SECTION B - CONTINUATION OF SF 1449 BLOCKS

B.1 SCOPE OF WORK

1. Background. The West Texas VA Health Care System (WTVAHCS) has a requirement to remove and install 28 outdated patient ceiling lifts, install 9 new patient ceiling lifts, and provide two (2) 825lb motor extra motors. Current ceiling lifts were installed on multiple floors of the main building in 2009. The lifts are unsupported by the manufacturer and require to be replaced.

2. Place of Performance: George H. O'Brien, Jr., Department of Veterans Affairs Medical Center, 300 Veterans Boulevard, Building 1, Big Springs, TX 797200

3. Period of Performance. 180 Days after contract is fully executed/awarded.

4. Summary of Requirements.

a. Contractor shall provide all equipment, tools, labor, material, transportation, and supervision required to deliver, remove old ceiling lifts (including rail systems) (where applicable), and install ceiling lifts. Contractor shall provide post-installation weight testing/ certification of all ceiling lifts and provide on-site end-user training/ education services. The Contractor shall have authorization to re-sell and install the proposed ceiling lift equipment.

b. This SOW describes a Brand Name requirement for Guldmann Patient Ceiling Lifts implemented by FAR 6.302-1. See Brand Name justification posted with the RFQ.

c. Anywhere in the SOW where removal is required, the Contractor shall not reuse any end-of-life parts which includes but not limited to rails and motors. All new parts are required.

d. Characteristics include advanced ceiling lifts as specified (with fast motion, continuous-charge room-covering rail systems, and tool-free emergency lowering, plus scales or positioning locks where specified) are to be delivered, installed, and tested, in places listed in the chart below. Deinstallation and disposal of old lifts and unneeded rail and hardware is the contractor’s responsibility.

5. Technical Elements. Contractor shall provide ceiling lifts that meet the technical specifications of this SOW (detailed below):

a. Lifting Capacity: Proposed ceiling lifts (to be installed) shall have a minimum lifting capacity of 550 lbs, 605 lbs, and 825 lbs., with a standard carry bar.

b. Emergency Stop Function: All proposed ceiling lifts shall have both an electrical and mechanical emergency stop. Mechanical emergency stop is required to be a pull cord stopping mechanism that may be reached from standing person’s height.

c. All proposed ceiling lifts shall have an electronic soft start and soft stop function.

d. Integrated Scale: Proposed ceiling lifts shall have an integrated digital scale (for certain designated areas) that may be used to weigh patients. The ceiling lifts shall be able to zero and display patient weight on a display screen. Scale shall be able to display weights in both kilograms and pounds. The scale accuracy specification shall meet or exceed +/- 0.5% at max load.

e. Scales must have readout in the hand control and must not decrease the lifting height of the system.

f. Emergency Lowering: All proposed ceiling lifts shall have an emergency patient lowering option to safely lower patients from a standing person’s height without the use of tools or ladders in the event of lift malfunction.

g. All proposed ceiling lifts shall be motorized units. Prefer one (1) motor.

h. Hand Control/ Pendant: All proposed ceiling lifts shall have an integrated hand control or pendant that allows end-users to lift and lower patients. The hand control units shall have an LCD display screen. Hand control units shall have wireless option availability.

Remove and Replace/Install

i. Battery: Provided batteries shall have a life expectancy of at least 5 years (minimum specification).

j. Battery Charging: All proposed ceiling lifts shall be able to charge the batteries while the lifting system is located at any point on the rail (rail charging). Ceiling lifts shall re-charge automatically without prompt or action required by end-users at any location along the rail system. The max charging time to recharge battery completely shall not exceed 5 hours.

k. Lifts must be able to lift a full-capacity load at least 21 times on a full, new battery charge.

l. Proposed ceiling lifts shall be capable of lifting patients off the floor.

m. Display function: Ceiling lifts shall be able to display the following data: Total lift count, total hours, last/next service (PM) required.

n. Ceiling lifts shall be 110/120V, 60Hz operating devices and shall be GFCI protected.

o. Notifications/ Alarms: Proposed ceiling lifts shall have the following notifications and alarms:

1) Scale overload notification (visual on display).

2) Battery low notification (visual and audible sounds).

