Performance Work Statement Sterile Processing Service (SPS) Temporary Hires
1. General Information
1.1 Introduction and Overview
This requirement is for Sterile Processing Service (SPS) staff at New Mexico Veteran Healthcare System (VAMC). The incumbent rotates assignments among the various sterilization, process, decontamination, surgical/ dental tray assembly, flexible endoscope reprocessing, reusable inventory, medical equipment processing, and quality assurance functions. The incumbent may be required to work days, evenings, nights, weekends, and/or holidays.
1.2 Background
SPS now has over 200 complex Standard Operating Procedures and Manufacturer instructions and is currently lacking fully skilled technicians needed to interpret and carry out these complex instructions. Due to recruitment difficulties, SPS has been forced to hire less experienced staff in which SPS will have to use existing technicians to train these individuals in order to become fully functional and productive Medical Supply Technicians (GS-0622-6). The major challenge is related to the experience levels of the candidates being received and the compensation levels of the vacant positions. The candidates meet the OPM qualification standards which do not translate to being a trained Medical Supply Technician. As an example, the OPM qualification standards states that a Laboratory Worker qualifies as a Medical Supply Technician although the Lab position does not perform High Level Disinfection/Sterilization of specialty items such as flexible endoscopes and/or surgical instrumentation. The lack of truly qualified candidates has resulted in the inability to fill the large number of vacancies quickly. The compensation level and not able to provide a recruitment/retention bonus has also added to the delay in filling the large vacancies.
1.3 Scope of Work
Procurement of six (6) contract staff that can and will be used at the sole discretion of the facility based on staffing needs which can consist of up to 40 hours per week.
Services shall be performed for the Albuquerque New Mexico VAMC facility.
Work Schedule: Contract technician(s) shall be available Monday through Friday from 0800 to 1630 and 1630-0030. The work hours can per FTEE range from eight (8) hours to forty (40) hours per week.
Personnel may rotate shifts as needed. The shifts are as follows: Mon-Fri 0800-1630, excluding Federal Holidays
2. Requirements
2.1 Tasks and Skills Required
2.1.1 Knowledge Required by the Position
· Knowledge of Nursing and SPS policies and programs.
· Knowledge of the Spaulding Classification System sufficient to determine sterilization parameters for Reusable Medical Equipment (RME).
· Knowledge of microbiology in order to implement infection control policies and techniques, handle cross-infection problems, contaminated waste, and universal precautions concepts.
· A thorough knowledge of aseptic principles and techniques including characteristics of various types of detergents/enzymatic solutions and cleaning brushes, cleaning techniques, sterilizing mediums, sterilizer operations, tests to determine effectiveness of sterilization, and packaging, storing, and shelf-life characteristics to ensure RME is sterilized and stored according to manufacturer specifications for individual RME.
· Must possess working knowledge of all equipment providing decontamination and sterilization of RME including but not limited to: time and temperature, steam saturations and pressures. Knowledge and ability to assess sterilization cycles sufficient to troubleshoot and whenever possible, provide changes to ensure sterilization parameters are met.
· Thorough knowledge of the use, operation, maintenance, assembly, and disassembly of the full range of RME and the specific cleaning, sterilizing, and storing requirements of each to ensure the integrity of RME is not compromised. This includes knowledge of the anatomy and physiology of various types of RME including those used for invasive medical procedures. Knowledge of external and internal components of endoscopic equipment including suction ports, valves, and auxiliary water ports.
· A thorough familiarity with medical center and clinic layout, staffing and operations, and the integration of the sterile supply operation into the medical center.
· Knowledge of human anatomy and physiology, familiarity of commonly performed therapeutic, medical and surgical procedures, and the instrumentation utilized during these procedures. Knowledge is used to effectively communicate with physicians and other allied health personnel and to problem solve in the event of RME shortages and the need to substitute and/or offer alternatives.
· Skills to apply knowledge in adapting instruments to perform a full range of specialized tests or non-routine diagnostic or treatment procedures.
· Ability and skill to document information for records, reports, etc.
