and additional 6 inches of soil will be removed until no debris are visible.

• All equipment and tools used during this abatement project shall be decontaminated using HEPA vacuum and wet wipe methods prior to being removed from the site.

• All asbestos waste shall be stored in a poly lined, labeled and lockable construction dumpster until future transport and disposal.

GENERAL

A licensed asbestos abatement contractor, will handle all asbestos abatement work and management of contaminated materials. The excavation and abatement work must be performed by persons knowledgeable, qualified and trained in the removal, treatment, handling and disposal of asbestos-containing materials and subsequent cleaning of the affected areas. In addition, such persons shall comply with applicable EPA, Cal-OSHA, SCAQMD Rule 1403 and local mandates. All asbestos-containing material shall be disposed as required by law.

The contractor shall supply all labor, equipment, materials, services, insurance and incidentals that are necessary or required to perform the work in accordance with applicable governmental regulations.

Disposal shall be performed by issuing proper waste manifests (hazardous and non-hazardous), signed by the owner or designated site representative and the disposal site.

All waste manifests and site field logs and data shall be submitted to the Project Construction Manager at the conclusion of the abatement project.

I. Executive Summary, Introduction, Purpose and Scope of Services

Mr. Ulises Monsalvo of Terra Environmental Services Inc. (TERRA) performed a limited asbestos inspection and site survey on December 14, 2018 at 11303 Pershing Ave. Los Angeles, CA 90025. On December 28, 2018 Terra Environmental collected additional samples on the North section of the site (Compacted soil area).

The purpose of the survey was to identify asbestos containing material (ACM) and asbestos containing construction material (ACCM) that will be impacted by the excavation for the VA-Field North of Bldg. 116.

II. Visual Survey, Sampling Methodology, and Analytical Procedures

a. Visual Survey

The Visual Survey consisted of a walk-through and visual inspection of the materials to be impacted. It included the identification of all suspect ACBM and the physical touching of suspect ACBM in an effort to determine the friability and condition of said materials. In surveying the building, we used our training in identifying asbestos-containing materials, our familiarity with building construction and our general experience to locate potential sources of ACM and ACCM.

b. Sampling Methodology

The next phase of the survey was the selection of sampling areas and collection of bulk samples. Material sampling areas were grouped based on material homogeneity. A homogeneous material is one, which contains the same texture, color, and uniform, applied during the same general time period. Terra representative used destructive sampling methods, some of the materials were damaged at the time of the inspection.

c. Analytical Procedures

Analysis of the bulk samples was performed in general accordance with the procedures outlined in the United States Environmental Protection Agency’s (EPA) “Method for the Determination of Asbestos in Bulk Building Materials” (EPA/600/R-93/116, July 1993), This method employs Polarized Light Microscopy (PLM) with dispersion staining to identify the type and approximate quantity of asbestos present in the sample, if any.

The EPA considers materials to be asbestos- containing only if it contains more than one percent asbestos as determined by visual area estimation (NESHAP regulation - 40 CFR 61 Part M, dated November 20, 1990). The California Division of Occupational Safety and Health (Cal/OSHA) has defined Asbestos Containing Construction Material (ACCM) as any manufactured construction material, which contains more than one-tenth of one percent (0.1%) asbestos by weight.

The bulk samples were collected and submitted, utilizing chain-of-custody (COC) procedures to AIH Laboratory located at 12611 Hiddencreek Way #B in Cerritos, CA, to perform the requested analyses. AIH Laboratory is accredited under the National Institute of Standards and Technology (NIST) and the National Voluntary Laboratory Accreditation Program (NVLAP # 500079-0).

III. Discussion of Survey Findings and Recommendations

TERRA collected a total of six (6) samples of suspect asbestos containing materials for analysis. The visual inspection and bulk sample analysis results revealed the following Asbestos-Containing Materials:

On December 28, 2018 nine (9) soil core samples and two surface were collected on the compacted soil area, the samples were collected on a random grid pattern method at three different depths ranging from 6 to 48 inches. Asbestos containing debris were observed at few locations (grids) and those locations were not sampled and determined to be contaminated. See attached site plan. The visual inspection and bulk sample analysis results revealed the following Asbestos-Containing Materials:

Asbestos containing materials were detected by the laboratory analysis of the bulk sample, soil core samples and soil surface samples.

IV Recommendations for handling ACM:

Removal and disposal of Asbestos Containing Underground TSI pipe, TSI Debris and Contaminated Soil must be performed by a California Licensed asbestos abatement contractor, in accordance with all applicable regulations, including but not limited to, 29 CFR 1926.1101 (OSHA), 40 CFR 763 (AHERA), 40 CFR Part 61 (NESHAPS) and 8 CAC 1529 (Cal/OSHA Asbestos), including mandatory and non-mandatory appendices as applicable, and Local Air Quality Management District regulations (SCAQMD 1403) Procedure 5.

Should materials different to those identified in this report or, other forms of suspect hazardous materials be discovered during the renovation process, the contractor should be instructed to cease all work activities which may initiate an exposure episode and notify the appropriate management personnel.

V. General Recommendations and Notes

TERRA has endeavored to observe the existing conditions within the subject property using generally accepted procedures. Regardless of the thoroughness of a survey, there is always a possibility some areas containing asbestos were overlooked or were inaccessible, or are different from those at specific sample locations. Therefore, conditions at every location may not be as anticipated by our field representative. In addition, renovation or construction may uncover altered or differing conditions.
