1. Page 9 of the RFP lists Special Standards of Responsibility. In which volume, price or technical, should this information be included? Whatever is not price related goes into technical.
2. Page 6 of the RFP states that we must include a copy of our join At venture agreement. In which volume, price or technical, should this information be included? Technical but does not count against the 30 pages
3. The RFP include Representations and Certifications. Do these need to be submitted in our proposal? If so, in which volume should they be included? Reference FAR 52.204-7 and 52.204-8
4. Page 9 of the RFP states that, for Factor 1: Past Performance, “Offerors are cautioned to submit enough information in the format specified in the solicitation.” What format would the Government like us to use for this information, in addition to the provided PPQ form? You can use any format that reflects the required information. Commonly used is the PPQ and CPARS.
5. Can you please tell me what in the mechanical schedule can you define is remaining as existing equipment? All mechanical schedules on drawing MH601 are new equipment.
6. What equipment is part of the bid? Specifically are the hot and cold water coils marked HC- 17 and CC-7. existing, or needed to be included in bid? Hot Water Coil HC-17 and Chilled Water Cooling Coil CC-17 are parts of new AHU-17. HC-17 and CC-17 are new and should be included in bid with AHU-17. All Mechanical Equipment coils accessories as required for a complete replacement of the AHU and all other equipment scheduled or specified shall be
NEW.
7. Page 5 of the solicitation, Statement of Work, paragraph II, states, “The existing Cath Lab equipment is old, out of useful life and no longer function.” Please confirm whether or not the GC is responsible for removing this equipment or if the VA will remove all existing equipment.
If the GC, please confirm that the existing equipment is not to be salvaged in any way, and the contractor can remove and dispose of this equipment in any manner they see fit within the requirements of VA standards, federal, state and local laws. Philips will disconnect and remove existing cath unit(s) and associated equipment.
8. Section 01 45 00, paragraph 3.4(B), states, “The CQC system Manager is required to be a graduate engineer, graduate architect, or a graduate of construction management, with a minimum of 5 years construction experience on construction similar to the scope of this Contract.” This requirement is costly and not typical for the OKCVA. In an effort to reduce cost to the VA, would the VA please consider accepting a QC Manager with no degree, but 10 years of construction experience with five years of Quality Control Manager experience on similar projects? Acceptable
9. In an effort to reduce cost to the VA, would the VA consider allowing one person to serve as the Site Superintendent, SSHO and QC Manager, so long as they meet all the qualifications?
If not, would the VA consider allowing the Superintendent to serve as the SSHO, therefore only requiring two full-time site personnel. Having three full-time onsite personnel for this project will significantly drive up the cost. No issue if they meet all requirements to fulfill each position.
10. Section 01 45 00, paragraph 3.4(C), states, “These [QC Specialist] individuals or specified technical companies are directly employed by the General Contractor and cannot be employed by a supplier or subcontractor on this project…” The paragraph then lists out civil, mechanical, electrical, structural, architectural, environmental, submittals, concrete, pavement and soils, TAB, and design quality control manager. It is highly unusual for a small business to employ all these individuals. And, if not, hiring consultants for all these positions will significantly drive the cost up for this project. Would the VA please consider allowing these positions to be filled by either the GC’s Superintendent, SSHO or QC Manager, or personnel from subcontractors? Also, some of these categories are not applicable to the project. A single person can cover more than one area provided that the single person is qualified to perform QC activities in each designated area. Civil, Soils, Paving, and other categories not applicable shall be disregarded.
11. Please confirm that the existing Unistrut system is to be removed and reinstalled in a new location for the new equipment, and that no new Unistrut is required for this project. See sheet SF101. Detail F5 new Work shows additional rail and hangers compared to removed system rails shown on F1. Bracing requirements may also require additional Unistrut, refer to drawings and Cath equipment installation requirements. Damaged Unistrut during removal shall also be replaced with new. Unistrut removed and inspected can be reused if not damaged. Include in bid accordingly.
12. Please confirm that the Cath Base Plate shown on structural drawing S401, detail F6, is the same as the Philips Adapter Plate referenced on Note #4 on AE101. And if so, please confirm that that plate is provided by Philips, but installed by the GC in accordance with Philips drawing SL. Confirmed as “Adaptation Plate” F2 per Philips drawing SL, provided by Philips and installed and anchored by GC.