2.1.2 Supervisory Controls
The employee works under the general supervision of the SPS Supervisor, Assistant Chief, and/or Chief. The supervisor assigns the employee to various work areas. Employee works independently, consulting with the supervisor on unusual problems that develop which could include seeking urgently needed supplies from other medical facilities on a loan basis. While working on days, evenings, or weekend shifts, supervisory assistance may not be available for problems arising and the employee is responsible for solving these issues in consultation with the medical staff of the facility. The supervisor reviews work through observation of the work area, through discussion with the employee, and by means of feedback from the medical center staff.
2.1.3 Guidelines
The employee is required to apply and interpret VA and Joint Commission policies, procedures, and guidelines.
Guidelines include agency regulations, VA and VHA directives, Nursing Service manuals, and accepted practices. Must have in-depth knowledge and practice all guidelines set forth by the International Association of Healthcare Central Service Material management (IAHCSM) as well as both AORN and AAMI.
Guidelines consist of publications on sterile supply operations, clinic and surgical schedules, manufacturer guidelines, assembly and troubleshooting manuals, equipment operating instructions and shelf-life guidelines. Incumbent must follow established guidelines, but must use judgment in such activities as selecting pre-sterilization cleaning methods and sterilization mediums, placement of instruments in trays, substitution of unavailable items, resolving malfunctions in equipment and in emergency situations the employee may have to decide to use less desirable or more costly processing to make available urgently needed supplies.
2.1.4 Complexity
Assignments involve selecting the appropriate series of steps from several alternatives in each of the several different work areas. Work involves sterilization testing and recording the results on the equipment located in Sterile Processing Service. The incumbent will also be responsible for the preparation and sterilization of clinical and surgical sets and trays. The incumbent will be required to perform high level disinfection of endoscopes and various RME throughout the facility.
Assignments consist of several duties such as those involved in decontaminating, cleaning, and pre-sterilizing medical supply items. Assembling and sterilizing the full range of trays, and sets used in both the clinical setting and the Operating Room. Assignments consist of a series of steps involved in loading and unloading sterilizers, inspecting packages for integrity, validating the proper recording of data and placing this information in the department sterilization records. Concentration and discernment are required to the more complex instruments for those which are not complex. The incumbent uses judgment and initiative including selecting the appropriate equipment and methods, recognizing conditions which cause erroneous results, and troubleshooting technical problems which may occur during the decontamination and sterilization process.
2.1.5 Score and Effect
The work involves segments of the overall function of the section, ensuring the availability of a large number of medical supplies and equipment to medical center Operating Room, clinical services and other customers. The employee’s work directly affects patient care at the medical center and other organizations serviced. The work product consists of preparing trays, sets, and high level disinfection of reusable medical equipment such as flexible endoscopes, probes, and transducers.
2.1.6 Personal Contacts
Incumbent will have personal contact with other employees throughout the medical center who use sterile supplies and must have the ability to communicate effectively with them. They will be required to communicate with vendors and official visitors.
2.1.7 Purpose of Contacts
The purpose of contact is to receive requests for medical supplies or equipment, to clarify requests, explain delays, receive and check vendor implants and to provide advice concerning substitutes for supplies or other supply resources. This requires knowledge of nomenclature for a variety of different items. Since many of these items may have several names, this task required advanced experience and knowledge.
2.1.8 Physical Demands
The incumbent usually stands and moves from one part of the room to another while performing duties. Is frequently required to lift packages weighing 50 pounds, and regularly pushes carts weighing up to 300 pounds for distances up to 2,640 feet.
2.1.9 Work Environment
On a regular and recurring basis, the employee alternates between a contaminated environment and a carefully controlled clean environment. In both instances, the employee is required to wear special clothing, hair cover, mask/beard cover, full face shield and shoe covers, which can be uncomfortably warm. While handling contaminated medical supplies, the employee also uses high risk rubber gloves and an impervious gown. The work area is often hot and humid. The employee is subject to slipping on wet floors. The employee must use padded gloves to remove carts from sterilizers and trays from the carts. The employee is subject to burns from accidentally touching hot items, to cuts from both sterile and contaminated instruments, knife blades and needles, and dermatitis from frequent contact with cleaning agents.
