The purpose of this amendment is to:

1) Post Questions and Answers to regarding the RFQ 36C25522Q0120 - 5 DRAWER PYXIS - Wichita VAMC.

Questions and Answers – Pyxis

Reference: FOB: Destination

Question: The Solicitation states FOB Destination but BD is required to charge for Shipping. Will the VA add a CLIN for Shipping? We Typically do not pay for shipping; however, you may include the shipping cost to the product cost, or you may include the shipping to installation if installation applies

Reference: 3. Scope of Work: The contractor shall provide all personnel, equipment, tools, materials, supervision, and non-personal services necessary to provide the installation service of the BD Pyxis Anesthesia Station ES and 12 months support.

Question: The Solicitation states Firm Fixed Pricing for Maintenance/Support but there is not a CLIN for Support. Does the VA plan to include a CLIN for a Base + 4 year Option for Support? No, this requirement is for commodities only, the 12 months warranty is required until service contract can be established.

Reference: Salient Characteristics for BD Pyxis Anesthesia Station ES

· Shall allow the nurse to narrow the profile list to the orders due during a specified time frame or a snapshot of their patients and their medication administration schedules.

· Shall provide lidded pockets that restrict access to all medications except one.

· In addition to the unit dose drawers, main unit of the station should contain locked & covered drawers.

· Shall alert or warn the user if a medication is being removed from the station outside of the timeframe allowed by hospital defined criteria.

· Inadvertent loading of unsafe medications within selected Pyxis MedStations.

Question: The above requirements are specific to Pyxis MedStation ES and not for the Pyxis Anesthesia System. Will the VA remove these requirements? Yes, remove salient characteristics not applicable to the anesthesia system.

Reference: 5.3. Other Requirements

a. Scheduling: Work shall be completed as coordinated with the site. May require after hours installation Monday through Friday, excluding federal holidays, to minimize impact to patient care.

Question: The BD project manager will create a mutually agreed upon project plan that will contain tasks directly related to installation of the contracted equipment. After hours work will require agreement from the BD project manager. (This item contradicts item 5.1) Is this acceptable to the customer? Yes, the Government will coordinate the installation with BD project as required for installation. No additional cost from the Government.

Reference: 5.3. Other Requirements

e. Documentation of Products to Be Used: The Contractor will be required to submit a material approval submittal of all supplies and material to be used in the project, as required in the submittal section of this scope. Any material safety data sheet, company specification, and brochure will be attached to the form. The form must be returned to the CFS approved prior of using the material or supplies. The CFS must comply with all comments, remarks and instructions attached to the form. A separate sequential logged must be maintained by the Contractor and COR to keep track of all submittals processed.

Question: BD does not provide supplies and materials as part of the implementation project that require MSDS. Is this acceptable to the customer? Yes, MSDS is not necessary if not applicable.

Reference: 5.3. Other Requirements, f.

bi. Safety: While in the performance of the work, the Contractors personnel shall abide by the safety requirements of the Medical Center. The Contractor shall always exercise extreme care in making the work area safe. The number one priority is to protect patients and hospital staff from any possible injury resulting from the equipment or materials. The Contractor is responsible for security of equipment each day. There is no storage area available on site. The hallways nor corridors shall not be used to store, pile or place construction materials or supplies. In the event of a situation requiring use of the corridor for such purposes, approval of the VA Medical Center Safety Manager/Officer shall be obtained previously. No painting material or supplies shall be placed where patients may recover them. Any safety violation or emergency situation should be reported to a hospital official immediately. The Contractor shall conform to all Federal, State and local regulations governing painting chemicals and equipment, which may be in effect. All equipment used to perform the work will conform to OSHA regulations.

Question: Once the Pyxis equipment is delivered to the customer site, it becomes the responsibility of the customer to provide a storage/staging area, and proper security for the equipment is the responsibility of the customer. Is this acceptable to the customer? Yes, the Government will provide storage for Pyxis equipment upon delivery until installation is scheduled and completed.

Reference: B.4 PRICE/COST SCHEDULE Question: BD’s standard ARO is 180 days. The BD project manager will create a mutually agreed upon project plan with the customer. The implementation project plan will contain key milestone dates for tasks required for a successful implementation. The BD team will then work towards completing tasks according to the project plan dates. Is this acceptable to the customer? Please include in your quote the proposed delivery/installation time frame/ARO.

Reference: Access to VA Information and VA Information Systems

c. Contract personnel who require access to national security programs must have a valid security clearance. National Industrial Security Program (NISP) was established by Executive Order 12829 to ensure that cleared U.S. defense industry contract personnel safeguard the classified information in their possession while performing work on contracts, programs, bids, or research and development efforts. The Department of Veterans Affairs does not have a Memorandum of Agreement with Defense Security Service (DSS). Verification of a Security Clearance must be processed through the Special Security Officer located in the Planning and National Security Service within the Office of Operations, Security, and Preparedness.

Question: Will the VA allow BD to leverage its already existing vetted IT-Level II NACL DoD workforce? The Security language applies as dictated by the VHA Handbook 6500.06 Appendix C.

Reference: Security Incident Investigation Question: Will the customer accept provided written report of any possible security incident within 10 business days per the executed BAA?

Reference: Liquidated Damages for Breach Question: Will the VA confirm that those damages constitute VA’s remedy for a data breach or privacy incident? The Security language applies as dictated by the VHA Handbook 6500.06 Appendix C.