13. Please confirm that the Philips’ provided insulation plate that goes between the floor and floor plate is provided and installed by Philips. Confirmed per Philips drawings SD1.
14. On drawing AD101, note #1 states to temporarily remove and replace acoustical ceiling panels as needed to install MEP components along corridors outside of main ICRA contained area.
Please confirm that removal/replacement of the acoustical ceiling panels is only limited to what can be removed/replaced within the mobile containment cart area, and that no work can be performed within the corridor that is not within this mobile containment cart area. Correct, ICRA containment this area is highest risk and electrical work above ceilings in hallway requires Class III measures indicated.
15. Drawing AE701 lists multiple wall types, but drawing AE101 does not show any wall types F1, B6 or A. Please confirm wall types, F1, B6 and A are not applicable to this project.
Confirmed, wall types indicated are not applicable.
16. Drawing FA102 legend shows three types of alarms – F=fire alarm pull station;
=audio/visual alarm; and = visual alarm. However, within the Scrub room (1F122C), a different symbol is used - . Please define this symbol. Refer to symbol legend on sheet
FA102.
17. Is there an interstitial space below and above this space that plumbing, HVAC and electrical lines are ran? On each floor there is space above the finished ceiling and below the structure above but not an interstitial floor. It is approx. 1’-9” from bottom of deck to finish ceiling elevation above cath equipment, see RCP for ceiling elevation, with structural beams within the 1’-9” dimension. Contractor shall account for this in their bid as more conduit turns and careful trade coordination will be required to ensure MEP systems are encapsulated in the spaces and soffit shown on the drawings. Refer to drawings for more information. There is a structural floor frame system below with ceiling space below that will be required for access to accomplish some plumbing work as shown on drawings. See D6/S-401 and AE601 for representation of available above ceiling space.
18. PD101 includes several sanitary lines that require demolishing. Are those lines run below the floor in an interstitial space, or are they below a slab-on-grade in the earth, which will require significant concrete removal, excavation, backfill and pour-back, or are they located above the ceiling in space below, which would require removal of ACT/ceiling systems to access the piping? Pipes run below floor in space above ceiling at level below that will require access above ceilings to accomplish work. No slab on grade. Containment requirements apply similar to mobile cart work indicated in drawings outside of main scope area shown.
19. The Philips drawing A1 indicates that the doors into the storage room are to be hung from a ceiling mounted track, not floor track. However, drawing AE101, shows swing doors for Equipment Room 1F122B, and note #7 states to provide sound reducing door and frame.
Drawing AE701 states these doors are #4, which are wood doors with hollow metal frames. Is AE701 to be followed? If not, please revise the drawings and specifications as there is no information on what type to provide for the sliding doors on a track system. Provide acoustic rated swing doors and hardware per A/E drawings and disregard Philips sliding door information.
20. Section 08 71 00, Door Hardware, includes hardware sets that require power for automatic door operators and card readers. However the electrical drawings don’t appear to include power requirements for the ADO’s or card readers, as well as locations for the card readers.
Please advise. See Sheet EY101 for card reader locations in addition to hardware schedule indications. Refer to specs and existing door alarm system for wire requirements for card readers. ADO requires power, include in bid to provide power circuits to vestibule door ADOs indicated powered by circuit L12LS space 3 currently labeled as “spare”.
21. To help with calculating night and weekend work what hours are considered “Normal working hours” at this facility? 7:30 am to 4:00 pm. Refer to specifications for additional considerations on noise-producing activites.
22. Would we have to schedule after hours approval for work on all electrical panels or only for work on critical power panels? Panels serving this space not affecting other spaces can be worked on during normal hours. Any utility shutdowns including panels affecting occupied areas of the hospital shall be worked on after hours.
23. E-601, Conduit Required, Electrical Legend, Not all of the origin and termination points are shown on the plans, will the VAMC or Philips determine these locations at a later time? See Philips drawings for equipment locations. Information is contained in these drawings for conduit runs including shown on elevations and details within Philips set. Few items are called out on plans for “exact location to be coordinated by [VAMC] and Philips during installation”.
24. E-601, Electrical Legend, Note B “Furnished by customer/contractor and installed by customer/contractor” Should it be assumed that all lines marked with Note B will be furnished and installed by the contractor? Correct
25. EL-101, Rm 1F122E, What switching device do these two “C” type fixtures need to be tied in to? Response: The “C” is the light fixture designation, refer to light fixture schedule on drawing
EL501.