The employee often works around body fluids, mucus, excretions, and bits of tissue, some of which may be foul smelling. Strong, unpleasant odors are encountered while disinfecting instruments, patient care equipment, and work surfaces. The work area is noisy due to the clatter of metal instruments, rumbling of carts, and operation of pre-sterilizing equipment. Use of ultrasonic equipment may require use of hearing protection.
Employee is exposed, on regular and recurring basis, to the residuals of the sterilant, ethylene oxide (EtO), a colorless gas with ether like odor. EtO is a skin, eye, and respiratory irritant. Prolonged contact with skin may result in delayed burns. Breathing high concentration of EtO may cause nausea, vomiting, and irritation of nose, throat, and lungs. Special storage precautions are to be taken, such as continuous ventilation and free sources of ignition. EtO must not be exposed to chlorides, oxides, acids, or other combustible materials. Employee must exercise various safety precautions, protective measures/devices, etc., e.g., wearing protective gown, mask, and gloves.
The above conditions place additional demands on the employee due to the stressful condition of being exposed to EtO gas, and the uncertainty of being exposed to a possible carcinogen and other long range effects.
2.2 Other Significant Facts:
HEALTH AND SAFETY
Complies with all safety and fire prevention rules and regulations, using protective equipment when required and provided. Promptly reports all accidents and notifies supervisory of unsafe or unhealthful conditions in the workplace. Attends scheduled training sessions and participates in facility or service level safety promotions as directed by supervisor. Knows fire drill and emergency plans for work area, including location of all emergency fire equipment.
CUSTOMER SERVICE
Meets the needs of customers while supporting the medical center and service missions. Consistently communicate and treats customers (patients, visitors, volunteers, and all medical center staff) in a courteous, tactful, and respectful manner. Provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the customer, constructively and appropriately.
ADP SECURITY
Protects printed and electronic files containing sensitive date in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, federal regulations, VA statutes and policies, and VHS&RA policy. Protects the data from unauthorized release or from loss, alteration, or unauthorized deletion. Follows applicable regulations and instructions regarding access to computerized files, release of access codes, etc., as set out in the computer access agreement the employee signs.
The C&A requirements do not apply. A Security Accreditation Package is not required.
3. Supporting Information
3.1 Place of performance
Staff will work in Sterile Processing Service (SPS) at the Raymond G. Murphy Veterans Affairs Medical Center (VAMC) located in Albuquerque, NM.
3.2 Period of Performance
The period of performance is one year after the Date of award (Base) with two (2) year options.
3.3 Special Considerations
The services provided by the contractor will be performed in accordance with VA policies, procedures, New Mexico VAMC memorandums, VHA directives, Nursing Service policies and procedures, Joint Commission Accreditation Manual and Patient Safety goals. Copies of policies will be provided to employees upon request, to include:
· Employee Attire
· Hand Hygiene Practices
· Absence and Leave
· Exposure Control Plan for Blood borne Pathogens (BBP)
· Information Security Program
· Privacy and Release of Medical Information and Freedom of Information Requests
3.4 Contractor Furnished Materials – N/A
3.5 Government Furnished Materials and Services
All agency staff must complete VHA Privacy training and VHA Cyber Security Awareness training provided to all new nursing employees prior to receiving any work assignments. Any monies paid out for orientation/training will be reimbursed by agency to facility in the event the employee does not return to duty.
3.6 Contracting Officer’s Representative
The Contracting Officer’s Representative (COR) will be designated in writing for furnishing technical guidance and advice during the term of the contract. This includes monitoring the general direction of the work to ensure full compliance with all terms and conditions of the contract.
3.7 Requests for Service / Cancellation
The COR or designee will provide to the contractor the number of hours required.
In the case of short-term notices for personnel, the Government shall use its best efforts to request personnel at least four (4) hours prior to the start of the shift.
3.8 Personnel Policy
The contractor shall be responsible for protecting the personnel furnishing services under this contract. To carry out this responsibility, the contractor shall provide the following for these personnel:
· Worker’s Compensation
· Professional Liability Insurance
· Income Tax Withholding
· Social Security Payments
The parties agree that such personnel shall not be considered VA employees for any purpose and shall be considered employees of the contractor. The contractor shall be responsible for the provision of appropriate parking passes for their personnel furnishing services under this contract.
