This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

Solicitation number is 36C24820Q0139 and is issued as a Request for Quote (RFQ). This solicitation documents and incorporates provisions and clauses in effect through Federal Acquisition Circular 2020-01, effective 12 November 2019. This solicitation starts 21 Nov 2019 and will end 9 Dec 2019 at 10:00 am EST.

North Florida South Georgia Healthcare System, Network Contracting Office 8, Regional Procurement Office - East (RPO-E) intends to award a Firm Fixed Price service contract for Annual Maintenance and Inspection of Boilers located at the Malcom Randall VAMC in Gainesville, FL (3 boilers) and the Lake City VAMC in Lake City, FL (3 Boilers). This procurement is being conducted under FAR Subpart 13 and simplified acquisition procedures apply. Period of performance is 16 December 2019 – 15 December 2020. The is a base year with four (4) one year options requirement. The North American Industrial Classification System (NAICS) code for this procurement is 811310 with a small business size standard of $8 million.

This procurement is set-aside 100% for Service Disabled Veteran Owned Small Business (SDVOSB). If award cannot be made, the solicitation will be cancelled; and the requirement resolicited.

This solicitation will use Comparative Evaluation. Award will be made to the offeror determined to be acceptable, price reasonable, and determined is determined to be best value to the government; all eligible offerors may submit quotes. All offerors shall be registered in the System for Award Management (SAM) at https://www.sam.gov ; and www.vip.vetbiz.va.gov , if applicable, to be considered for an award of a federal contract. The record shall be complete and not missing elements such as representations and certifications; and SDVOSB shall be certified.

Contractors interested in providing these services should submit their quotes via email to Reginald.Nettles@va.gov . Quotes shall be received 09 Dec 2019 at 10:00 AM Eastern Time Zone and reference the solicitation number. Please direct all questions regarding this procurement to Reginald L. Wage Determination. The Wage Determination for this requirement will cover the following counties: Alachua and Columbia. Wage determination can be extracted from the Department of Labor at https://beta.sam.gov.

Contractor Request for Information (RFI). All questions regarding this RFQ shall be in writing and may be sent by e‐mail to reginald.nettles@va.gov NLT 2 Dec 2019 by 11:30am EST. No further questions will be accepted after that date and time. Answers to contractor questions will be posted on FBO no later than 3 Dec 2019 by 4:30 PM Eastern.

Quote Submission. Contractors interested in providing these services should submit their quotes via email to Reginald.Nettles@va.gov . Quotes shall be received NLT 9 Dec 2019 at 10:00 AM Eastern Time Zone and reference the solicitation number. Quotes received after this date and time will be considered late in accordance with FAR Provision 52.212-1(f) and will not be evaluated unless the Contracting Officer determines the late quote will be accepted IAW 52.212-1(f)(2)(i)(A-C).

The Government will award a Firm Fixed-Price contract resulting from this RFQ to the responsible contractor whose quote is determined to be acceptable, price reasonable, and determined to be best value to the government in accordance with a Comparative Evaluation of the information requested in this solicitation at FAR Provision 52.212-2.

SCHEDULE OF SERVICES

The North Florida South Georgia Healthcare system requires Annual Maintenance and Inspection for Boilers located in Gainesville, FL and Lake City, FL. The contractor shall furnish all labor, materials, parts, supervisions, services and incidentals to Annual Maintenance and Inspection of Boilers located at the Malcom Randall VAMC in Gainesville, FL (3 boilers) and the Lake City VAMC in Lake City, FL (3 Boilers) as described in the attached Statement of Work (SOW). Contractors shall quote prices for each Contract Line Item Numbers (CLIN) for a Base year and 4 one year options. See Attached Price Schedule for quote submission

QUOTATION SUBMISSION

Offerors shall submit quotes electronically by the date and time in accordance with the Request for Quote instructions. Quote shall be no more than 5 megabytes or exceed the limits to be submitted electronically via email.

52.212-2 Evaluation-Commercial Items (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other criteria considered. At a minimum, the following criteria shall be included for evaluation of offers:

Failure to provide ALL requested items is reason to be determined unresponsive. Evaluation is based on a Comparative Evaluation of ALL Criteria.

1) Technical.

a) Provide methodology to be used to internally and externally inspect and test boiler safety devices on the six boilers that provide steam to the Malcom Randall VA Medical Center, Gainesville, FL and the Lake City VA Medical Center, Lake City, FL.

b) Provide a list of personnel assigned to perform the titled service.

c) Provide copies of Training Certification of personnel identified for boiler inspection and testing.

2) Past Performance

a) Provide three (3) references of same scope and cost of work requested.

3) Price.

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 14 days.
NOTE: The specified rates under this clause will be those rates in effect under the contract each time an option is exercised under this clause.
52.217-9Option to Extend the Term of The Contract (Mar 20000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 14 days provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 14 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5yrs, 6mos.
52.219-14Limitations on Subcontracting (Jan 2017)

(a) This clause does not apply to the unrestricted portion of a partial set-aside.

(b) Applicability. This clause applies only to-

(1) Contracts that have been set aside or reserved for small business concerns or 8(a) participants;

(2) Part or parts of a multiple-award contract that have been set aside for small business concerns or 8(a) participants; and

(3) Orders set aside for small business or 8(a) participants under multiple-award contracts as described in 8.405-5 and 16.505(b)(2)(i)(F).

(c) By submission of an offer and execution of a contract, the Offeror/Contractor agrees that in performance of the contract in the case of a contract for-

(1) Services (except construction). At least 50 percent of the cost of contract performance incurred for personnel shall be expended for employees of the concern.

(2) Supplies (other than procurement from a nonmanufacturer of such supplies). The concern shall perform work for at least 50 percent of the cost of manufacturing the supplies, not including the cost of materials.

(3) General construction. The concern will perform at least 15 percent of the cost of the contract, not including the cost of materials, with its own employees.

(4) Construction by special trade contractors. The concern will perform at least 25 percent of the cost of the contract, not including the cost of materials, with its own employees.