26. EL-101, Rm 1F124, Does this room also require an override switch for the ceiling mounted occupancy sensors? Response: Room is a storage space and does not require an override switch.
27. EP101-A, Duplex receptacles, What types of receptacles will be installed in these areas? Do different color receptacles need to be installed on the Critical Power circuits? Response:
Receptacles will be provided as indicated on floor plan, refer to specification 262726 for color requirements of bodies and cover plates.
28. EP502, Electrical Riser Diagram, 15, Will you provide contact information for the contractor that maintains the VAMC fire alarm system? Question references incorrect sheet. Refer to specification 28 31 00 for fire alarm requirements. VA personnel maintain fire alarm operation and maintenance.
29. EP502, Will modifying the currently installed fire alarm systems void current warranties or certifications? Will the contractor be responsible to get the modified systems recertified?
Question references incorrect sheet. Refer to specification 28 31 00 for fire alarm requirements, including certifications of workers, testing, inspections, maintenance. VA requires contractor test fire alarm operation under observation of VA fire alarm personnel who will provide sign-off.
30. ET101, General Electrical Telecommunication Plan Notes, G, Since the network cabling routing has already been designed, do we still need to provide our own engineered, signed drawings? The contractor shall provide required submittals of wiring system as specified as well as as-built drawings at conclusion of project.
31. ET101, Electrical Telecommunication Plan Notes, 2, Will VAMC terminate both ends of all CAT 6A cables? Yes, VA OIT personnel will provide cable terminations.
32. EY101, Electrical Power Plan Notes, 1,2, Will you provide contact information for the contractor that maintains the VAMC security system? VA personnel in locksmith department maintains system.
33. EY101, Electrical Power Plan Notes, 4, Will you provide contact information for the contractor that maintains the VAMC nurse call system? VA Biomed maintains nurse call.
34. EY101, Electrical Power Plan Notes, 5,6, Will you provide contact information for the contractor that maintains the VAMC audio system? VA electrical and fire alarm personnel maintain alarm/mass notification systems.
35. EY101, Will modifying any of the currently installed systems void current warranties or certifications? Will the contractor be responsible to get the modified systems recertified? Refer to specifications specific system requirements, including certifications of workers, testing, inspections, maintenance.
36. Will contractor be responsible for utility costs during construction? No, disregard metering and utility payment portions of specifications on this size of project.
37. Page 13 of Section 01 45 00 section states project to be completed in 180 days. SF 1442 states
300. Please advise which duration is correct. SF1442 governs, 300 days.
38. Can the superintendent fill the roles of SSHO and QC Manager or do these all need to be separate individuals? See item 10 response.
39. Please clarify who will be testing the existing slab for moisture? We are assuming we will need to hire a 3rd party testing company is to perform the test but want to make sure the VA isn’t responsible for this. Omit moisture testing requirement for slab, this requirement applies to slabs on grade for possible vapor transmission. This area is on a framed floor.
40. We cannot find a specific product/manufacturer listed on the Epoxy specifications. Please provide a list of approved manufactures and products please? Product shall meet specification requirements for ASTM, performance and warranty requirements and other specific requirements indicated in section. No specific products indicated.
41. The specifications and finish schedule on the drawings show two different LVT brands (one says Tarkett and the other says Armstrong). Please advise which brand is correct. Armstrong Jet Natural TP782 per sheet AI101 is correct.
42. Please clarify if a quartz broad cast should be included with the epoxy flooring. All bidders remove all epoxy flooring RES-1 from project drawings and specs. Substitute in bid for all RES-1 to change to WSV-1, refer to sheet AI101 and finish schedule for WSV-1 information.
Disregard epoxy specification.
43. In section 08 71 00 page 9- part 2.7 letter B- it states “provide sectional (lever x rose) lever design matching existing unless directed other by COR. No sub lever material shall be accepted. However the hardware sets show the mortise locks have an E type Escutcheon with the 17 Lever. Please advise if you do want the sectional lever and rose or the escutcheon and rose. Lever and rose
44. In section 08 7100 – hardware set 06- it states the 25 auto flush bolts, electric unlatch strike, overlapping astragal, to be lead lined. The manufacturers we go through are checking if they can lead line those items. If they cannot, did you have a specific manufacturer in mind so I can get pricing for you? Please advise. Lead lined astragals are common and can be utilized to shield over flush bolt hardware so flush bolts are not required to be lead lined. Research has found established suppliers with electric strike options for lead lined doors.