3.9 Medical Records
Patient medical records are confidential and are protected by the Privacy Act of 1974, 5 U.S.C. 552a; VA Claims Confidentiality Statute, 38 U.S.C. 5701 and Drug/Alcohol Abuse Confidentiality Statute, 38 U.S.C. 7332 [Authority: Drug/Alcohol Reg. 38 C.F.R. 1.460]. Any individual making unauthorized disclosures may be criminally liable for violations of the Act.
3.10 Contract Administration
The Contracting Officer is the only person authorized to approve changes or modify any of the requirements under this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes, which will affect price, quantity, or quality of performance of this contract. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority, and no adjustment will be made in the contract price to cover any increase in costs incurred as a result thereof.
4. Qualifications of Key Personnel
4.1 Medical Supply Technician
· Experience in processing, assembling, trouble-shooting, preparing, sterilizing, storing, and issuing sterile and unsterile medical supplies and equipment.
· The applicant must possess knowledge of the technical names, general uses, physical characteristics, and operating conditions of medical supply items; knowledge of decontamination, cleaning, preparation, sterilization, and storage procedures; knowledge of aseptic techniques; and skill in operating a washer-sterilizer, ultrasonic instrument cleaners, and gas and steam sterilizers.
· Examples of qualifying specialized experience include work as an operating room technician, medical instrument technician.
· The applicant must be certified by (CBSPD) The Certification Board for Sterile Processing and Distribution
The contractor will be responsible to ensure that contractor employees providing work on this contract are fully trained and completely competent to perform the required work. The contractor is required to maintain records that document competence / performance level of contractor employees

5.2 Contractor Personnel Security Requirements
All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to all subcontractor personnel requiring the same access. If the investigation is not completed prior to the start of the contract, the contractor will be responsible for the actions of those individuals they provide to perform work for VA.
· Position Sensitivity - The position sensitivity has been designated as low risk.
· Background Investigation - The level of background investigation commensurate with the required level of access is National Agency Check with written inquiries.
· The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM), the contractor shall reimburse VA within 30 days.
· The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship and are able to read, write, speak and understand the English language.
· The contractor shall submit or have their employees submit the following required forms to the Contracting Officer within 30 days of receipt:
· Optional Form 306, Declaration for Federal Employment
· Department of Veterans Affairs Electronic Fingerprinting Memorandum
· Background Investigation Worksheet
· The contractor, when notified of an unfavorable determination by the Government, shall withdraw the employee from consideration from working under the contract.
· Failure to comply with the contractor personnel security requirements may result in termination of the contract for default.

C.2 SUPPLEMENTAL INSURANCE REQUIREMENTS
In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract:
(a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.
(b) General Liability: per occurrences.
(c) Automobile liability: per person; per occurrence and property damage.
(d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage.

C.3 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 Days.

C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 Days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 Days days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 09/30/2021.

C.5 VAAR 852.203-70 COMMERCIAL ADVERTISING (JAN 2008)
The bidder or offeror agrees that if a contract is awarded to him/her, as a result of this solicitation, he/she will not advertise the award of the contract in his/her commercial advertising in such a manner as to state or imply that the Department of Veterans Affairs endorses a product, project or commercial line of endeavor.
(End of Clause) C.6 VAAR 852.219-10 VA NOTICE OF TOTAL SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESS SET-ASIDE (JUL 2016)(DEVIATION)
(a) Definition. For the Department of Veterans Affairs, “Service-disabled veteran-owned small business concern or SDVSOB”:
(1) Means a small business concern:
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans or eligible surviving spouses (see VAAR 802.201 Surviving Spouse definition);
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans (or eligible surviving spouses) or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran;
(iii) The business meets Federal small business size standards for the applicable North American Industry Classification System (NAICS) code identified in the solicitation document;
(iv) The business has been verified for ownership and control pursuant to 38 CFR 74 and is so listed in the Vendor Information Pages database, (https://www.vip.vetbiz.gov); and
(v) The business will comply with subcontracting limitations in 13 CFR 125.6, as applicable
(2) “Service-disabled veteran” means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).
(b) General.
(1) Offers are solicited only from verified service-disabled veteran-owned small business concerns. Offers received from concerns that are not verified service-disabled veteran-owned small business concerns shall not be considered.
