Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C25019AP4991

1. Contracting Activity: Department of Veterans Affairs, VISN 10, Network Contracting Office 10, Kettering, Ohio. The purchase request number is 552-19-3-121-0221.

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount: The Dayton VA Medical Center is in need of Twenty (20) Barrier-Free™ Exam Tables that include scale, recessed rollers, drawer heater, pelvic tilt, and electrical receptacle, and Twenty (20) Upholstery Tops, 28”. The estimated value of this procurement is , with an expected delivery date of 30 days ARO.

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used: Midmark has two models of barrier-free™ exam chairs. They are the only contractor found on the open market and on FSS schedules that can provide these exam tables that are barrier free and come with scale. The specific 626-003 exam table includes the following features that meet the requirements of the agency’s needs: Exam table with a built-in patient weigh scale at 650 lb. capacity, recessed rollers, drawer heater, pelvic tilt, and electrical receptacle. This exam table also includes automatic BMI calculations.

4. Description of market research conducted and results or statement why it was not conducted:

Market research was conducted on SAC, NAC, and GSA FSS schedules. These barrier exam tables were carried on VA NAC and GSA schedules by 1 SDVOSB and 1 VOSB sources;

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