45. In section 08 7100- page 15-under 2.19 finishes- go to letter F- it states in the paragraph that hand operated hardware shall be provided with the antimicrobial coating. If you go to hardware sets 01, 03, 04, and 05 have the L series mortise locks. This series does not offer the antimicrobial coating. (avail on the hospital latches) but not on these. Is there another manufacturer you had in mind that provides this with this type of mortise lock? Please advise.
Research has found established suppliers offering this coating in L series mortise locks.
46. In section 087100 hdw set 07 the auto door bottom gives ansi certified # r0y346 im showing this ansi #does not exist with the manufacturers we use. Are you wanting this auto door bottom mortised or surface? Is there a certain manufacturer you had in mind for these door bottoms?
Please advise. Mortised into bottom of door in doors not lead-lined. Surface mount at lead lined doors if mortise cannot integrate into door without affecting lining. No specific mfr.
indicated, hardware shall meet warranty and other general hardware requirements in specifications.
47. G1 102 – G1 105 Will we need to work from a lift? Assuming we will need a safety plan? It also shows four phases. Will there be any down time between phases, or can we roll from one phase to the next phase? See specifications for lift and safety plan requirements. Phases are shown primarily in order of importance ensuring cath lab is operational as soon as possible with other phases able to be completed as determined by contractor schedule. Concurrent phasing acceptable provided it does not delay operational timeline of cath lab.
48. Commissioning is required, will the VA contract for the commissioning or do we need to bring a 3rd party commissioning agent in to complete this? Contractor responsibility for commissioning agent.
49. MD 102 note 6. Is there room for a crane in the area of the existing louver to be removed?
Also, is there space for staging the new AHU and removal of the old AHU? If so, assume we need to show it on our plan? Contractor responsible for siting of crane and traffic diversion and signage, staging shall be coordinated with and accepted by COR, space is available on site.
Previous alternative installation has been by transporting segments through freight/receiving areas to west stairwell from freight elevator at floor above and into mechanical floor through west access door, depending on unit assembly.
50. Would it be acceptable to provide CPARS evaluations in lieu of PPQs? Yes as long as it is relevant past performance
51. Would it be acceptable to provide previously completed PPQs? Yes as long as it is relevant

52. Per Sheet EY101 Electrical Power Plan Note states the existing Access Control Software is Software House but it is believed the existing is Lenel. Please Confirm. Schneider - Andover Continuum is the integrated access control system in this area.
53. Per Sheet FA101 General Fire Alarm Plan Notes states the existing Fire Alarm System is
Comba RH-7W22b-R but it is believed the existing is Gamewell FCI. Please Confirm.

54. Comba-RH-7W22b-R is believed to be DAS system. Please confirm if any work is needed for this system. No work on this system.
55. Will the proposed nurse call system be required to OIT TRM approved? Question not applicable as no new system is a part of this project.
56. Will the new nurse call be required to connect to Vocera communication badges, Cisco nurse phones, or any other facility system? Intent is for new devices simply to extend from existing nurse call central notification system in place. Nurse call tie in with other systems not a part of this project.
57. Will the Government require the new nurse call to be networked between other Acute Care or CBOC buildings? If so, which building are required to be networked together? Question does not apply.
58. Will the Government require the new nurse call to be connected to the Code Blue system, Overhead paging, wireless pagers, and Smart Beds for fall risk insight? No new system only extension of existing system with 4 new devices.
59. Will the Government require the new nurse call to send notifications or information to any system? No new system only extension of existing system with 4 new devices.
60. Will the Government require the new nurse call to enable a nurse to call back into the patient room over the pillow speaker for care-at-a-distance and safety reasons? If so, what version of the of Cisco Call Manager is deployed at the facility? Question not applicable.
61. Will the new nurse call be installed on Biomed HTM owned servers or OI&T owned servers? No new system only extension of existing system with 4 new devices.
62. Is the Government aware that connecting the nurse call to any of the systems mentioned in the above question list will require the new nurse call system to be TRM approved? No new system only extension of existing system with 4 new devices.
63. Will the nurse call need to meet the MOU dated March 26, 2020 VAAR Parts 802, 812, and
852 regarding Gray Market? No new system only extension of existing system with 4 new devices.