3) Visual display for when scale requires service.

p. Lifting Speed: Proposed ceiling lift shall have a minimum lifting speed specification of 5.5cm/s at max capacity load.

q. Proposed patient lifts shall not beep or flash during normal operation in order to avoid alarming staff and or patients.

r. There shall be disposable lifting sling options available for the proposed ceiling lifts.

Disposable lifting sling will not be procured as part of this contract.

s. Overload Protection: Proposed ceiling lifts shall have an overload protection mechanism.

t. Proposed ceiling lifts shall accommodate 2-point hanger bars that do not have easily breakable parts, such as rubber flaps.

u. Rehabilitation Training Function: Proposed ceiling lifts shall have a rehabilitation training function (capability) or mode (only for certain designated areas) that allows up to 220lb (100kg) of weight relief for a minimum of 30 minutes. The training function shall allow patients to conduct gait training, balance training, and sitting and standing exercise training with weight relief. This function is only required in Physical Medicine and Rehabilitation Services (PM&RS) rooms.

v. Positioning Locks: Ceiling lifts in the PM&RS rooms shall have positioning locks that allows end-users to secure the lift system in any position along the rail system.

6. Specific Tasks and Capabilities:

a. Removal and Installation shall be coordinated with the Contracting Officers

Representative (COR). Patient care is our top priority, work must be scheduled according to the availability of rooms as not to inconvenience the Veterans. The schedule shall be coordinated after contract award.

b. All installation must comply with requirements of Patient Safety Alert AL14-07, including the Checklist for Installation and Relocation. The installation checklist must be filled out for each installed lift, in addition to any checklist required by the manufacturer, and supplied to the COR. The checklist is included in the solicitation as Attachment 1.

c. Contractor shall provide personnel that are authorized/ certified installers of the Original Equipment Manufacturer (OEM) of the provided lifts, no exceptions. Personnel that are not certified/ authorized installers of the ceiling lift OEM are not permitted to execute ceiling lift installations (not acceptable to VA). Installers must have current certification to work on the provided overhead lift and rail systems. Certifications shall be provided as part of the contractors quote.

d. Certifications shall be provided by the manufacturer who will conduct testing to ensure that the ceiling lift and charging system are safe and in compliance with ISO 10535 & UL 60601-1.

e. The Contractor shall perform site survey of the pre-existing conditions and as-built drawings above and below finish ceiling at the installation location to confirm existing structural and ceiling conditions. Contractor will provide evaluation of existing structure where ceiling lifts are needed and submission of plans to WTVAHCS Engineering service, to assure that workable systems are installed and comply with VA design requirements and relevant standards. Plans shall be submitted after contract award.

f. Contractor will provide a list of all construction or modifications required to install these lifts, to be presented after contract award. The goal is to install around any obstructions without requiring VA to make construction modifications. This list shall be provided after contract award.

g. Verify minimum clearances for operation are compliant with manufacturer recommendations. (Ensure room clearance and that the ceiling height is adequate for lift usage.)

h. Contractor will coordinate with Engineering service to verify access to engineering mechanical, HVAC, and fire systems components within the mounting area of the lift units.

i. Contractor will provide all qualified personnel, materials, equipment, and services necessary to install ceiling lifts for the rooms described in the detailed table 2 including lifts, tracks, spreader bars, attachment hardware, and all other equipment necessary for safe operation. Detailed requirements are described in the Standards and Special Requirements sections, paragraph 7. Lift systems may be configured in any way that covers the lift area and meets the requirements.

j. Contractor shall complete testing and certification (weight/ safety testing) of all installed ceiling lifts prior to VA acceptance. Contractor shall be able to certify that all ceiling lifts are safe for patient use after the completion of post-installation testing/ certification.

