Combined Synopsis/Solicitation Notice
Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Items,” as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.
This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-05.
This solicitation is a TOTAL SMALL BUSINESS SET ASIDE
· Please register with the Department of Veteran Affairs using the information in the form outlined provided in Attachment B. Failure to do so may significantly delay award time and payment due to processing time being 10-14 Business days
The associated North American Industrial Classification System (NAICS) code for this procurement is 339114 Dental Equipment and Supply Manufacturing with a small business size standard of 750 Employees The FSC/PSC is 6520 Dental Instruments and supplies
The Phoenix VA Health Care System is seeking to purchase Intra Oral Scanners

Salient Characteristics
Scanner
· Be able to open STL files
· Have scanner tips which can be autoclaved on average more than 150 times before replacement
· Have the ability to take a color shade measurement
· Be able to take intraoral camera HD photograph
· Be able to do patient specific motion
· Be able to identify tongue or moving object automatically
· Operate wirelessly
· Have trueness/accuracy at no greater than 6.9 +/- 0.9 µm
· Have precision/consistency of 4.5 +/- 0.9 µm
· Possess digital impression system without the requirement for powder or impractical stillness in scanning.
· Have indications for full dentures, aligners and additional ortho treatments.
· Have Color Pod Wireless 3D Scanners with five autoclavable scan tips
· Have three batteries and a charging hub.
· Have an annual operations license and one-year Limited Warranty
· Allow the design of crown/bridge creation via virtual design with models and splints design exportable via STL Design capability.
· Have caries, diagnostic and monitoring tools.

Segmentation laptops must
· Have be a 17.3” gaming laptop with three USB ports and a CD burner. A gaming laptop is a minimum requirement for having high speed processing of graphic software for viewing and segmenting STL files.
· Have 2.0 speaker configuration and 7.1 digital audio out using HDMI out con.
· Have a 256GB PCie SSD M.2 Hard Drive
· Have 16GB, 2x8GB, DDR4, 2400Mhz Memory
· Have at least CPU Intel Core i7-9700 processor and Windows Operating System
· Have an NVIDIA GeForce RTX 2070 6GB CDDR6 (OC Ready) Display
· Have dental design and implant processing software installed prior to delivery
· Have Killer E3000 2.5 Gbps Ethernet NIC, Killer Wireless 1550 2x2 AC and Bluetooth 5.0 (Standard) Network capabilities. The increased speeds are a new technology for helping process 3D imaging software and segmentation files for editing and viewing.
· Have design and implant software which allows for planning, placement and design of surgical guides via STL Design File Exportation.
· Have software platform functionality for design, implant, orthodontics, patient monitoring, treatment simulation, smile design and single platform.

· Have implant planning and surgical guides
· Have patient monitoring software which enables the ability to follow a patient’s dental situation over time with a visual overview of the patient’s history which can be compared between two specific visits for measuring changes between scans.
· Have a treatment simulator which allows the scanning of patient’s teeth to provide a visual for orthodontic treatment planning and case approval chairside.

The following solicitation provisions apply to this acquisition:
1. FAR 52.212-1, “Instructions to Offerors–Commercial Items” June 2021
2. FAR 52.212-3, “Offerors Representations and Certifications–Commercial Items” Feb 2021
Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, “Offerors Representations and Certifications–Commercial Items.” If paragraph (j) of the provision is applicable, a written submission is required.
The following contract clauses apply to this acquisition:

· FAR 52.212-5, “Contract Terms and Conditions Required to Implement Statutes or Executive Orders (July 2021

The following subparagraphs of FAR 52.212-5 are applicable:
All quoters shall submit the following:

· Please ask all questions via email only. No questions will be answered via phone call
· No hand delivered or mail delivered quotes will be accepted
Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government.
The following are the decision factors:

· Meeting all requested Salient Characteristics
The award will be made to the response most advantageous to the Government.
Responses should contain your best terms, conditions.
To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."

"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"
Quoters shall list exception(s) and rationale for the exception(s), if any.
