Attached is the site visit sign in sheet and a mold risk management.

The proposal due date has been changed from 7/28/2018 to 7/29/2018 at 1:00PM MST to donna.gillette@va.gov

All other terms and conditions remain the same.

Scope of Work for Construction George E. Wahlen Department of Veterans Affairs Medical Center 500 Foothill Dr. Salt Lake City, UT 84148 The VA Salt Lake City Healthcare System located at 500 South Foothill Drive, Salt Lake City, Utah has a requirement to upgrade interior finishes in VCS Canteen. Due to health and safety concerns the current deficiencies below require immediate attention.

· All asbestos floor tile and mastic in kitchen Hallway, fire riser room and dry Storage room (1A02E) will be abated and replaced with VCT tile and cove base.

· Mold Growth on wall near hand washing sink in food prep area (1A02G) and on the South wall of the sink area will be remediated. The wall tile will be removed and all walls in the area will have FRP panels installed and 48” stainless steel corner guards on the outside corners.

· Hand washing sink located in food prep area (1A02G) and eyewash station located in Dishwashing room (1A02H) does not currently have hot water. Hot water plumbing needs to be repaired or replaced.

· There are floor, wall and ceiling penetrations in kitchen, food prep and food serving area that need to be properly sealed. Epoxy shall be used for floor and wall ceramic issues. Caulking or ceiling tile replacement for the ceiling.

· The flooring under the Soda Box stand is missing. An Epoxy floor sealant will be applied to this area.

· Kitchen and storage room (1A02E,1A02F,1A02G) walls have peeling paint and damage from moving carts and shelves. All walls shall have the damaged repaired and painted.

· Catering room(1A02F) has broken floor tile and partially cut off metal studs from the bathroom stall patriations protruding from the floor, there is also an abandoned toilet flange in the floor with a seal installed. Existing toilet flange shall be removed and capped, damaged floor tile shall be repaired with an epoxy floor resin.

· Walls in the Catering room walls have peeling paint and damage from moving carts and shelves. All walls need to have the damaged repaired and painted.

· Kitchen Hallway has been damaged from moving carts and shelves. Walls in hallway will have the damaged repaired and painted.

Plumbing

Contractor will be responsible for removal of all old plumbing lines in the contract area.

Asbestos All asbestos abatement shall be coordinated with the Asbestos Coordinator and done according to VA Asbestos Abatement Design Manual. The successful contractor will need to coordinate when this kind of work can be performed with the VA’s Contracting Officer or COR.

Period of Performance is 15 calendar days from Notice To Proceed.

1. The products used to remove the asbestos mastic underneath floor tile need 5 to 7 days minimum to outgas before new flooring can be installed. This needs to be allowed for in the schedule and therefore the 15 days appears too short. Please add for this in the contract duration. Once the Asbestos is completely removed the VA will address the schedule duration and determine what is required for the new flooring to be installed correctly.

2. Where wall tile has holes or cracks you appear to want an epoxy caulk to put into the holes or a skim. Please clarify this is correct. Fill holes and cracks with epoxy and smooth the surface over the holes or cracks.

3. There is visible mold growth on the South wall of the sink area. Is this in the scope of work? The SOW will be amended to show mold to be remediated.

4. Can the windows in the South Storage Area be opened to exhaust filtered negative air? No

5. Will the metal wall sheeting & dish washing equipment be removed to access the south side of the wall where the visible mold is? Only if the metal or equipment is required to be removed to complete correct remediation.

6. Will the VA’s 3rd party IH be running Base Line & Post Mold Air samples for this project. YES

7. Is there a written Mold removal protocol for this project? I have attached our internal Mold guidelines.

8. Can more time be allocated to the project to address solvent off-gassing during floor mastic removal? Once the Asbestos is completely removed the VA will address the schedule duration and determine what is required for the new flooring to be installed correctly.

9. Will the V/A representative hire a third party environmental consultant to run area and clearance samples, or will the V/A take care of this in house?

As per VHA Directive 7714 and local VHA Asbestos Policy, the MC will provide a VPIH to conduct area sampling outside of containment and final clearance samples.

10. What is the V/A going to do for clearance on the mold remediation area? Visual inspection only or air clearance samples?

The MC provided VPIH will also conduct clearance air samples for fungal spores.

11. If the V/A is going to run clearance air samples on the mold remediation containment there will need to be Pres-sampling done to determine a base line of what is already present on the interior and exterior of the building. Will the V/A take care of this in house?

The MC provided VPIH will conduct base line air samples. This has not occurred yet.

12. Just to clarify. On the SOW it says to remove asbestos floor tile and mastic in room 1A02G. However, there is no asbestos floor tile present only ceramic floor tile in that room. With does the V/A not want the abatement contractor to remove the ceramic tile in this room and others like it within the areas of construction?

The asbestos-containing 9” x 9” resilient floor tiles located in the dry storage room (1A02E) (and the green ceramic tiles located in the corner - the grout is suspect asbestos containing materials); in the kitchen hallway, on the far end of the serving line, and in the fire-riser closet are the asbestos related flooring to be abated. The ceramic tile located in 1A02G (kitchen), 1A02H (dish room), and 1A02F (former bathroom/catering storage) is not expected to be removed.