Contractor will provide evidence of testing of all provided/ installed ceiling lift systems via a written report, according to VA checklist requirements. The VA checklist can be found in Attachment 1.

k. Contractor will provide as-built drawings and structural engineering drawings for lift units after contract award.

l. Contractor will mitigate any new hazards created by installation.

m. Contractor will meet construction safety requirements, including construction orientation, safety training requirements, dust control requirements from the Infection Control Risk Assessment, TB testing, electrical safety requirements, and Above Ceiling Permit requirements including re-sealing any penetrations of fire barriers.

n. Contractor is required for setting up any required barriers as required by VHA Infection Control Risk Assessment (ICRA) and Pre-Construction Risk Assessment (PCRA) documentation. Contractor is required for providing required equipment or materials for complying with ICRA/ PCRA requirements.

o. Contractors are expected to complete basic clean up after installing lifts into rooms, to include removal of their equipment and basic debris removal. Contractor is not required to conduct deep cleaning/ terminal cleaning of rooms after installation are completed (VHA responsibility).

p. Turnkey installation only applies to installation activities required to install the newly procured ceiling lift systems. Contractors will not be required for other room requirements, such as painting, providing ceiling tiles, or terminal room cleans.

Contractors will only be required to fix/ repair damages they cause to rooms.

q. Contractor shall clearly display the weight limit / capacity on each patient ceiling lift in a location that is clearly visible by all staff members.

r. If additional parts, labor, travel, and/or repairs are necessary outside the scope and pricing of the original contract, the Contractor shall contact the COR before completing such work. The Contractor shall be liable for any additional parts, labor, travel, and/or repairs (including labor hours) provided that are outside the scope and pricing of the original contract unless 1) the COR has been contacted about the additional parts and/or repairs required and 2) a corresponding contract modification has been put in place to cover and account for the additional parts and/ or work required.

7. Standards and Special Requirements:

a. Ceiling Lifts shall meet all Underwriters Laboratory requirements, life safety codes, national electric codes, fire safety codes, and all other applicable federal, state, and local codes. Appropriate documentation should be provided.

b. Ceiling Lifts shall comply with ISO 10535 (Hoists for the transfer of disabled persons – Requirements and test methods).

c. Ceiling lifts must include a 2-point hanger bar, suitable and allowable for use with the full range of loop-based Guldmann slings, Humancare/Barton slings, EZ Way slings, Handicare slings, and Alpha Modalities repositioning slings, with 2 hooks facing different directions on each end of the bar and without easily breakable parts such as rubber flaps.

d. Ceiling lift manufacturer must allow and warrant the use of their lifts with the full range of Guldmann, Handicare, and Humancare loop slings.

e. The lifts shall be mounted using ceiling supports as determined by building structure and room covering layout needs to provide lifting capability for the maximum area possible within each room which means wall to wall – not just over the bed.

f. Contractor must install lifts without relocating or interfering with operations of existing sprinkler heads, lights, HVAC grills, curtain tracks, IV tracks, televisions, permanent furniture, or other permanent structure. Contractor must describe any such fixtures that must be moved during contract bid and pre-construction meetings. Installation may be delayed, canceled, or moved to a mutually acceptable location if building changes cannot be made in ways that allow lift installation.

g. Contractor shall not breach any firewalls during installation of these lifts. Any fire barrier that must be penetrated must be discussed with VA safety personnel and must immediately have fire barrier repaired. An Above Ceiling Permit must be filled out before ceiling work, and the COR will check the ceiling after ceiling work, to make sure any fire barrier penetration was repaired.

h. Holes in the wall or ceiling tile must be reported to the COR and local management before leaving the room to allow the facility to repair as necessary.

i. Ceilings must not be slotted exposing space above ceiling to the room below.

j. The contractor must have visited the sites and assessed the structure of each ceiling lift location to ensure safe installation of the equipment and shall supply the installation plans to the WTVAHCS Engineering Service and Construction Safety Group, who must accept them before installation.

k. Ceiling lift installations shall comply with NFPA 13 for fire sprinklers and with NFPA 99 and NFPA 70 for proper grounding and bonding and access to electrical and safety systems.

l. The contractor should not require movement of any lights, sprinklers, televisions, or other fixtures to install ceiling lifts. If movement of preexisting fixtures is unavoidable, the contractor must include such notice well before installation, when the drawings are submitted.

m. Ceiling lifts must not add obstacles or impair any normal operations in patient care rooms.

n. It is permissible to raise the lifting height or extend the range of a lift system that is being replaced, so long as designs are approved by the facility in advance and all obstacles are avoided to avoid interference with the operation of curtains, ventilation, or other moving or ceiling mounted items.

o. All installation that affects existing areas and electrical work shall be coordinated with the WTVAHCS Engineering service.

p. The contractor will install all necessary mounts, braces, and any other components needed for full lift function and safety.

