Scope of Work Project # 662-18-013 Replace and Correct Deficient Doors/Stairs/ Electrical/Steam Line San Francisco VA Health Care System

GENERAL INTENTION:

1. Contractor shall furnish all required labor, materials, equipment, and supervision to perform the work required by this Statement of Work and referenced attachments at the San Francisco VA Medical Center, 4150 Clement Street, San Francisco, CA 94121.

2. Project duration shall be 360 calendar days from the issuance of the Notice to Proceed by the Contracting Officer.

3. This project must be coordinated with the facility through the Contracting Officer's Representative (COR), to ensure that impacts to patient care and normal operations of the hospital are minimized.

SPECIFIC INSTRUCTIONS:

1. Per Spec Section 010000 on use of heavy equipment such as long reach fork lifts, cherry pickers, and cranes which will block roadways inside SFVA, contractor shall provide minimum 2 weeks’ notification to the COR for proper coordination with the VA Police. When parking spaces will be impacted, the minimum notification will be 4 weeks.

2. SFVA has on going agreement with the neighborhood community to limit construction work hours to Monday to Friday 7:00 AM to 6:00 PM. For weekends it shall be 8:00 AM to 6:00 PM. All after hours and weekend work shall be coordinated with the COR to allow adequate notifications to the neighbors.

3. All construction activities shall be coordinated with the COR to ensure that patient care and normal operations of the hospital will not be interrupted.

4. Provide a schedule of work to the COR within 14 days after Notice to Proceed.

5. After award of contract, in the schedule of values, the contractor shall identify the cost associated with each task of the project that will be applied to the invoice submittals.

WORK REQUIRED:

Task 1 – Replace/Create Doors at various Buildings:

Replace all doors listed below. See Attachment - A for door locations. Except otherwise noted, all replacement doors shall be exterior grade 90 minutes rating metal construction. Each replacement door shall be hung by continuous hinge; painted to match existing doors of their respective locations; plus, offset mounted 4” W x 24”H minimum rated glazing; plus 8”H minimum Stainless-Steel kick plates full width of door; plus, new exit hardware with cored lever on the outside; and new closure; and applicable to both leaves in the case of double doors. Note that where a kick plate is specified for a door regardless of the height, the door assembly must still retain the 1-1/2-hour fire rating.

1. Double Door at B200 Basement Exit into the Sunken Yard (For Location see Attachment A, Page1). Replace double door and frame. Install Stainless Steel 3-1/2 ft. tall kick plate, both leaves.

2. Double Doors at Building 200 Entrance into Building 2 (For Location see Attachment B). Remove and discard existing double doors and hinges and patch all holes on existing frame by approved methods to not compromise the 2-hour fire rating of existing walls at that location. Install a new 90-minute rated door frame in front of the existing frame and double doors to replace the removed doors. Note door swing shall be from Building 200 into Building 2. Work shall include relocating the existing door holding magnets from building 200 to work with the new door leaves. Per Attachment - B, position new door as noted to work with the relocated magnetic holders. Both new door leaves shall be hung with Stainless Steel full swing type 3 hinges. Install only flush mount exit crash bar hardware with top mount latching. Levers shall be coreless type. Caulk and paint gap in between the old and new frame for a seamless installation. Paint doors with color matching other stairwell doors of building 200. The existing pull station on the building 2 side shall be retained but relocated to work with the revised door swing. A new pull station shall be installed on the building 200 side and connected to the fire protection system of building 200. Contractor shall be responsible for all the work associated with integrating all the above into the existing fire alarm system of building 200 or building. The magnets shall be programmed to be released by the fire protection system of either building. Note that this hallway is heavily used by patient and staff and therefore must remain open to foot traffic. All work at this location shall be performed only in the weekends.

3. B200, Ground Floor Level, GA101 entrance door into Storage Closet. (For Location see Attachment A, Page 3). Glazing will not be required on this door. (Note the back door into the laundry chute is not to be replaced in this project).

4. B200, Ground Floor Level, Clinic exit into West Stairwell (For Location see Attachment A, Page3). – this area must be secured after hours. Work must be performed during afterhours. All hardware must be carried thru the exterior door of building 200 into the stairwell to reach the door location, not thru the interior clinic areas; and the same shall apply to the removal of all debris.

5. B200, Ground Floor Level, Clinic exit into East Stairwell (For Location see Attachment A, Page3). – this area must be secured after hours. Work must be performed during afterhours. All hardware must be carried thru the exterior door of building 200 into the stairwell to reach the door location, not thru the interior clinic areas; and the same shall apply to the removal of all debris.

6. B200, First Floor Level, Medical Practice exit into West Stairwell (For Location see Attachment A, Page 4). – this area must be secured after hours. Work must be performed during afterhours. All hardware must be carried thru the exterior door of building 200 into the stairwell to reach the door location, not thru the interior clinic areas; and the same shall apply to the removal of all debris.

7. B200, First Floor Level, Eye Clinic exit into East Stairwell - (For Location see Attachment A, Page 4). this area must be secured after hours. Work must be performed during afterhours. All hardware must be carried thru the exterior door of building 200 into the stairwell to reach the door location, not thru the interior clinic areas; and the same shall apply to the removal of all debris.

8. B200, Second Floor Level, Radiology exit into West Stairwell (For Location see Attachment A, Page 5). – Work must be performed during afterhours.

9. B200, Second Floor Level, Radiology exit into East Stairwell -(For Location see Attachment A, Page 5). Work must be performed during afterhours.

10. B200, Third Floor Level, OR Suite exit into West Stairwell (For Location see Attachment A, Pg. 6). – The OR Suites area has the most stringent Infection Control requirements of the hospital. All materials and debris must be carried into the site via the stairwells. Work areas must be completely segregated from the inside of OR and SPD by full plastic barriers sealing floor to the ceiling. Workers entering these areas must be fully suited up by OR gowns to be picked up from the OR and the SPD Reception Desks. Only after setting up the construction barriers can the existing door be removed and the new one installed. When setting up the barriers, before the barriers are fully operational, and when removing the barriers at the completion of work, all workers must be fully suited up since they will be physically in contact with the interior areas. Only carry the new doors up the stairwells, not thru the SPD area and the same shall apply to the removal of all debris. All work must be completed and all debris removed within the same day including painting. Work can only be performed from Friday after 6 PM thru Monday before 5 AM. In case any OR suite is needed to perform emergency surgeries, contractor may be instructed to immediately reinstall the existing door and terminate the work, to be recommenced after the completion of the emergency surgeries.

11. B200, Third Floor Level, SPD Suite exit into East Stairwell (For Location see Attachment A, Pg. 6)- the SPD area is one of the highest secured areas of the hospital. All requirements listed for the OR Suite exit into West Stairwell also apply to this door except as stated. All work must be performed in business hours within the same day under full escort by designated SPD personnel. Provide minimum 2 weeks’ notice to the COR to make this arrangement.

12. B200, Third Floor Level, double door adjacent room 3B-163 (For Location see Attachment A, Page 6) the replacement doors shall be smoke rated because the associated wall sections are smoke partitions. Levers shall be coreless type. Note the leaves of this double door open in opposite directions and shall retain that way. Note also the flooring is slightly higher on one side.
