Services include providing all necessary equipment, supplies, labor, management, and transportation for home oxygen care. The solicitation will utilize firm-fixed pricing for an indefinite-delivery, indefinite-quantity contract with a one-year base period and four one-year options. The closing date for proposals was extended to May 11, 2018 at 5:00 PM EST due to the amendment. Additional details provided include page limit increases, clarification on submission requirements, and responses to offeror questions on evaluation criteria and past performance relevance thresholds.

Evaluation Factors and Submission Instructions
· 3. Proposal Format
· “Price Proposal NTE 179 pages”
· “Envelope No. 1 shall….If the Offeror can confirm that they have completed their annual representations and certifications electronically at http://www.sam.gov, then they can submit a copy with their proposal and 52.212-3 Offeror Representations and..”
Question:
1. May the Government please clarify if the offeror must submit the entire report from SAM, which currently has 54 pages, or may the offeror only submit the first page of the report, which reflects the Certification Validity period?
A. Government Response:
Yes, submit the entire report if the representations and certifications have been completed electronically.
“Offerors shall include supporting documentation (i.e. resumes, staff matrix, descriptive literature and facilities schematics) as part of the 300-page technical proposal.”
Question:
2. May the Government please clarify if the resumes and staff matrix required to be submitted within the technical proposal should only include the contractor’s staff entering the patient homes?
A. Government Response:
No, this includes all staff.
3. Seeing as though the Price Proposal includes a completed copy of the entire solicitation (around 298 pages), consider revising the page count requirement for the Price Proposal.
“EVALUATION FACTORS AND SUBMISSION INSTRUCTIONS., Offerors should ensure that each page provides identification of the submitting Offeror in the header or footer.”
Question:
4. Does this requirement pertain to each volume or just the Technical Volume?
A. Government Response:
This pertains to each volume.
“EVALUATION FACTORS AND SUBMISSION INSTRUCTIONS., Offerors shall include supporting documentation (i.e. resumes, staff matrix, descriptive literature and facilities schematics) as part of the 300-page technical proposal.”
Question:
5. To adequately display our employee resumes and credentials we will need many more pages than the 300-page limit. Consider revising to 500 pages.
A. Government Response:
The page limit has been increased to 500 pages for the technical proposal.
“EVALUATION FACTORS AND SUBMISSION INSTRUCTIONS., Redacting of Hard & CD Copies.”
Question:
6. Please clarify that the redaction pertains to every volume for this proposal.
A. Government Response:
Yes, as specified in Envelope 1 and Envelope 2 sections, all hard copies and one (1) CD shall be redacted. Only one (1) CD/ Electronic copy is un-redacted.
“Factor 3: PAST PERFORMANCE CRITERIA:, Past Performance Evaluation,
0. (b). The Government will evaluate the offeror’s ability to successfully perform the service based on demonstrated past and present experience under recent and relevant contracts. Relevancy is defined as contracts performed in the last three (3) years involving:
0. • Experience with Home Oxygen Service contracts
0. • Demonstrated management of experienced/qualified professional staff and administrative support personnel
0. • Demonstrated ability providing home oxygen services in a large geographical area with at least 5,500 patients.
(c). Recent past performance consists of contracts, completed or in progress, performed for Federal agencies and commercial customers within the last three (3) years from date of issuance of this solicitation. The purpose of the past performance evaluation is to allow the government to assess the offeror’s ability to perform the effort described in this RFP, based on the offeror’s demonstrated present and past performance. The assessment process will result in an overall performance confidence assessment in accordance with the ratings defined below. Offerors with no relevant past or present performance history or the offeror’s performance record is so limited that no confidence assessment rating can be reasonably assigned shall receive the rating "Unknown Confidence," meaning the rating is treated neither favorably nor unfavorably.”
Questions:
7. Based on the above highlighted areas, in order for past performance to be evaluated as relevant, references must be for a large geographical area with at least 5,500 patients. The term “at least” is basically saying any number of patients less than that is not relevant, even if it’s 5,499. I encourage the VA to revise this language to “Demonstrated ability providing home oxygen services in a large geographical area with at least 3,000 patients” A. Government Response:
The language will remain unchanged.
“You state: The assessment process will result in an overall performance confidence assessment in accordance with the ratings defined below. No ratings were provided in the solicitation. “
8. Please provide the ratings.
A. Government Response:
This is a typo and the sentence has been removed.
“You state: Offerors with no relevant past or present performance history or the offeror’s performance record is so limited that no confidence assessment rating can be reasonably assigned shall receive the rating "Unknown Confidence," meaning the rating is treated neither favorably nor unfavorably. Since the requirement is to provide a minimum of 3 past performance references, and contractors don’t provide that, then the only confidence rating should be the lowest rating, not unknown. “
9. Consider revising.
A. Government Response:
The language remains unchanged. It explains that the rating will not reflect favorable or unfavorable against the Offer.
“ENVELOPE NO. 1 – PRICING INFORMATION & VETERANS INVOLVEMENT
Envelope No. 1 shall contain two (2) hard copies redacted, one (1) electronic copy (CD) redacted and one (1) un-redacted electronic copy of the Pricing proposal. Provide a complete copy of the RFP issued herein (this document) including the completion of the representations and certifications at FAR 52.212-3 and FAR 52.209-5 (Deviation) and 52.209-7 in the Addendum to 52.212-1 and Veterans Involvement Information.”
Question:
10. May the government please clarify if the above should be interpreted as followed:
· 2 hard copies redacted to include Pricing Information and Veterans Involvement along with responses for all Factors, 1 through 5.
· 1 CD redacted with only Pricing Proposal
· 1 CD unredacted with only Pricing Proposal
A. Government Response:
Please review sections Envelope No. 1 and Envelope No. 2 in the “Evaluation Factors and Submission Instructions”. Envelope No. 1 states the requirements for the Price proposal and Veterans Involvement which two (2) redacted hard copies, one (1) redacted electronic copy and one (1) un-redacted electronic copy.
Envelope No. 2 states the requirements for the Technical proposal, which six (6) redacted hard copies, one (1) redacted electronic copy (CD) and one (1) un-redacted electronic copy.