8. Warranty Requirements.

a. Contractor shall provide (for each ceiling lift system) a warranty that shall cover all tracks, installation, motors, other parts, labor, batteries, and travel. A three (3) year warranty term is preferred.

b. For future services planning only, the Contractor shall provide a specific opportunity for Service Agreements, Preventive Maintenance Agreements, and Extended Warranty Agreements with pricing. This information will assist with future Market Research. This requirement is not an evaluated pricing line item.

b. The warranty period shall be effective once all lifts are installed and accepted by the Government.

9. Training.

a. The contractor shall provide information for any free training offered by the manufacture.

b. In addition to any free training offered by the manufacturer, the Contractor shall provide certified in-service training for all shifts of direct patient care staff for any equipment new to this facility (up to 10 sessions in coordination with the COR). This training must include hands-on lift use and be extensive enough to prepare participants to train others.

c. The training Line Item shall be invoiced only for the amount of training sessions conducted.

d. Training must include each new type of equipment and sling used in an area.

e. Contractor shall provide certified in-service training on each site, multiple times in a day, for biomedical engineering staff in coordination with the COR.

10. Manuals. Contractor shall provide electronic copies of the user manuals and repair manuals for each different model of lift installed under this order or contract at no additional cost to the government.

11. Inspection.

a. Each room’s lift system shall be weight and function tested after installation in compliance with the VHA checklist required by Patient Safety Alert AL14-07. Each checklist shall be provided to Contracting Officer Technical Representative within 7 days of installation along with any checklist required by the manufacturer. Any changes in inspection requirements during the period of installation will be communicated to the Contractor immediately.

b. In accordance with Patient Safety Alert AL14-07, Attachment 1, design and structural drawings must be provided, and WTVAHCS stakeholders must have an opportunity to approve or ask for changes before installation.

12. General Information.

a. Concrete is the material used for deck construction.

b. There is parking available for a truck and 16-foot trailer during the installation process.

c. The laydown area is required to be fenced during the installation period.

d. There is enough room in the VA area to accommodate a 10’x20’ storage container.

e. There is storage area available at the VA facility for the patient lift motors.

f. A phased installation period shall be coordinated with the VA POC after contract award.

13. Contractor Conduct Requirements.

a. Equipment and supplies must not be shipped before designs are approved and contractor has approval to install. WTVAHCS cannot store uninstalled equipment for a significant length of time.

b. All employees and subcontractors must wear a visible name and company identification when onsite. When more than one person is onsite performing work under this contract, one must be designated as a supervisor and must be fully responsible for the work to be performed.

c. The Contractor shall provide all supplies, materials, equipment, qualified personnel, supervision, management, and transportation to perform all tasks as identified herein.

All work is to be performed in accordance with the guidelines established by Federal, State, and local ordinances and The Joint Commission. The Contractor’s procedures and quality control procedures shall conform to these guidelines.

d. Contractor personnel must conduct their work so as not to interfere with the normal functioning of the facility and will stop work if asked by the COR. Hours of work will need to be limited to daytime on units where patients sleep or to nighttime or weekends in areas where patients would be exposed by daytime work.

e. The Contractor shall take all precautions necessary to protect the lives and health of occupants of the building.

f. The Contractor must follow procedures required by WTVAHCS Infection Control personnel and the construction safety committee, to include any required dust control specified in the Infection Control Risk Assessment.

g. The Contractor must state what efforts will be made to minimize noise during installation of equipment.

h. The Contractor shall immediately correct any fire and safety deficiencies caused by his personnel. If the contractor fails or refuses to correct deficiencies promptly, the Contracting Officer may issue an order stopping all or any part of the work and may hold the contractor in default of the contract.

i. The Contractor shall comply with applicable Federal, State, and local safety and fire regulations and codes which are in effect at the beginning of the contract period. The contractor shall keep abreast of any changes in these regulations and codes applicable to the contract.

j. All material and equipment will be removed from the facility or stored properly at the end of each workday and secured during the workday in the areas specified by the

COR.