(2) Any award resulting from this solicitation shall be made to a verified service-disabled veteran-owned small business concern.
(c) Agreement. A service-disabled veteran-owned small business concern agrees that in the performance of the contract, the concern will comply with the limitation on subcontracting requirements in 13 CFR §125.6.
(d) A joint venture may be considered a service-disabled veteran owned small business concern if the joint venture complies with the requirements in 13 CFR 125.15, provided that any reference therein to SDVO SBC is to be construed to apply to a VA verified SDVOSB as appropriate.
(e) Any service-disabled veteran-owned small business concern (non-manufacturer) must meet the requirements in FAR 19.102(f) of the Federal Acquisition Regulation to receive a benefit under this program.

C.7 LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011)
This solicitation includes . Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an “Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement” to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs. Furthermore, if VA determines any services the support contractor(s) will perform in assessing compliance are advisory and assistance services as defined in FAR 2.101, Definitions, the support contractor(s) must also enter into an agreement with the contractor to protect proprietary information as required by FAR 9.505-4, obtaining access to proprietary information, paragraph (b). The contractor is required to cooperate fully and make available any records as may be required to enable the CO to assess the contractor's compliance with the limitations on subcontracting or percentage of work performance requirement.
C.8 VAAR 852.232-72 ELECTRONIC SUBMISSION OF PAYMENT REQUESTS (NOV 2012)
(a) Definitions. As used in this clause—
(1) Contract financing payment has the meaning given in FAR 32.001.
(2) Designated agency office has the meaning given in 5 CFR 1315.2(m).
(3) Electronic form means an automated system transmitting information electronically according to the Accepted electronic data transmission methods and formats identified in paragraph (c) of this clause. Facsimile, email, and scanned documents are not acceptable electronic forms for submission of payment requests.
(4) Invoice payment has the meaning given in FAR 32.001.
(5) Payment request means any request for contract financing payment or invoice payment submitted by the contractor under this contract.
(b) Electronic payment requests. Except as provided in paragraph (e) of this clause, the contractor shall submit payment requests in electronic form. Purchases paid with a Government-wide commercial purchase card are considered to be an electronic transaction for purposes of this rule, and therefore no additional electronic invoice submission is required.
(c) Data transmission. A contractor must ensure that the data transmission method and format are through one of the following:
(1) VA’s Electronic Invoice Presentment and Payment System. (See Web site at http://www.fsc.va.gov/einvoice.asp.)
(2) Any system that conforms to the X12 electronic data interchange (EDI) formats established by the Accredited Standards Center (ASC) and chartered by the American National Standards Institute (ANSI). The X12 EDI Web site (http://www.x12.org) includes additional information on EDI 810 and 811 formats.
(d) Invoice requirements. Invoices shall comply with FAR 32.905.
(e) Exceptions. If, based on one of the circumstances below, the contracting officer directs that payment requests be made by mail, the contractor shall submit payment requests by mail through the United States Postal Service to the designated agency office. Submission of payment requests by mail may be required for:
(1) Awards made to foreign vendors for work performed outside the United States;
(2) Classified contracts or purchases when electronic submission and processing of payment requests could compromise the safeguarding of classified or privacy information;
(3) Contracts awarded by contracting officers in the conduct of emergency operations, such as responses to national emergencies;
(4) Solicitations or contracts in which the designated agency office is a VA entity other than the VA Financial Services Center in Austin, Texas; or
(5) Solicitations or contracts in which the VA designated agency office does not have electronic invoicing capability as described above.

C.9 VAAR 852.237-70 CONTRACTOR RESPONSIBILITIES (APR 1984)
The contractor shall obtain all necessary licenses and/or permits required to perform this work. He/she shall take all reasonable precautions necessary to protect persons and property from injury or damage during the performance of this contract. He/she shall be responsible for any injury to himself/herself, his/her employees, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by his/her employees fault or negligence, and shall maintain personal liability and property damage insurance having coverage for a limit as required by the laws of the State of New Mexico. Further, it is agreed that any negligence of the Government, its officers, agents, servants and employees, shall not be the responsibility of the contractor hereunder with the regard to any claims, loss, damage, injury, and liability resulting there from.
