Yes, the correct form to utilize is the VA Form 10091 with the “Update” box checked, as opposed to the “New” box.

Block 14 of Amendment 2 states the purpose of the amendment, which was to replace verbiage found in the “Information… to bidders”, and update dates on block 10 of the solicitation. The Statement of Work found in Section B remains unchanged.

Typically for work on Federal facility licensing from another state has been acceptable, please clarify if license from another state satisfies this section or if only the C-10 License is acceptable as a response.

Confirm if no updates to existing information/SAM information we are only updating the station information and that won’t cause an issue with our
SAM registration must NOT be expired. SAM registration must be active. SAM information must match information on the VA Form 10091. I do not believe the station information provided on the 10091 is existing set up which is working fine for multiple stations, including Loma Linda.
within SAM, and therefore should not have any adverse effect.
However, I recommend utilizing the contact info provided on the VA Form 10091 to receive validation, as it is a separate agency responsible for these programs.