k. The Contractor is responsible for supplying, completing, and submitting all reports required or requested by Federal, State, or local ordinances which pertain to any duties contained in the contract.

l. The Contractor will be required to furnish the WTVAHCS with material safety data sheets for all chemicals used during installation. This information is required by the VA for emergency treatment in the event of ingestion of and/or contact with the material by humans and is required by OSHA regulations.

m. The Contractor’s onsite lead installer must attend a WTVAHCS construction safety orientation and supply job hazard analyses as required by Construction Safety Officer to show how hazards to Contractor and WTVAHCS personnel are being minimized.

n. All Contractor personnel must have OSHA 10-hour construction safety training cards.

o. The Contractor’s personnel shall follow applicable policies of the WTVAHCS, to include policies concerning fire and disaster preparedness programs.

p. The contractor shall furnish at his own expense all labor, materials, machinery, and appliances which may be necessary or appropriate in the performance of this contract.

q. All accumulated rubbish is to be collected and placed in the dumpster provided by the contractor.

14. Changes to Statement of Work. Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.

15. Reporting Requirements. The VHA checklist and any manufacturer’s checklist are the sole report and must be provided to the COR by 1 week after installation. Lift installation is not considered complete and cannot be paid for until the checklist is complete and provided to the COR.

16. Government Responsibilities.

a. Provide guidance for contractor site visits and information on which rooms need lifts.

b. Coordinate to allow for lift installation as rooms are available. (Patient Safety/COR in association with affected services.)

c. Provide technical information on the building and requirements for construction.

(Engineering)

d. Coordinate and schedule for training efforts. (Patient Safety/COR in association with affected services.)

e. Provide oversight of installation testing. (Patient Safety)

f. Coordinate for electrical outlet installation where necessary. (COR in association with

Engineering)

g. Answer Contractor questions during the purchasing and installation process. (CO and

COR.)

17. Government Holidays. The Government hereby provides notice and Contractor hereby acknowledges receipt that Government personnel observe the listed days as holidays:

18. Security Requirements.

a. Information System Officer, Information Protection: The contractor will not have access to VA Desktop computers.

b. Privacy Officer: During the services provided to the VA, if removal of equipment from the VA is required, any memory storage device will remain in VA control and will not be removed from VA custody. All research data available for Contractor analysis is de-identified. The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. Consistent with the requirements of 38 U.S.C. 5725, a contract may require access to sensitive personal information. If so, the contractor is liable to VA for liquidated damages in the event of a data breach or privacy incident involving any SPI the contractor/subcontractor processes or maintains under this contract. However, it is the policy of VA to forgo collection of liquidated damages in the event the contractor provides payment of actual damages in an amount determined to be adequate by the agency.

c. Training: Per VA Handbook 6500.6, Contract Security Appendix C, All contractor employees and subcontractor employees requiring access to VA information and VA information systems shall complete the following before being granted access to VA information and its systems:

(1) Sign and acknowledge (either manually or electronically) understanding of and responsibilities for compliance with the Contractor Rules of Behavior, Appendix E relating to access to VA information and information systems;

(2) Successfully complete the VA Cyber Security Awareness and Rules of Behavior training and annually complete required security training;

(3) Successfully complete the appropriate VA privacy training and annually complete required privacy training; and

(4) Successfully complete any additional cyber security or privacy training, as required for VA personnel with equivalent information system access [to be defined by the VA program official and provided to the contracting officer for inclusion in the solicitation document – e.g., any role-based information security training required in accordance with NIST Special Publication 800-16, Information Technology Security Training Requirements.]

(5) The contractor shall provide to the contracting officer and/or the COR a copy of the training certificates and certification of signing the Contractor Rules of Behavior for each applicable employee within 1 week of the initiation of the contract and annually thereafter, as required.

(6) Failure to complete the mandatory annual training and sign the Rules of Behavior annually, within the timeframe required, is grounds for suspension or termination of all physical or electronic access privileges and removal from work on the contract until such time as the training and documents are complete.

19. Records Manager.

a. Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228.

b. Contractor shall treat all deliverables under the contract as the property of the U.S.

Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest.

c. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records.

d. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act.

e. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract.

f. The Government Agency owns the rights to all data/records produced as part of this contract.

g. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data.

h. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.].

i. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules.

j. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
